Rebuilding the pelvis: advances in robotic-assisted management of complex pelvic fractures.

Complex pelvic fractures are infamously challenging to fix surgically because of their fine anatomy and proximity to vital neurovascular structures. Traditional open reduction and internal fixation (ORIF) improves stability but is complicated by excessive blood loss, longer operative time, and morbidity. Robotic-assisted surgical methods, i.e. Robot-Assisted Fracture Reduction (RAFR) and the TiRobot platform, provide a paradigm shift toward precise, minimally invasive fracture reduction and fixation. The RAFR system blends preoperative high-definition 3D CT imaging with intraoperative cone-bea

APEX1, a transcriptional hub for endochondral ossification and fracture repair.

After injury, bone tissue initiates a reparative response to restore its structure and function. The failure to initiate or delay this response could result in fracture nonunion. The molecular mechanisms underlying the occurrence of fracture nonunion are not yet established. We propose that hypoxia-triggered signaling pathways, mediated by reactive oxygen species (ROS) homeostasis, control Bmp2 expression and fracture healing initiation. The excessive ROS leads to oxidative stress and, ultimately, fracture nonunion. In this study, we silenced Apex1, the final ROS signaling transducer that medi

Primary Versus Salvage Distal Femoral Endoprosthetic Replacement Following Native Distal Femur Fracture: A Systematic Review and Meta-Analysis.

BACKGROUND: Surgical management of native distal femoral fractures (DFFs) in elderly patients includes open reduction and internal fixation (ORIF) or distal femoral endoprosthetic replacement (DFR). When ORIF is complicated by nonunion or fixation failure, salvage DFR (sDFR) may be required. The comparative outcomes of primary DFR (pDFR) vs sDFR remain unclear. This systematic review and meta-analysis aimed to assess the quality of published literature and compared clinical and functional outcomes between pDFR and sDFR for native DFFs. METHODS: MEDLINE, Embase, and Cochrane databases were sea

Bone-targeted mitochondrial delivery via magnetic-temperature responsive artificial cells for repairing age-related fractures.

BACKGROUND: Age-related bone diseases, such as osteoporosis and degenerative joint disorders, pose a significant global health challenge, leading to over 9 million fractures annually, which not only diminishes quality of life but also imposes a substantial socioeconomic burden on healthcare systems. A major clinical obstacle in the aging population is the significantly reduced regenerative capacity of bone, often resulting in delayed fracture healing or nonunion fractures. Mitochondria, as the central regulators of cellular energy metabolism, are essential for determining cell fate and support

Cannabis and nicotine use are independently associated with adverse surgical, medical, and psychosocial outcomes following upper extremity fracture fixation.

BACKGROUND: Marijuana use is rising in the United States, yet its impact on perioperative outcomes remains poorly understood, particularly in orthopaedic trauma where cessation is often not feasible. This study evaluates the risks associated with cannabis and nicotine use in patients undergoing fixation of upper extremity fractures. METHODS: We performed a retrospective analysis of adult trauma patients with upper extremity fractures (2015-2023) identified using CPT codes for surgical fixation in the TriNetX database. Four cohorts were defined: cannabis-only users (n = 801), nicotine-only use

Multifunctional polyoxomolybdate cluster loaded into hydrogel for augmented bone regeneration through synergistic immunomodulation and osteogenesis.

Fracture nonunion or delayed union presents a significant challenge in orthopedic practice. Bone healing is a complex physiological process that initiates with the modulation of inflammatory immunity and progresses through critical stages, including angiogenesis, osteogenic differentiation, and biomineralization. The intrinsic link among immune homeostasis, bacterial clearance, and osteogenic microenvironments underscores the need for an integrated therapeutic strategy. To address these challenges, we developed a multifunctional molybdenum-based polyoxometalate cluster (Mo-POM) modified with g

Nobiletin promotes fracture healing by modulating macrophage polarization: a review.

Fracture healing is a complex process in which various cells, including macrophages, are involved. M1 macrophages and the cytokines they secrete exacerbate local inflammation and inhibit fracture healing, whereas M2 macrophages inhibit inflammation and promote tissue repair. An imbalance in the ratio of M1 to M2 macrophages leads to delayed healing and nonunion of the fracture. Nobiletin has received increasing attention in promoting fracture healing. Thus, the aim of this review article was to investigate the effects of nobiletin on macrophages during fracture healing, with a major focus on t

IL-6 blockade at the fracture site accelerates bone healing via inflammatory modulation of sensory nerve CGRP signaling.

BACKGROUND: Delayed fracture healing and nonunion are commonly associated with impaired callus remodeling and dysregulated interleukin-6 (IL-6) signaling; however, the precise role of IL-6 in fracture repair remains controversial. Calcitonin gene-related peptide (CGRP), a sensory neuropeptide expressed at fracture sites, has been shown to suppress IL-6 expression and promote bone regeneration. Nevertheless, whether IL-6 reciprocally regulates CGRP expression or function, thereby influencing fracture healing, remains unclear. This study aimed to determine whether local inhibition of IL-6 enhanc

Plate-tensioned reconstruction nail for proximal femur diaphyseal nonunion.

A "plate-tensioned nail" implant construct is described as a single-stage salvage procedure for proximal femur diaphyseal nonunion. This construct combines the stability of a medullary nail with the compression afforded by a tension band, permitting immediate full weight bearing. The proximal hole of a 4.5-mm plate is modified to accept a 6.5-mm recon screw placed through the plate and nail into the femoral neck and head. Plate-based compression or an articulated tensioning device is used to compress across the nonunion site while affixing the plate to the lateral femoral diaphysis. Eight adul

Treatment of Displaced Femoral Neck Fractures in Children: Mid-Term Outcomes and Predictors of Failure.

BACKGROUND: Displaced femoral neck fractures in children are rare but carry significant risk for complications such as avascular necrosis (AVN), nonunion, and malunion. Although adult literature increasingly supports closed reduction when adequate alignment is achieved, the pediatric literature lacks consensus on optimal management. This study evaluates whether the quality of reduction, regardless of open or closed technique, predicts adverse outcomes in pediatric displaced femoral neck fractures. METHODS: We retrospectively reviewed 56 patients (ages 2 to 18) with displaced Delbet type II or

A Novel Classification System for Fifth Metatarsal Base Fractures Is Reliable and Clinically Validated.

BACKGROUND: One goal of classifying fractures at the base of the fifth metatarsal is to reliably identify which fracture patterns have an increased risk of delayed union and nonunion and therefore merit consideration for operative fixation. The most common classification system of proximal fifth metatarsal fractures is the three-part Lawrence and Botte scheme, which has been shown to have poor interobserver and intraobserver reliability and can be ambiguous in terms of clinical treatment and prognosis. In particular, the assignment of the middle classification (Type II) has unclear implication

Enhanced Bone Formation in Segmental Defect Healing Using 3D Printed Scaffolds Containing Bone Marrow Stromal Cells and Small Molecules Targeting Chondrogenesis and Osteogenesis.

Background/Objectives: Nonunion bone healing results from a critical size defect that fails to bridge a bone injury to produce bony union. Novel approaches are critical for refining therapy in clinically challenging bone injuries, but the complex and coordinated nature of fracture callus tissue development requires study outside of the simple closed murine fracture model. Methods: We have utilized a three-dimensional printing approach to develop a scaffold construct with layers designed to sequentially release small molecule therapy within the tissues of a murine endochondral segmental defect
