Tibiotalocalcaneal Arthrodesis: Deformity Correction in Medically Comorbid Patients.

BACKGROUND: Tibiotalocalcaneal (TTC) arthrodesis is a common orthopaedic procedure used to manage end-stage pathologies of the ankle and hindfoot. A variety of surgical techniques exist to accomplish TTC arthrodesis. We retrospectively reviewed and compared postoperative outcomes of 2 different TTC arthrodesis constructs, including intramedullary nail (IMN) and open plating (OP) with a plate/screw construct, in high-risk patients with a wide range of deformity and comorbidities. METHODS: A retrospective review was conducted on 73 ankles of 69 consecutive patients who underwent TTC arthrodesis

Impact of Preoperative Heel Ulceration on Surgical Outcomes Following Open Reduction and Internal Fixation of Rotational Ankle Fractures.

BACKGROUND: As surgical treatment with open reduction and internal fixation (ORIF) becomes more common, attention to preoperative risk stratification has grown. Although multiple modifiable risk factors and comorbidities have been associated with adverse outcomes, the impact of preexisting decubitus heel ulcers in this context remains yet to be defined. METHODS: Using a nationally representative multi-institutional database, a retrospective analysis was conducted on adults undergoing ORIF for rotational ankle fractures between 2010 and 2022. Patients with a documented decubitus heel ulcer wit

Characterization and Evaluation of P Solubilizing Bacteria from Forest Soils for Plant Growth Promotion.

OBJECTIVE: To assess the usability and acceptability of nurse-led telemonitoring in managing gestational diabetes among Nepalese women. METHODS: We conducted a convergent mixed-method study among 91 pregnant women diagnosed with gestational diabetes at Dhulikhel Hospital, Nepal. Participants received glucometers and blood pressure monitors, along with training and instructions to measure and record their blood pressure and glucose levels at home once a week. Starting from the 28th gestational week, the study nurse reviewed measurements obtained at home during the biweekly telemonitoring follo

Open versus percutaneous fibular graft harvesting in long bone nonunion surgery: A comparative cohort study.

AIMS: The traditional open harvesting method used in fibular grafting is a well-established reconstructive technique, but it is associated with donor-site morbidity. However, percutaneous harvesting has been introduced as a minimally invasive method to reduce trauma while preserving graft adequacy. METHODS: A retrospective comparative analysis was conducted to measure the outcomes of percutaneous versus open fibular graft harvesting. Studied variables included patient demographics, intraoperative parameters (harvest time, estimated blood loss, incision length), and postoperative outcomes (pai

Reducing time from presentation to diagnosis of scaphoid fractures with cone beam CT: a before-and-after study.

BACKGROUND: Scaphoid fractures comprise approximately 50-70% of carpal bone fractures but can be difficult to detect on initial plain film radiographs. A delayed diagnosis can lead to a high rate of non-union, avascular necrosis and Complex Regional Pain Syndrome. Current literature supports cone beam CT (CBCT) (within 10-14 days) as an effective method for diagnosing scaphoid fractures. We implemented an early outpatient CBCT pathway, prior to specialist review, with the aim to increase the proportion of patients with suspected scaphoid fracture undergoing CBCT within 7 days. METHODS: We des

Pediatric Medial Epicondyle Fractures: To Fix or Not To Fix?

Pediatric medial epicondyle fractures are common elbow injuries that represent an avulsion fracture of the apophysis. These fractures are commonly associated with elbow dislocation, and as such, acknowledging the soft-tissue injury and resultant potential elbow instability is important. Treatment decision making is controversial because few absolute indications for surgery exist, and no true displacement cutoffs have been settled despite plentiful research. Although fracture incarceration in the joint, open fractures, ulnar nerve entrapment, and frank elbow instability are indications for surg

Contrast-Enhanced Ultrasound (CEUS) Reveals Perfusion of Human Bone in Acute Fracture Healing: A Pilot Study.

Bone fractures are common injuries with reported non-union rates of up to 9%. Current treatments for non-union include surgery with complication rates of up to 17% and significant costs. Microbubbles are used clinically in ultrasonography as contrast agents and have been shown to deliver therapeutics to desired locations by noninvasive stimulation of cavitation using extracorporeal ultrasound in preclinical studies. Contrast enhanced ultrasound (CEUS) has been used to determine the cause of established fracture non-unions, though this is not in widespread use clinically. This pilot study aimed

Radiological outcome of early surgical fixation versus cast immobilization for adults with a scaphoid waist fracture: five-year follow-up of the Scaphoid Waist Internal Fixation for Fractures Trial.

AIMS: In the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT), surgical fixation was compared with cast immobilization, with the primary endpoint being the outcomes at one year. The aim of the current study was to assess the radiological outcomes (union and the development of osteoarthritis (OA)) of the two forms of treatment at five years. METHODS: Patients who remained in the trial at five years after randomization were invited to have plain radiographs and a CT scan of the injured wrist, and a posterior-anterior radiograph of the contralateral wrist. This imaging was reviewed
