FracDet-v11: a multi-scale attention and wavelet-enhanced network for real-time pediatric wrist fracture detection.

Pediatric wrist fracture detection is diagnostically challenging due to subtle fracture morphologies and growth plate obscuration. To address this, we propose FracDet-v11, a specialized real-time framework based on YOLOv11s. We introduce a series of architectural enhancements to optimize feature representation and detection robustness: (1) a reconstructed backbone integrating Haar Wavelet Downsampling (HWD) and PKI-CAA to effectively preserve high-frequency details, complemented by a Dual-branch Channel Attention Module (DCAM); (2) a lightweight Slim-Neck for efficient feature fusion; and (3)

Concurrent pathologic femoral shaft fracture in bone metastasis and acute myocardial infarction: a case report.

An 86-year-old woman with a history of hepatocellular carcinoma was brought to our emergency department after an indoor fall. Her chief complaint was pain in the left thigh and chest. The patient was diagnosed with a pathological femoral shaft fracture, bone metastasis, and acute myocardial infarction. She underwent preoperative embolization and percutaneous coronary intervention, followed by internal fixation on the same day as the injury. The patient was discharged without postoperative myocardial infarction and was able to walk independently using a walker. Femoral diaphyseal fractures and

Femoral pathologic fractures are not a poor prognostic factor with appropriate treatment: A retrospective study.

BACKGROUND: The prognostic impact of pathologic fractures in patients with bone metastases remains controversial. In this study, we focused on pathologic femoral fractures, which are among the most common and are associated with significant morbidity. METHODS: We retrospectively reviewed cases of pathologic femoral fractures treated at our department between 2008 and 2020. A control group was selected from patients with metastatic bone tumors without pathologic femoral fractures during the same period. The two groups were matched 1:1 using propensity score matching based on age, new Katagiri

Thrombolytic Therapy After Return of Spontaneous Circulation in Patients With STEMI From Medically Underdeveloped Areas: A Case Series.

BACKGROUND Although current guidelines classify prolonged cardiopulmonary resuscitation (CPR) as a relative contraindication to thrombolytic therapy, this treatment may serve as a viable reperfusion strategy for patients with ST-segment elevation myocardial infarction (STEMI) who achieve return of spontaneous circulation (ROSC) when primary percutaneous coronary intervention (PCI) cannot be performed in a timely manner or is unavailable. This case series evaluated the safety and efficacy of thrombolytic therapy after ROSC in 12 patients with STEMI. CASE REPORT Twelve patients with STEMI (9 men

Association Between Immunohistochemical Profile and Radiographic Presentation of Breast Cancer Skeletal Metastases.

Background/Objectives: Understanding the biological factors that drive the behavior and clinical presentation of breast cancer (BC) skeletal metastases (SM) is critical for improving diagnostic accuracy and guiding treatment strategies. However, evidence regarding the immunohistochemical (IHC) profiles of SM and their association with radiographic characteristics and clinical features remains limited. This study aimed to evaluate the relationship between estrogen receptor (ER), progesterone receptor (PR), HER2 receptor status, and Ki-67 proliferation index with the radiographic presentation of

Biomaterials-based engineering of the bone microenvironment for osteoporosis therapy.

Osteoporosis is a progressive skeletal disorder marked by an imbalance between bone resorption and formation, resulting in compromised microarchitecture and increased fracture risk. However, conventional pharmacological therapies have systemic side effects and limited targeting efficiency. Therefore, these limitations highlights the need for innovative strategies, and biomaterials have emerged as versatile tools, offering both structural support and the ability to modulate the osteoporotic bone microenvironment. This review outlines the key pathophysiological changes in osteoporosis including

[Management of humeral shaft fractures in childhood and adolescence : Consensus report of the Pediatric Traumatology Section of the DGU].

Humeral shaft fractures are rare in childhood, accounting for less than 5% of all pediatric fractures. Indirect trauma predominates in younger children, whereas direct trauma is more common in older patients. To date, no uniform recommendations exist regarding diagnostic evaluation or treatment management. Therefore, during the 13th and 14th scientific meetings of the Pediatric Traumatology Section (SKT), the available evidence was reviewed, the management strategies were critically discussed within an expert panel and a consensus was formulated. The management remains predominantly conservativ

Evaluation of femoral neck fracture migration after internal fixation with an angle stable implant compared with gold standard cannulated cancellous screws in young adults: study protocol for a clinical RCT using radiostereometric analysis.

BACKGROUND: Hip fractures are most commonly suffered by elderly patients, but 2-11% of hip fractures affect younger patients. In young adults with femoral neck fractures, internal fixation is the recommended treatment for both displaced and non-displaced fractures. However, treatment with internal fixation carries a high complication rate. A known complication of internal fixation after gold standard cannulated cancellous screws is fracture compression resulting in shortening of the femoral neck, which is associated with clinically important decreases in functional outcome. New osteosynthesis

Clinical stem cell therapy in oral and craniofacial bone regeneration: a systematic review and meta-analysis.

UNLABELLED: Craniomaxillofacial bone regeneration poses significant clinical challenges due to the anatomical complexity of this region and the inherent limitations of conventional reconstructive techniques. Stem cell-based therapies have emerged as a promising alternative in that stem cells harness the capacities of multilineage differentiation and paracrine signaling to enhance tissue regeneration. Nonetheless, the overall clinical efficacy of stem cell therapy remains a subject of debate. In this systematic review and meta-analysis, we aimed to comprehensively evaluate the safety and effect

Open Chest Wound with Sternal Fracture in the Emergency Department, a Case Report.

UNLABELLED: This case highlights a rare and complex occurrence of an open chest wound with a pathologic sternal fracture. This resulted from a combination of remote chest trauma, chronic chest wall infection, malignancy, and ultimately, sternal osteomyelitis. A 69-year-old male presented with a large, open anterior chest wound, chronic ulceration, and weight loss. Thirty years earlier, the patient had sustained trauma from a firecracker, which led to a chronic wound for which he did not seek medical attention. Physical exam revealed a large open chest wound with an open sternal fracture and ex

Three-Dimensional Human Liver Micro Organoids and Bone Co-Culture Mimics Alcohol-Induced BMP Dysregulation and Bone Remodeling Defects.

Hepatic osteodystrophy (HOD) is a frequent complication of chronic liver disease, marked by impaired osteogenesis and elevated fracture risk, particularly under sustained alcohol exposure. Bone morphogenetic proteins (BMPs), which play a crucial role in maintaining bone homeostasis, are dysregulated in alcoholic liver disease. Specifically, decreased BMP2 and increased BMP13 have been linked to impaired osteogenesis and cartilage-like shifts in bone progenitors. A human in vitro system that recapitulates this hepatic BMP imbalance is needed to dissect mechanisms and identify targets. To addres

Effects of Mechanical Stress on Bone and Cartilage Metabolism: How Mechanical Stress Affects Energy Metabolism in Bone and Cartilage Tissues (Our Research Overview): Mini Review.

Bone resorption and formation are known to change in response to mechanical stress. The mechano-transduction mechanism by which bone tissue senses the stress, altering cellular activity in response via intracellular signaling pathways, ultimately leading to physiological and pathological changes, is beginning to be elucidated. Furthermore, excessive mechanical stress on bone and joints due to aging, obesity, overload, and overuse is thought to cause decreased chondrocyte activity, degeneration and destruction of the cartilage collagen matrix, degeneration of the subchondral bone, and joint dys

Impaired Bone Healing and Fracture Complications during Limb Lengthening in a Neurofibromatosis Type 1 Patient Receiving Selumetinib: Case Report and Literature Review.

INTRODUCTION: Neurofibromatosis Type 1 (NF1) is a genetic disorder that may lead to the development of plexiform neurofibromas (Pns). Selumetinib, a selective mitogen-activated protein kinase (MEK1/2) inhibitor, has shown clinical benefit in shrinking inoperable PNs. However, its long-term effects on the skeleton remain unclear. This case describes impaired bone healing and pathological fractures following limb lengthening in a child with NF1 receiving prolonged selumetinib therapy. CASE REPORT: A 5-year-old boy with genetically confirmed NF1 and left lower limb PN-induced overgrowth presente
