Low-grade osteogenic tumor with secondary aneurysmal bone cyst: Distinguishing osteoblastoma from low-grade osteosarcoma in the setting of noncanonical molecular findings.

Low-grade osteogenic tumors in the appendicular skeleton can show overlapping radiologic and histologic features, especially when secondary aneurysmal bone cyst (ABC) change is present. Osteoblastoma (OB) with secondary ABC and low-grade osteosarcoma (LG-OS) with secondary ABC may be difficult to distinguish, and noncanonical molecular alterations such as KMT2D or ZRSR2 variants may further complicate interpretation. We present an 18-year-old male presented with 6 months of atraumatic hip/knee pain and progressive difficulty weight-bearing. Imaging demonstrated a proximal femoral intramedullar

Solid Variant Aneurysmal Bone Cyst of the Trapezoid Bone: A Case Report.

An aneurysmal bone cyst (ABC) is classified as a benign osteoclastic giant cell-rich tumour. We report a case of a 48-year-old male patient with a solid variant ABC involving the trapezoid bone. We chose to fill the defect with bone cement after curettage. At the 12-month follow-up, the patient remained recurrence-free with satisfactory hand function. Carpal bone tumours are rare and typically benign, with malignant and metastatic lesions being extremely uncommon. We were unable to find any other reports of a solid variant ABC in the trapezoid bone. This case highlights the importance of histo

Intralesional resection and autologous fibular graft reconstruction for a giant cell tumor of the fourth metacarpal with a secondary aneurysmal bone cyst: a case report.

INTRODUCTION AND IMPORTANCE: Giant cell tumor (GCT) of bone rarely involves the metacarpal bones. Secondary aneurysmal bone cyst (ABC) changes arising in association with GCT create distinct diagnostic and therapeutic challenges in this uncommon skeletal location. CASE PRESENTATION: A 21-year-old right-hand-dominant female presented with progressive right fourth metacarpal pain and swelling following minor trauma. Radiographs and magnetic resonance imaging (MRI) demonstrated an expansile osteolytic lesion with cortical thinning and internal fluid-fluid levels suggesting GCT with secondary ABC

New Insight in Pediatric Orthopedic Oncology: The Use of a Xeno-Hybrid Bone Substitute in Loss of Bone Tissue After Oncological Resections, a Case Series.

Background: The management of bone defects in pediatric oncology represents a major challenge in orthopedics, as it requires preserving both joint function and skeletal growth. Traditional reconstructive approaches, such as autografts and allografts, are limited by availability, complications, and incomplete biological integration. In this context, xeno-hybrid bone substitutes have emerged as a promising alternative. The aim of this study was to evaluate the safety and effectiveness of SmartBone® ORTHO in the reconstruction of post-oncological bone defects in children. Methods: Twelve pediatri

[Primary bone tumors of the spine].

BACKGROUND: Primary bone tumors of the spine are rare causes of spinal symptoms and often present with nonspecific clinical findings. Reliable differentiation from degenerative or inflammatory conditions is usually only possible by imaging. Precise anatomical compartment allocation as well as the combination of magnetic resonance imaging (MRI) and computed tomography (CT) are essential for diagnosis. OBJECTIVE: The aim of this review is to provide an overview of primary benign and malignant bone tumors of the spine, focusing on their imaging characteristics and differential diagnostic feature

An Atypical Presentation of a Non-neural Granular Cell Tumor.

Non-neural granular cell tumors (NNGCTs) represent a rare variant of granular cell tumors that lack S100 expression and do not demonstrate nerve-sheath differentiation, distinguishing them from conventional granular cell tumors (CGCTs), which are typically S100-positive and of Schwann cell origin. This report describes the case of a 46-year-old male presenting with a soft tissue mass arising from the right maxillary alveolar ridge. A hemimaxillectomy was performed, and the mass was sectioned to reveal light tan fibrous tissue grossly involving the underlying bone. The mass was composed of larg

Pediatric Cervical Spine Aneurysmal Bone Cyst with Recurrent Atlantoaxial Rotatory Subluxation Requiring C1-C2 Fusion: A 7-Year Follow-Up Case Report.

INTRODUCTION: Aneurysmal bone cysts (ABCs) of the cervical spine in children are rare, locally aggressive lesions that may compromise spinal stability. Atlantoaxial rotatory subluxation (AARS) is an uncommon association and is typically related to trauma or infection. The coexistence of ABC with AARS, particularly with recurrence of instability after tumor resolution, is extremely rare and presents unique diagnostic and therapeutic challenges. CASE REPORT: An 8-year-old boy presented with progressive neck pain and torticollis without a history of trauma or infection. Imaging revealed an expan

Long-term follow-up of 2 techniques to fill Total Ankle Replacement cysts: autograft and/or PCa substitute versus PMAA cement.

INTRODUCTION: Periprosthetic cysts are a frequent cause of Total Ankle Replacement (TAR) revision. Curettage and backfill is a common treatment for cyst formation. We have compared functional and radiological results for two types of backfill. MATERIALS AND METHODS: This was a continuous, single-center, single-surgeon series of 32 TARs reoperated for cysts (22 AES, 10 Hintegra; 47% osteorathritis on laxity, 41% post-fracture; mean age at revision: 65.3 years) at a mean 6.4 years (2.4-14.5). Two types of filling were used successively: Group A - bone grafting, 7 cancellous autografts (May 2008

Non-Oncological Progression of an Aneurysmal Bone Cyst (ABC) in a Puppy Following Surgical Treatment.

Aneurysmal bone cysts (ABCs) are benign but locally aggressive osteolytic lesions that are rarely reported in dogs and may progress or recur following surgical treatment. This case report describes the diagnosis, surgical management, postoperative progression, and long-term outcome of an ABC affecting the left ulna of a four-month-old Labrador Retriever. Radiography and computed tomography revealed a rapidly progressive, expansile osteolytic lesion with marked cortical thinning. Histopathological examination supported the diagnosis of ABC, with no evidence of malignancy. Initial treatment cons

[Primary malignant bone tumours-part II].

Chondrosarcomas are tumours that typically occur in older adults. Although a few arise de novo, they often develop from pre-existing enchondromas, much more frequently from osteochondromas. In these secondary chondrosarcomas, the primary site of pre-existing chondrogenic lesion plays an important role (close to the trunk, centrally; appendicular skeleton, peripherally). Thus, low-grade chondrosarcomas in the periphery are referred to as atypical chondrogenic tumours (ACT), whereas tumours of the same histopathological type located close to the axial skeleton are called chondrosarcoma grade 1.

Is a Resorbable Citrate-based Bioceramic Device Associated With Osseous Integration? An Early Retrospective MRI Analysis.

BACKGROUND: Bioresorbable fixation devices may be advantageous because they eliminate the question of whether they ought to be removed, have biomimetic qualities, and minimize artifacts seen on MRI. Most bioresorbable devices utilized during orthopaedic surgery consist of thermoplastic polymers derived from lactic and glycolic acids. These devices are linked to implant-related osteolysis, inflammation, and bone tunnel enlargement, and their integration with surrounding bone is inconsistent. Citregen®, a resorbable composite of a citrate-based thermoset polymer and hydroxyapatite, recently has

Imaging of Fibrous Dysplasia: A Comprehensive In-Depth Analysis of Monostotic, Polyostotic, Syndromic Forms, and Bone Sarcoma Development.

Fibrous dysplasia is one of the most common skeletal lesions. The wide spectrum of clinical manifestations ranges from asymptomatic conditions (typical of monostotic forms) to severe skeletal diseases with deformity and fractures for polyostotic fibrous dysplasia. The classical radiological features include: an osteolytic geographic pattern, ground-glass bone matrix, cortical thinning/cortical scalloping, bone deformities and enlargement, concavity of margins (evaluated with MRI), and cystic areas (MRI). All the bones can be affected, and the proximal femur is the most common one (about 30% of

Demineralized Bone Matrix in Foot and Ankle Surgery: Products, Mechanisms, and Clinical Outcomes.

BackgroundBone grafting is a commonly used adjunct in foot and ankle surgery to enhance osseous healing in fracture fixation and arthrodesis. Although autologous corticocancellous bone is commonly used, donor site morbidity and limited graft availability have driven the development of allogenic alternatives such as demineralized bone matrix (DBM).FormulationsDemineralized bone matrix is produced through decalcification of donor bone, preserving a collagen-rich scaffold and biologically active proteins that support osteoconduction and osteoinduction. Commercial formulations vary in carrier syst

Novel Application of the AGN1 Triphasic Calcium-Based Implant for a Large Simple Bone Cyst in a Child With 3-Year Follow-up: A Case Report.

INTRODUCTION: Simple bone cysts (SBCs) are benign lesions in children, typically in the proximal humerus. While many are asymptomatic, some necessitate treatment because of impending pathological fractures. Different treatment modalities exist, but no consensus on the best method has been established. CASE REPORT: To the best of our knowledge, this case report describes the first reported clinical application of AGN1 (Ossure™), a CE-marked triphasic calcium-based resorbable implant, in the treatment of a large SBC.A 10-year-old girl with a history of recurrent fractures secondary to a large S

Nonsurgical Treatment of Radiculopathy From a Lumbar Synovial Cyst Using the Minimally Invasive Lumbar Decompression (MILD) Procedure: A Case Report.

Lumbar synovial cysts are fluid-filled lesions arising from the facet joints that can cause lateral recess stenosis and neurogenic claudication. While surgical decompression is the gold standard for symptomatic cysts, patients with significant comorbidities, such as chronic obstructive pulmonary disease (COPD) and emphysema, face elevated perioperative risks under general anesthesia and delayed wound healing. This case report describes the successful application of the minimally invasive lumbar decompression (MILD) procedure as a safe alternative for a high-risk surgical candidate. A 68-year-o

A Case of Large Rice Body Containing Cyst With an Irreparable Rotator Cuff Tear and Hill-Sachs Lesion Treated With Reverse Shoulder Arthroplasty.

Subacromial-subdeltoid bursitis with rice bodies, associated with shoulder instability, is uncommon. The pathogenesis of rice body formation remains unclear; however, it is considered a nonspecific response to chronic inflammation. We present the case of a 79-year-old woman with subacromial-subdeltoid bursitis of the right shoulder, numerous rice bodies, and rheumatoid arthritis (RA). She had a history of right shoulder dislocation, an irreparable rotator cuff tear, and a large Hill-Sachs lesion. She underwent reverse shoulder arthroplasty (RSA) with rice body removal and showed symptom improv

Xanthogranulomatous epithelial tumour of the calcaneus in a 32-year-old female: a rare entity.

Xanthogranulomatous epithelial tumour (XGET) is a recently described, rare mesenchymal neoplasm characterized by a HMGA2::NCOR2 gene fusion. We report a case of primary intraosseous XGET in the calcaneus of a 32-year-old female presenting with a 10-month history of heel pain. Initial differential diagnosis based on imaging features of the lytic calcaneal lesion included giant cell tumour of bone (GCTB) and solid aneurysmal bone cyst (sABC). Histopathology revealed a bland appearing lesion with xanthomatous histiocytes and giant cells, but immunohistochemistry showed diffuse cytokeratin positiv
