Plasma concentrations of 25 essential (EAA) and nonessential (NEAA) amino acids were measured pre- and postdialysis in 46 chronic hemodialysis therapy (CDT) patients. Sixteen of these patients with prior weight loss of 14.5 +/- 2.37 pounds in 24 months were administered a GAA solution (EAA + NEAA + glucose) for 20 weeks during each dialysis. Four patients with acute myelosclerosis were treated with chemoradiotherapy and bone marrow transplantation. Temporal bone changes in a 14-month-old female infant with congenital rubella are described. The inner ears showed cochleo-saccular degeneration, with more severe involvement of the right side. Thirty-five patients with metastatic malignant melanoma underwent treatment with chlorozotocin administered as a single dose of 120 mg/m2 by means of rapid intravenous infusion every six weeks. Six patients with bone marrow cultures yielding Mycobacterium avium complex were encountered at the Mount Sinai Hospital between 1969 and 1976. One additional isolate of the same mycobacterial species was recovered from splenic cyst fluid of a seventh patient. The authors evaluated methylmethacrylate as an adjunct to internal fixation of 47 pathologic fractures of long bones (10 of the humerus and 37 of the femur) in 43 patients with metastatic disease. Although tuberculous disease of bones and joints is becoming uncommon, it still occurs and may cause devastating sequelae. It is frequently not diagnosed prior to the onset of permanent damage to the joints or spine; the most important reason for this delay may be the fact that it is not considered in the differential diagnosis of monoarthritis or back pain. Most persons with the disease have other evidence of tuberculosis. Not infrequently an aggressive approach (including synovial biopsy or surgical exploration of the back) is needed to confirm the diagnosis when there are no other clues.

Over half the patients in most pain clinics in the United States are seeking relief from the "failed disk syndrome." Careful initial diagnostic evaluation and aggressive nonoperative therapy are essential to avoid poor results. Causes of continued pain after disk surgery include original pain syndromes not due to a disk, inadequate surgery, development of arthritis, additional disk disorders and nerve root fibrosis. Treatment demands precise diagnosis. Diagnostic injections and nerve blocks are helpful when other techniques fail.
