Inflammatory cell death networks in osteoporosis: Mechanisms of pyroptosis, ferroptosis, and their crosstalk with apoptosis, autophagy, and PANoptosis in bone immune remodeling.

Osteoporosis is a systemic bone metabolic disease characterized by reduced bone mass and disrupted bone microstructure. Its pathogenesis is no longer viewed solely as a process of imbalance between bone formation and resorption. Recent advancements in bone immunology have revealed that bone immune dysregulation driven by chronic low-grade inflammation serves as an important pathological foundation for bone loss associated with aging, estrogen deficiency, and metabolic stress. In this context, inflammatory regulated cell death (RCD), which has high immunogenicity, is increasingly recognized as

Clinical Characteristics and Five-Year Outcomes of Patients Aged 75 Years or Older Receiving Maintenance Hemodialysis at a Japanese Regional Core Hospital.

Introduction The number of older patients receiving maintenance hemodialysis is increasing in Japan. Patients aged 75 years or older often have multimorbidity, frailty, nutritional vulnerability, and a higher risk of adverse clinical outcomes than younger patients. However, real-world longitudinal data from regional hospitals regarding mortality and clinically important complications in this population remain limited. This study aimed to compare five-year mortality, baseline clinical characteristics, laboratory findings, GI bleeding, and fractures between maintenance hemodialysis patients aged

Artificial Intelligence for Evidence Synthesis of Emerging Biologics to Improve Skeletal Health in Osteogenesis Imperfecta: Systematic Review and Meta-Analysis.

BACKGROUND: Osteogenesis imperfecta (OI) is a rare genetic disorder characterized by bone fragility and recurrent fractures. Emerging biologics demonstrate promise by targeting bone-remodeling pathways, yet evidence for their efficacy and safety remains fragmented and heterogeneous, and no prior systematic review in OI has incorporated artificial intelligence (AI) to synthesize it. OBJECTIVE: This study aims to systematically evaluate the efficacy and safety of novel biologics in patients with OI using an AI-assisted workflow for evidence synthesis. METHODS: We conducted a systematic review

Local ablative treatment and continuation of systemic therapy for oligoprogressive metastatic breast cancer: a retrospective cohort study.

INTRODUCTION: Patients with metastatic breast cancer (MBC) typically receive palliative systemic therapy until unmanageable toxicity or progression, at which point treatment is changed. However, this approach may be suboptimal for patients with oligoprogression (OLP), defined as progression in one or few metastatic sites while other metastases remain controlled. Local ablative therapy (LAT) targeting oligoprogressive lesions may allow continuation of otherwise effective systemic treatment. METHODS: We searched the registry of a tertiary cancer center for patients with MBC and oligoprogression

A Case of Thrombocytopenia, Anasarca, Fever, Reticulin Fibrosis/Renal Dysfunction, and Organomegaly (TAFRO) Syndrome Refractory to Treatment With Combination Therapy of Corticosteroid, Tocilizumab, and Rituximab: A Case Report.

We report a case of TAFRO (thrombocytopenia, anasarca, fever, reticulin fibrosis/renal dysfunction, and organomegaly) syndrome in a previously healthy 76-year-old man who was treated with glucocorticoids, tocilizumab, and rituximab. Despite this combination therapy, he developed multiple infections, and the disease progressed, ultimately leading to death. The patient initially presented with bilateral lower extremity edema and was referred to our hospital for evaluation of severe renal dysfunction and elevated inflammatory markers. Computed tomography revealed mild generalized lymphadenopathy.

Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography (FDG PET/CT) Monitoring of Rapidly Progressive Low-Risk Cutaneous Melanoma: A Case Report of Durable Response to Combined Stereotactic Body Radiotherapy (SBRT) and Immunotherapy.

We present a case of metastatic cutaneous melanoma without a BRAF mutation that showed an unusually aggressive disease course despite favorable initial histopathological features. The patient was initially diagnosed with superficial spreading melanoma with a negative sentinel node. After resection of the subcutaneous metastasis, rapid and extensive systemic dissemination occurred within two months. Metastases affected the liver, lungs, bone, soft tissue structures, and a large parailiac tumor. Therapy included combined immunotherapy (pembrolizumab + ipilimumab) and multiple series of stereotac

Safety and preliminary efficacy of allogeneic bone marrow-derived mesenchymal stromal cells (BMSC) for painful lumbar facet arthropathy: study protocol for a randomised, double-blinded, placebo-controlled, crossover phase II clinical trial.

BACKGROUND: Lumbar facet arthropathy (LFA) is a degenerative, arthritic joint disorder and a common specific cause of axial low back pain and functional impairment. Currently, no disease-modifying therapies are available for symptomatic LFA. Mesenchymal stem/stromal cell (MSC)-based therapy represents an emerging regenerative treatment modality with the promise to target the underlying pathophysiology of LFA. Despite encouraging clinical evidence supporting the safety and efficacy of MSC therapy in other osteoarthritis, its therapeutic utility in LFA remains largely unexplored. We recently com

Functional profiling of ovarian cancer models reveals Bcl-xL/NOTCH targeting to overcome resistance.

Platinum resistance remains a major therapeutic challenge in ovarian cancer (OC) and is one of the main causes leading to disease relapse and mortality. Although PARP inhibitors have improved outcomes for a subset of patients, most women continue to rely on platinum-taxol based chemotherapy and ultimately develop recurrent, treatment-resistant disease with limited further therapy options. Therefore, there is a critical need to identify actionable, patient-specific vulnerabilities that would help as an alternative for chemoresistant patients. To identify such therapeutic opportunities, we estab

Spatial lipid-barrier coupling in knee osteoarthritis: a mechanism-based perspective centered on the infrapatellar fat pad.

Knee osteoarthritis (KOA) is a heterogeneous whole-joint disorder in which mechanical loading, aging, metabolic stress, synovitis, microvascular dysfunction, matrix injury, subchondral remodeling, pain-related neurovascular change, and impaired repair interact over time. The infrapatellar fat pad (IFP) is increasingly recognized as an active synovio-adipose tissue, yet IFP inflammation, lipid disturbance, hypoxia, angiogenesis, matrix catabolism, and subchondral change are often considered parallel abnormalities rather than as a spatially organized propagation process. This Perspective propose

Mesenchymal stem cell therapy for diabetes: clinical evidence, emerging strategies, and future perspectives.

Diabetes mellitus (DM) is a multifactorial metabolic disorder in which chronic hyperglycemia arises alongside adipose-tissue dysfunction, ectopic lipid accumulation, endothelial injury, and progressive multiorgan damage. These processes form an interconnected network, and because lowering glucose or blocking any single pathway leaves the other nodes active, the disease continues to advance even when glycemic targets are met. Therapies that act on several nodes at once are therefore conceptually attractive. Mesenchymal stem cell (MSC) therapy fits this requirement, engaging the immune, vascular

Skeletal metastatic burden as a strong prognostic factor of survival in advanced NSCLC treated with chemo-immunotherapy: An exploratory imaging-based analysis.

BACKGROUND: Metastatic burden is a key determinant of prognosis in advanced solid tumors, yet current RECIST 1.1-based assessment does not fully capture total or site-specific disease extent. Emerging evidence suggests that quantitative imaging-derived metrics may improve prognostic stratification beyond binary metastatic classification. METHODS: We retrospectively analyzed 52 patients with advanced non-oncogene-addicted lung cancer. Baseline clinical variables and imaging-derived metastatic lesions were assessed across multiple organ sites (lung, lymph nodes, bone, brain, liver and adrenal g

Chronic THC Inhalation During Early Adulthood Exacerbates Ovariectomy-Induced Bone Loss in a Rat Model of Osteoporosis.

Introduction: Osteoporosis is a debilitating bone disease that affects a large segment of the population. Currently, pharmacological treatment options for osteoporosis, including antiresorptive drugs and those that prevent bone loss, are limited. Multiple studies have shown that the endocannabinoid system and cannabis may play a significant role in bone remodeling. As both the use of cannabis and high-quality research on its effects continue to grow, novel medicinal uses of selected components, such as tetrahydrocannabinol (THC), are being investigated. The present study examined the potential

Impact of Pharmacy Benefit Manager Organizations on Diabetes Management.

Pharmacy benefit managers (PBMs) administer prescription drug benefits for insurers, employers, and other payers through formulary design, rebate negotiation, pharmacy network management, and utilization controls. In diabetes care, cost-containment strategies such as formulary-driven medication switching, step therapy, and formulary exclusivity have increasingly restricted access to essential medications and technologies. These rebate-driven practices often prioritize financial incentives over clinical appropriateness, leading to forced nonmedical switching, higher out-of-pocket costs, reduced

Biologic Therapies for Knee Osteoarthritis: A Structured Narrative Review of Joint Preservation Strategies and Clinical Outcomes.

Knee osteoarthritis (KOA) is among the leading causes of chronic pain and physical disability worldwide, representing a major clinical challenge as currently available treatments primarily address symptoms rather than the biological mechanisms driving disease progression. As interest in joint preservation strategies has grown, biologic therapies have emerged as promising approaches capable of modulating the intra-articular environment and potentially delaying invasive interventions. This structured narrative review critically synthesizes contemporary evidence retrieved from PubMed, the Cochran
