No difference in outcomes for posterior shoulder instability surgery in patients with a normal vs. pathological radiologist reported magnetic resonance arthrogram study.

BACKGROUND: Despite improvement in diagnostic imaging techniques, there remain patients with clinical posterior shoulder instability (PSI) with so called normal or nonpathologic findings on magnetic resonance arthrogram (MRA). This diagnostic dilemma creates challenges for clinicians and patients who fail conservative treatment when determining treatment options due to the incongruent findings on physical exam and MRA. We hypothesized that patients undergoing capsulolabral repair with a clinical exam consistent with PSI but normal radiologist reported MRA findings would experience similar clin

Congenital depressed skull fracture in a neonate without obstetric trauma.

UNLABELLED: Congenital depressed skull fractures (ping-pong fractures) without obstetric trauma are rare. A term male neonate delivered via uncomplicated caesarean section, demonstrated a right parieto-temporal skull depression (5 cm × 5 cm) at birth. Computed tomography revealed a 4 mm parietal depression without intracranial injury. No instrumental delivery or maternal trauma were present. The likely aetiology was intrauterine compression ('faulty foetal packing'). The patient was managed conservatively with close follow-up. CONTRIBUTION: This case underscores the importance of perinatal hi

Primary Intraosseous Malignant Nerve Sheath Tumor Of Humerus Presenting With Pathological Fracture: A Rare Case Report With Review Of Literature.

INTRODUCTION: Malignant peripheral nerve sheath tumors (MPNST) are rare, aggressive soft-tissue sarcomas arising from peripheral nerves, usually associated with neurofibromatosis Type-I. Intraosseous MPNSTs are exceedingly uncommon and are typically located in the maxillofacial region. There are only 45 documented cases of intraosseous MPNSTs outside the head and neck region, of which only two are of humeral origin and only one reported as pathological fracture. CASE REPORT: We present a rare case of a de novo MPNST of the humerus that presented with pathological fracture and radial nerve pal

Avascular Necrosis of the Talar Body Presenting with a Vertical Fracture and Dome Collapse in a Low-Risk Patient: A Case Report.

INTRODUCTION: Avascular necrosis (AVN) of the talus is an uncommon condition, although it can arise due to the bone's limited blood supply and typically occurs following high-energy trauma or in patients with recognised medical risk factors. Fractures developing as a result of AVN, particularly vertical fractures through the talar body with early dome collapse, are relatively rare. This report describes a unique presentation in a patient without classical risk factors, highlighting delays in diagnosis and the complexities associated with management. This appears to be one of the few reported c

Application of Coltosol F with a cotton-spacer technique for endodontic temporization: a clinical retrospective study.

BACKGROUND: Temporary restorative materials are important for maintaining coronal seal during multi-visit endodontic procedures. Coltosol F, a moisture-activated, self-setting material, is valued for its simplicity and sealing ability, though concerns exist regarding its potential to induce tooth fractures. OBJECTIVE: To evaluate the clinical performance of Coltosol F as temporary restoration material, focusing on tooth fracture incidence, spontaneous dislodgement and the role of a cotton pellet as a spacer material. DuoTemp placed with a cotton pellet and Coltosol F placed without a cotton p

Targeting BMP2 signaling pathway in osteogenesis and osteoarthritis: A path toward safer regenerative therapies.

Various signaling pathways are linked with osteogenesis and osteoarthritis progression. Bone Morphogenetic Protein 2 (BMP2), a key regulator within the TGF-β superfamily, is central to skeletal development through its ability to guide mesenchymal stem cells (MSCs) toward osteogenic and chondrogenic lineages. By activating canonical SMAD1/5/8 cascades alongside non-canonical MAPK branches (ERK, p38, JNK), BMP2 enhances transcriptional programs such as Runx2, thereby stimulating extracellular matrix synthesis, bone regeneration, and cartilage differentiation. Recombinant BMP2 (rhBMP2) has thus f

Pathomechanism of LYPD6 enhancing abnormal hyperostosis via Wnt/β-catenin signaling pathway in diffuse idiopathic skeletal hyperostosis.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a noninflammatory metabolic bone disorder marked by progressive ectopic ossification at spinal and peripheral entheses. Its molecular etiology is poorly understood. We investigated whether Ly6/uPAR domain-containing 6 (LYPD6) regulates osteoblast differentiation and drives pathological hyperostosis in DISH. METHODS: Primary osteoblasts from DISH patients and matched controls were assessed for ALP activity, calcium deposition, and expression of osteogenic markers. RNA sequencing identified LYPD6 as a candidate; its expression was v

Oxidative stress and bone marrow adipocytes: Molecular mechanisms and possible implications.

Redox homeostasis is crucial for maintaining cellular processes and is closely linked to human skeletal health. Prior research has demonstrated that oxidative stress is important for regulating osteoblast and osteoclast differentiation in the bone microenvironment, leading to a reduction in bone mass and skeletal degradation. The bone marrow is a complex niche containing various cell types, including bone marrow adipocytes (BMAs), which engage in dynamic interplay with osteo-associated cells through processes governed by redox equilibrium within the marrow compartment. During aging, a decrease

Targeting the PINK1/Parkin-FNDC5 pathway: a novel mechanism of icariin in regulating muscle-bone metabolic coupling in osteosarcopenia.

BACKGROUND: Osteosarcopenia (OS) significantly elevates fracture risk in the elderly due to concurrent skeletal and muscular metabolic decline. Mitochondrial autophagy dysregulation is a significant mechanism driving myoskeletal metabolic imbalance, and myofibroblast-derived factor 5 (FNDC5)/irisin is a crucial mediator in the myoskeletal dialogue. It is unclear if the medicinal ingredient icariin (ICA) from the traditional Chinese herb Epimedium can work in concert to prevent and treat OS through this pathway, despite research suggesting that exercise may enhance muscular performance by modul

Prognostic value of tip-to-apex distance in prophylactic fixation for pathological peritrochanteric femur fractures.

INTRODUCTION: Intramedullary prophylactic fixation provides early postoperative mobilization for patients with metastatic bone disease. Prior trauma literature demonstrates a tip-apex distance (TAD) > 25 mm significantly increases implant failure risk, but limited data exists for pathologic fixation. The primary purpose was to examine the significance of TAD in predicting failure of prophylactic fixation of impending pathologic peritrochanteric femur fractures. METHODS: Retrospective chart review identified patients at an academic institution from January 1, 2010 to January 1, 2020 undergoing

Enhancing bone metastasis CT report analysis: a comparison of local and proprietary large language models for privacy and resource efficiency.

BACKGROUND: Computed tomography (CT) reports for patients with bone metastases include key clinical details, but they are usually written as free text. Different doctors write reports in different ways, which makes it hard to extract information in a consistent way. Large language models (LLMs) may help pull out important details automatically. But using external models can raise data privacy and management concerns in hospitals. This study looked at whether proprietary models and locally deployed open-source LLMs can help extract key diagnostic information and create structured reports in set

Biodegradable Zn-Mg-Ca-Sr alloy intramedullary nails promote vascularized bone regeneration and suppress osteoclastogenesis for osteoporotic fracture repair.

Osteoporotic fractures are commonly associated with impaired bone regeneration and insufficient mechanical stability, posing major challenges for load-bearing internal fixation. Biodegradable metallic implants that combine mechanical support with biological regulation are therefore highly desirable for osteoporotic fracture repair. Here, we designed and evaluated a biodegradable Zn-Mg-Ca-Sr quaternary alloy as small-diameter intramedullary nails (IMNs) for osteoporotic load-bearing fracture fixation. Benefiting from multi-element alloying and hot extrusion, ZnMgCaSr(0.2) exhibited the most fav

Case Report: Reconstruction of the chest wall with titanium alloy plates after resection of a rare malignant spindle cell tumor of the sternum complicated by ankylosing spondylitis.

BACKGROUND: Primary malignant sternal spindle cell tumors are clinically rare, and the aggressive nature leads to a large chest wall defect because of extended resection. To date, no cases of sternal malignant spindle cell tumor complicated by ankylosing spondylitis have been documented in the literature. CASE PRESENTATION: We present a case of primary sternal malignant spindle cell tumor occurring in the setting of ankylosing spondylitis. Given the high risks associated with ankylosing spondylitis and the existing pathological fracture, preoperative biopsy was withheld after MDT evaluation.

Bilateral healing of pseudofractures in hypophosphatasia with teriparatide: a case report.

BACKGROUND: Hypophosphatasia is a rare, inherited metabolic bone disease characterized by impaired activity of tissue-nonspecific alkaline phosphatase leading to pathological mineralization of hard tissue, predisposing to poorly healing fractures. Limited access to enzyme replacement therapy makes the treatment of adults with hypophosphatasia challenging. A few case reports have described a beneficial effect of teriparatide on fracture healing in hypophosphatasia. However, this is the first case of a male patient with pediatric-onset hypophosphatasia, not previously exposed to antiresorptive t

Ultrasound-guided one-point puncture lumbosacral plexus block combined with laryngeal mask airway general anesthesia for thigh amputation in a patient with mucopolysaccharidosis: a case report.

Mucopolysaccharidosis (MPS) is a group of rare inherited lysosomal storage disorders caused by deficiencies of specific enzymes, leading to abnormal accumulation of glycosaminoglycans in tissues throughout the body. It is often associated with a difficult airway, cervical spine instability, restrictive pulmonary dysfunction, and cardiovascular pathologies, which significantly increase perioperative risks. This manuscript reports a case of a 36-year-old male MPS patient (clinical phenotype highly suggestive of type IV) who underwent right mid-to-upper thigh amputation for a pathological fractur
