Effect of Visual Crowding on Quality of Life in Glaucoma Patients.

PURPOSE: To investigate the association between visual crowding and vision-related quality of life (QoL) in patients with glaucoma. METHODS: This cross-sectional study included 50 patients diagnosed with POAG. Visual crowding was quantified by measuring the critical spacing (Scritical). The Scritical was assessed in each visual field quadrant. For each eye, a global Scritical was calculated as the mean of the eligible quadrant-specific values. Vision-related QoL was assessed using the 25-item National Eye Institute Visual Function Questionnaire (NEI VFQ-25), scored using Rasch analysis. Assoc

Discovery of Hyperelastic Constitutive Laws from Experimental Data with EUCLID.

Aseptic nonunion is prevalent in orthopedic practice, causing persistent pain and functional impairment. Humeral shaft fractures, accounting for 3-5% of all fractures, have nonunion rates of 2-33% in nonoperative and 5-10% in surgical management. This study, the largest case series on operative management of humeral shaft nonunion, examines surgical techniques and outcomes. This retrospective study, conducted at a referral center in eastern India, included 132 patients with aseptic humeral shaft nonunion from 2002 to 2020. Cases were from May 2002 to April 2012 and May 2015 to December 2020. P

Personalized Biomechanical Modeling of Pathologic Fracture: CTFEA Reveals Limitations of Traditional Fracture Risk Assessment in Benign Bone Tumors.

Benign bone tumors such as chondroblastoma, giant cell tumors (GCT), and aneurysmal bone cysts (ABC) are rare but clinically significant lesions that frequently occur in the epiphyseal regions of long bones, particularly near load-bearing joints in children and young adults. These tumors compromise the structural integrity of bone, leading to an elevated risk of pathologic fracture. Traditional methods for estimating fracture risk rely on simple geometric thresholds and volumetric ratios, but they fail to account for patient-specific differences in bone geometry, material heterogeneity, and ph

Arthroscopic Curettage and Bone Grafting of Scaphoid Enchondroma.

Benign tumors of the carpal bones are uncommon, and their optimal treatment is still a topic of debate due to the condition's rarity. While there is consensus on treating enchondromas with pathological fractures to alleviate symptoms, the best approach for removing these benign tumors remains uncertain. With the growing popularity of wrist arthroscopy for various wrist pathologies, we present our technique of arthroscopic enucleation, bone grafting, and fixation of scaphoid enchondroma. Our method showed favorable outcomes at the one-year follow-up. We aim to describe our technique, including

Metastatic Bone Disease With Pathologic or Impending Pathologic Fractures: Resolve, Refer, or Collaborate.

It is inevitable that every orthopaedic surgeon will need to treat patients with pathologic or impending pathologic fractures secondary to metastatic disease. Although the input of an orthopaedic oncologist is usually helpful and prudent, transferring care to an orthopaedic oncologist may not be feasible at all institutions in a prompt time frame. To avoid potentially detrimental delays to the patient, many cases can be appropriately managed or resolved by a nononcologic orthopaedic surgeon. It is therefore imperative for all orthopaedic surgeons to be familiar with the appropriate clinical an

Comparison of treatment options with long intramedullary nail and dynamic hip screws for metastatic subtrochanteric fracture: A cohort study.

BACKGROUND: Pathological subtrochanteric fractures pose challenges due to the high biomechanical demands of this region and the systemic complexity of metastatic disease. At present, the optimal fixation method remains controversial, particularly between intramedullary nailing (IMN) and dynamic hip screw (DHS) fixation. This study compared the mechanical, oncological, and functional outcomes of these two treatment strategies. METHODS: This retrospective study reviewed patients treated for metastatic subtrochanteric fractures at a single tertiary referral center between 2002 and 2018. Demograp

Result of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Nail.

Subtrochanteric femoral fractures account about 10.0% to 34.0% of all hip fractures with a high complication rate. Anatomically the fracture is proximal to adequately control with implants for femoral shaft and distal enough to control with implants used for intertrochanteric fractures. Inherent instability and forces of muscles giving the fracture a tendency to varus collapse. Due to high rate of implant failure; extramedullary and intramedullary implants are not suitable these fractures. Recently Proximal femoral nail (PFN) has been applied in treatment of subtrochanteric fractures. It has a

ITAM-Syk signaling mediates the rebound phenomenon after anti-RANKL antibody discontinuation.

Rebound bone loss following discontinuation of antiresorptive therapy, especially denosumab, represents a major clinical issue due to rapid osteoclast activation and increased fracture risk. However, the molecular basis of this phenomenon remains poorly understood. In this study, we investigated the role of the DAP12/TREM2-Syk signaling pathway in rebound-associated bone resorption using a murine model treated with and withdrawn from a RANKL-neutralizing antibody. Wild-type (WT) mice exhibited marked rebound bone loss, with elevated expression of DAP12/TREM2-related genes (Tyrobp and Trem2), i

Outcome of surgical treatment for metastatic bone disease of the distal femur: Observational single-center study of 47 patients.

INTRODUCTION: There is a paucity of data regarding the surgical treatment of distal femoral metastatic lesions. In this retrospective study, we aim to describe the outcome of surgery in this location and further analyze the findings based on the type of surgical reconstruction. METHODS: 47 patients (48 fractures) who underwent surgery due to pathological fractures of the distal third of the femur, between 2000 and 2024, were included in the analysis. There were 29 prostheses and 19 osteosyntheses (10 plates, 9 nails). Local complications, implant revision rate, functional outcome regarding pa

Surgical treatment of displaced intraarticular pediatric condyle fractures.

AIM: Conservative treatment of pediatric condyle fractures remains the generally accepted norm even today. However, longterm results of conservatively treated displaced pediatric condyle fractures often display adverse effects on the affected temporomandibular joint (TMJ), and also on the growing facial skeleton. We analyzed results of surgical treatment in pediatric intraarticular condyle fractures. PATIENTS AND METHODS: Children with displaced intraarticular condyle fractures were treated surgically, with open reduction and internal fixation (ORIF). The articular disc was addressed in each

Dual-action bisphosphonate hydrogel preserves cortical bone integrity and restores healing cascade in osteoporotic fracture-related infection.

In osteoporotic fracture-related infection (FRI), the coordinated bone healing is compromised by cortical bone lysis and persistent bacterial colonization. Current treatments primarily focus on infection control, but fail to preserve bone microarchitecture or restore disrupted healing cascade, particularly in fragile osteoporotic bone. Here, we develop a bioactive nanocomposite hydrogel based on bisphosphonate-modified hyaluronan (HABP) and either free bisphosphonate (BP) or pyrophosphate (TSPP), carrying antibacterial Ag⁺ ions and Vancomycin. Triggered by the pathological pH of biofilms, this

Micro-computed tomographical and histopathological analyses of medial tibial plateaus from patients with subchondral insufficiency fracture or osteoarthritis of the knee.

BACKGROUND: Subchondral insufficiency fracture of the knee (SIFK) is a rare and rapidly progressing knee joint disease. The typical diagnostic finding of SIFK by magnetic resonance imaging (MRI) is a subchondral hypointense line around the bone marrow lesions (BMLs), which are also commonly observed in knee osteoarthritis (OA). Subchondral bone fracture and its repair reactions are implicated for SIFK. However, pathological changes of SIFK and BMLs and the mechanism by which SIFK is induced remain elusive. We aimed to examine characteristics of SIFK and OA by micro-computed tomography (micro-C

Total Hip Replacement in Fracture of Hip with Unicameral Bone Cyst - A Rare Case Report.

INTRODUCTION: Unicameral (simple) bone cysts (UBCs) are fluid-filled intramedullary cavities commonly seen in the metaphysis of long bones in young patients under the age of 20 years. Pathological hip fractures associated with UBCs in the proximal femur are rare, especially in non-osteoporotic elderly patients. The management of such tumors, such as benign lesions of bone causing pathological fractures at the biomechanically critical load-bearing zone of the neck of the femur, remains a clinical challenge due to limited literature on optimal treatment strategies. CASE REPORT: A 60-year-old no

The Importance of Closed Reduction and Internal Fixation done with Intramedullary Implants in Pathological Fractures - A Case Series.

INTRODUCTION: Pathological fractures resulting from metastatic bone involvement, multiple myeloma (MM), and atypical femoral fractures present complex therapeutic challenges. Intramedullary (IM) nailing offers a biomechanically stable solution that facilitates early mobilization and enhances functional outcomes. This case series aims to evaluate the clinical and radiological results of IM fixation in pathological fractures, emphasizing pain relief and early functional outcomes. CASE SERIES: We report a series of five patients with pathological fractures of long bones who underwent IM nailing,

Combined Approach (Surgery + Systemic Adjuvant Therapy) in a Giant Cell Tumor of Proximal Femur: A Rare Case Report.

INTRODUCTION: Giant cell tumor of the bone (GCTB) is a locally aggressive benign tumor involving the ends of the long bones seen in the second or third decade of life, more commonly in women. Even though it is benign, it has a potential for malignant transformation in 10% of cases and metastasis to the lung in 1-4% of patients. GCTB of the proximal femur has a high recurrence rate, high incidence of pathological fracture, and poor prognosis. CASE REPORT: A 29-year-old man presented with pain and swelling in the left proximal femur of 1 year duration. After confirmation of his diagnosis using

Successful Chemotherapy Induction Following Management of Obstructive Jaundice and Femoral Pathological Fracture in Extensive-Stage Small Cell Lung Cancer: A Case Report.

Patients with poor Eastern Cooperative Oncology Group performance status (ECOG PS) ≥ 3 may be considered for systemic chemotherapy when the decline is driven by reversible tumour-related factors, but assessing reversibility is often difficult, and some patients ultimately receive best supportive care. We report the case of a 76-year-old man with extensive-stage small cell lung cancer (SCLC) whose condition declined to PS 3 due to obstructive jaundice from pancreatic metastasis and a painful femoral fracture. Biliary stenting improved hepatic function; however, after the first cycle of carbopla

Pathological fracture and epidural hematoma as rare complications of calcified cephalohematoma.

Calcified cephalohematoma is generally regarded as a benign condition with cosmetic consequences. Because of its benign clinical course, reports with follow-up periods exceeding 10 years are lacking. To date, no reports have linked it to increased susceptibility to severe head trauma. Here, we describe a rare case of pathological fracture and acute epidural hematoma arising from trauma to a calcified cephalohematoma in an adolescent. A 15-year-old boy with a history of right parietal calcified cephalohematoma was followed up conservatively until the age of 3 years. During baseball practice, th
