Nocturia ≥ 3/Night and 5-Year Fragility Fracture Risk in Older Urology Outpatients.

AIM: To evaluate whether nocturia ≥ 3/night stratifies 5-year fragility-fracture risk in older urology outpatients and to explore its prognostic performance alongside the N-QoL and IPSS. METHODS: We conducted a retrospective cohort study of urology outpatients aged ≥ 65 years at Hachinohe City Hospital, Japan, between 2015 and 2024. After exclusion of patients with prior fragility fracture, bone metastasis, or pathological fracture, and application of a 3-month landmark, 2290 participants were included. The primary outcome was first non-pathological fragility fracture. Bone metastasis and dea

Non-union of Bone Plating in a Mandibular Fracture With Osteomyelitis: A Case Report.

Mandibular fractures are a common occurrence, often associated with complex complications such as non-union and osteomyelitis. Non-union, the failure of a fracture to heal, can be particularly challenging in the mandible due to the limited soft tissue coverage and compromised blood supply in atrophic areas. Additionally, the presence of osteomyelitis, a bone infection, further complicates the healing process and increases the risk of implant failure. This case report aims to provide an overview of the management strategies for non-union of bone plating with mandibular fracture associated with

[Sternum fracture and troponinism after forced inspiration].

Elevated troponin is frequently interpreted as acute coronary syndrome, yet it may reflect non-ischemic myocardial injury. We report a 79-year-old woman presenting with acute dyspnea and chest pain following forced inspiration, found to have a spontaneous sternal fracture and marked troponin elevation without ischemic ECG changes or significant ventricular dysfunction. The case highlights the diagnostic challenge of distinguishing non-STEMI from secondary troponin elevation in the setting of chest wall trauma and emphasises the need for careful clinical assessment in elderly patients.

Locally manufactured versus imported proximal femoral nails: inverse probability of treatment weighting: adjusted comparison of costs and complications in intertrochanteric femur fractures.

OBJECTIVE: To compare domestic and imported proximal femoral nails (PFNs) in the treatment of intertrochanteric femoral fractures with respect to implant cost trends and complication rates. METHODS: This retrospective cohort study included adults aged 65 years and older who underwent proximal femoral nailing for intertrochanteric femoral fractures. Fractures were identified using ICD-10 code S72.1 and AO/OTA types 31A1 and 31A2. All patients were treated between January 2019 and June 2024 at a tertiary academic trauma center. Patients with pathological fractures, prior ipsilateral hip surgery

Case Report: Primary epithelioid angiosarcoma of the femur: two rare cases with atypical skeletal involvement.

BACKGROUND: Primary epithelioid angiosarcoma (EA) is an exceptionally rare malignant vascular tumor with aggressive behavior and nonspecific imaging features. Femoral involvement is particularly uncommon and may mimic other destructive bone neoplasms. CASE REPORT: We describe two men with primary EA of the femur. One patient presented with progressive right hip pain and a pathological fracture of the proximal femur, whereas the other presented with progressively worsening pain in the distal left thigh. Imaging demonstrated destructive osteolytic lesions with intramedullary involvement and var

Orthopaedic Management of Bone Metastases from Lung Cancer.

BACKGROUND/OBJECTIVES: Bone metastases from lung cancer can cause pain, pathological fracture, spinal instability, metastatic spinal cord compression and loss of independence. Lung cancer-specific evidence for orthopaedically relevant local management remains fragmented. This review assessed outcomes across spinal, appendicular and pelvic skeletal disease, while explicitly examining the anatomical distribution and limitations of the evidence. METHODS: PubMed, Web of Science, Europe PMC and Scopus were searched for original studies published from January 2015 to December 2025. Eligible studies

Managing Rebound Bone Loss After Denosumab Discontinuation: Mechanisms, Risk Stratification, and Sequential Therapy.

Denosumab is a fully human monoclonal antibody targets receptor activator of nuclear factor kappa-Β ligand (RANKL), potently suppresses bone resorption, increases bone mineral density (BMD), and reduces vertebral, hip, and non-vertebral fracture risk. However, its pharmacokinetic reversibility renders discontinuation clinically hazardous. This narrative review synthesizes current evidence on the mechanisms, clinical consequences, and risk determinants of rebound bone loss following denosumab discontinuation and critically evaluates available preventive strategies. Upon denosumab cessation, rap

Biomechanical analysis of T10-T11 vertebrae under the combined effects of osteoporosis and L4 spondylolisthesis: a finite element analysis based on computed tomography.

AIMS: Osteoporotic vertebral compression fractures are common; however, the biomechanical link between L4 spondylolisthesis and non-adjacent thoracic fractures remains unclear. This study used 3D finite element (FE) analysis to assess the biomechanical effects of L4 spondylolisthesis and osteoporosis on T10-T11. METHODS: Four FE models of T10-L5 were reconstructed from CT images of a healthy adult male: normal (A), isolated spondylolisthesis (B), isolated osteoporosis (C), and combined (D). A 500-N axial load and 10-N·m moments simulated six motions. Maximum von Mises stresses of whole verteb

Low-Volume Superior Trunk and Superficial Cervical Plexus Block With Sedation as the Sole Regional Anesthetic for Proximal Humerus Fracture Fixation in a Patient With Alzheimer Disease.

BACKGROUND Proximal humerus open reduction and internal fixation (ORIF) is usually performed under interscalene brachial plexus block (ISB) or general anesthesia (GA). ISB causes ipsilateral phrenic nerve paresis in nearly all patients, and GA may raise the risk of postoperative neurocognitive disorders in elderly patients with cognitive impairment. We report the feasibility of a low-volume regional technique, used as the sole regional anesthetic with sedation, in a patient in whom GA avoidance was desirable. CASE REPORT A 68-year-old woman (American Society of Anesthesiologists physical statu

Retrograde Intramedullary Nailing Versus Locking Plate Fixation in Extra-Articular Distal Femur Fractures: A Prospective Study.

INTRODUCTION: Distal femur fractures require stable fixation that permits restoration of alignment and early functional recovery. Retrograde intramedullary nailing distal femur nailing (DFN) and distal femoral locking plate fixation locking compression plate (LCP) are commonly used for extra-articular fractures, but their comparative functional and radiological outcomes remain debated. AIM: This study compared operative, functional, radiological, and complication outcomes of DFN and LCP in adult patients with extra-articular distal femur fractures. MATERIALS AND METHODS: This prospective stu

Severe skeletal fragility in SCN8A-related developmental and epileptic encephalopathy.

In this child with a phenotype compatible with the SCN8A gain-of-function spectrum, more than 20 bone fractures were documented. Together with previously reported cases, these observations raise the possibility that skeletal fragility may represent an underrecognized feature of the clinical spectrum associated with pathogenic SCN8A variants. Experimental and clinical findings provide biological plausibility for altered bone remodeling, while the marked reduction in fracture burden following pamidronate therapy underscores the importance of early recognition and management of skeletal complicat

"Single-Centre Audit of Bone-Modifying Agents in Neuroendocrine Neoplasm Bone Metastases".

Bone metastases in neuroendocrine neoplasms are uncommon but cause significant morbidity. A recent Italian survey of 320 patients with neuroendocrine neoplasms and bone metastases reported skeletal-related events in 23% and bone-targeted agent use in 31%, highlighting limited evidence for bone-modifying agents. We identified adults with confirmed or clinically suspected neuroendocrine bone involvement who received a bone-modifying agent at a UK tertiary centre (January 2019-September 2024), using institutional prescribing records. Demographic, tumour, treatment, and toxicity data were extracte

Early Ambulation Accelerates Time to First Bowel Movement After Separation Surgery for Spinal Tumors.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To identify factors associated with the timing of bowel function recovery after separation surgery for metastatic spinal tumors, while incorporating the temporal dimension of postoperative rehabilitation. SUMMARY OF BACKGROUND DATA: Separation surgery for metastatic spinal tumors is more extensive compared to conventional spinal surgery and carries a higher risk of postoperative ileus (POI). However, the factors influencing bowel recovery following this procedure remain poorly understood. Prior studies on conventional spinal decompression

Osteoporosis risk in patients with primary open-angle glaucoma using muscarinic and prostaglandin agonists: A retrospective cohort study.

BackgroundGlaucoma is the leading cause of irreversible blindness worldwide, with primary open-angle glaucoma (POAG) arising as the most common subtype. Recent studies have shown that muscarinic and prostaglandin pathways can modulate bone remodeling under systemic, context-dependent conditions. M3 muscarinic receptor activation has been linked to increased bone formation and decreased resorption, whereas continuous prostaglandin E2 (PGE2) administration inhibits bone resorption, with intermittent use inducing bone formation. Given the minimal systemic findings of topical ophthalmic medication

Pathological fractures revealing Langerhans cell histiocytosis in an elderly female: a rare case report.

INTRODUCTION AND IMPORTANCE: Langerhans cell histiocytosis (LCH) is a rare clonal proliferative disorder of dendritic cells, predominantly affecting children. Adult-onset LCH is uncommon and often misdiagnosed due to its atypical presentation. Pathological fractures as the initial manifestation are particularly rare in older adults. CASE PRESENTATION: We report a 63-year-old female who sustained a pathological femoral fracture following minor trauma, with radiographs revealing cortical thinning and lytic changes. Subsequent imaging demonstrated multifocal osseous lesions involving the pelvis,

Age-Related Lamin B1 Deficiency Activates SLC7A11-Dependent Disulfidptosis to Impair Osteogenesis via Intercepting Mitochondrial Fission.

Senile osteoporosis (SOP) is an aging-related disease characterized by decreased bone mass and susceptibility to fracture. The pathological mechanism of SOP, which involves impaired osteogenesis of bone marrow-derived mesenchymal stem cells (MSCs), warrants further investigation to facilitate the development of novel therapeutic agents. In this study, we demonstrated that age-related lamin B1 (LMNB1) deficiency in SOP-MSCs led to lamina-associated domain (LAD) detachment, causing super-enhancer (SE) formation to effectively drive SLC7A11 expression. Elevated SLC7A11 expression activated disulf
