AIMS: The conclusion of the one-year analysis of the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) was that initial cast immobilization with surgical fixation for nonunion was the optimal treatment for patients with a fracture of the waist of the scaphoid. However, there remained significant uncertainty about the medium-term outcomes and how these could influence the patient's health-related quality of life (HRQoL), care requirements, and costs. The aim of this study was to explore how additional data from the five-year follow-up influenced the cost-effectiveness of the treatme

AIMS: Total elbow arthroplasty (TEA) has shown favourable outcomes in the management of severely comminuted distal humeral fractures in low-demand elderly patients, leading to its increased use in recent years. Less is known about the outcomes of TEA when used for the failure of either internal fixation or conservative management, particularly whether not performing TEA acutely influences the options for further management if fixation fails. This systematic review aimed to assess the clinical and functional outcomes following TEA when used for the post-traumatic sequelae of a distal humeral fr

AIMS: In the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT), surgical fixation was compared with cast immobilization using the Patient-Rated Wrist Evaluation (PRWE) score at one year as the primary endpoint. The aim of this longer-term study was to assess the clinical effectiveness of the two treatment pathways at a five-year follow-up.
METHODS: Patients who had not withdrawn from the trial were invited for a clinical assessment at five years after randomization and asked to complete a patient-reported questionnaire in the clinic, or by post or telephone. The PRWE was collected

The failure cases of peritrochanteric fractures are occasionally encountered due to poor bone stock. For proximal humeral fractures, a strategy using a segment of the fibular allograft (FA) placed endosteally was introduced. Several studies have demonstrated favorable outcomes with FA. Inspired by this, a fixation strategy using FA with a locking plate was performed in patients with unfavorable bone stock conditions. Ten patients with poor bone stock and nonunion of peritrochanteric fractures were treated with FA and a locking plate at a tertiary hospital between 2016 and 2022. We analyzed the

PURPOSE OF THE STUDY: Fractures of the calcaneus are always very serious injuries that, due to their nature or when inadequately treated, can have permanent consequences. Minimally invasive methods, such as the C-nail, help reduce both intraoperative and postoperative risks. The aim of the study is a retrospective evaluation of a group of 260 patients with 280 calcaneus fractures treated surgically with the C-nail in the period from 2011 to 2023. The hypotheses of a lower incidence of postoperative infection, a lower risk of sural nerve injury, and a higher AOFAS functional score need to be co

A key decision in orthopaedic surgery is whether to leave Kirschner wires (K-wires) protruding through the skin or to cut and bury them subcutaneously. This choice may affect infection risk, need for secondary procedures, patient comfort and cost. However, the evidence comparing buried versus exposed K-wires has been conflicting, leaving surgeons with no clear consensus. We performed a meta-analysis to quantitatively compare outcomes of buried versus exposed K-wires in phalangeal, metacarpal and distal radius fractures. We followed the Preferred Reporting Items for Systematic Reviews and Meta-

BACKGROUND: Quiescent non-union refers to a state where a fracture fails to heal, and the site appears clinically inactive despite the possible presence of a dormant or low-grade infection. This condition often follows open fractures or prior surgical interventions. Although typical signs of infection-pain, swelling, or discharge-are absent, latent infections may reactivate, leading to severe complications. Diagnosing infection in such cases is challenging due to the lack of definitive tools, and clinicians typically rely on inflammatory markers like C-reactive protein (CRP), erythrocyte sedim

Background Femoral neck fractures are common in elderly patients due to osteoporosis and falls. A significant complication is avascular necrosis (AVN), leading to bone tissue death and joint dysfunction. The Femoral Neck System (FNS) was developed to provide minimally invasive fixation with improved rotational and angular stability, aiming to reduce AVN risk; however, clinically significant complications still occur. Hence, this study aimed to assess complication and reoperation rates following femoral neck fracture fixation using FNS in a major trauma centre, and to compare these findings wit

INTRODUCTION: Volar locking plate fixation with cancellous bone grafting is an effective treatment for scaphoid non-union. However, the plate can impinge on the volar rim of the radius, causing pain or restricted movement that often necessitates plate removal. The primary aim of this study was to evaluate the effect of hardware removal on wrist function and patient-reported outcomes after plate fixation for scaphoid non-union.
METHODS: This prospective study assessed 49 of 113 patients who underwent plate fixation for scaphoid non-union and later required plate removal due to functional impai

INTRODUCTION: Distal tibial comminuted fractures are usually a result of high-energy trauma. The subcutaneous location of the bone pre-disposes to open fractures and decreased vascular supply. All these local and biological factors contribute to increased risk of non-union. Mechanical factors such as stability of the fixation and method of fixation also add to it. Hence, it is prudent to evaluate all these factors in a case of distal tibial non-union and choose optimal method of management of distal tibia non-union. This case presentation is portrayal of one such scenario.

INTRODUCTION: Complex intra-articular proximal tibia fractures (Schatzker types V and VI) are difficult to manage due to articular involvement, malalignment, and soft-tissue compromise. Conventional plating techniques carry risks of infection and soft-tissue morbidity. Suprapatellar intramedullary nailing (IMN), combined with condylar screws, offers a minimally invasive option with potential biomechanical and clinical advantages.
MATERIALS AND METHODS: A prospective case series was conducted at a tertiary trauma center in Chennai, India, including 10 consecutive adults with Schatzker type V (
