Osteitis Condensans Ilii: An Uncommon Cause of Back Pain.

Osteitis condensans ilii (OCI) is a benign cause of low back pain, which is self-limiting. Though OCI is still an orthopedic mystery, mechanical stress across the joint is a significant triggering factor according to the prevailing theories. The traditional location of involvement is around the ileum, and can be misinterpreted as sacroiliac joint (SIJ) involvement. We present a case of bilateral OCI with sclerosis based on radiological finding in a 30-year-old female presenting with chronic low back pain.

CT-guided transarticular biopsy of the sacroiliac joint: Technique and histomorphological results. A preliminary study.

OBJECTIVE: To introduce and evaluate computed tomography (CT)-guided transarticular needle biopsy of the cartilaginous sacroiliac joint (SIJ) and to assess the biopsy results microscopically. MATERIALS AND METHODS: The new CT-guided transarticular biopsy of the SIJ was performed in a young corpse and ten patients, two males and eight females aged 18-81 years. All patients had abnormal findings by magnetic resonance imaging (MRI) of the SIJs, including bone marrow edema, related to different types of joint disorders. The biopsies were focused on areas with bone marrow edema. The quality of the

Sacroiliitis mimics: a case report and review of the literature.

BACKGROUND: Radiographic sacroiliitis is the hallmark of ankylosing spondylitis (AS), and detection of acute sacroiliitis is pivotal for early diagnosis of AS. Although radiographic sacroiliitis is a distinguishing feature of AS, sacroiliitis can be seen in a variety of other disease entities. CASE PRESENTATION: We present an interesting case of sacroiliitis in a patient with Paget disease; the patient presented with inflammatory back pain which was treated with bisphosphonate. This case demonstrates comorbidity with Paget disease and possible ankylosing spondylitis. We also present a review

Clinical and imaging mimickers of axial spondyloarthritis.

The first step in the diagnosis of a patient with suspected axial spondyloarthritis is to differentiate the signs and symptoms of the disease from other disorders, potentially manifesting with similar clinical and imaging features. This review examines diffuse idiopathic skeletal hyperostosis, osteitis condensans ilii, and other developmental and metabolic disorders that may mimic axial spondyloarthritis, highlighting the diagnostic caveats and discussing shared and distinguishing aspects of these conditions in order to improve the clinician's ability to set them apart.

Osteitis condensans ilii may demonstrate bone marrow edema on sacroiliac joint magnetic resonance imaging.

AIM: To reveal whether bone marrow edema (BME) may appear with osteitis condensans ilii (OCI) on sacroiliac joint (SIJ) magnetic resonance imaging (MRI) and to evaluate the characteristics of BME demonstrated with OCI. METHOD: Altogether 27 symptomatic female OCI patients with both complete clinical records and SIJ-MRI examination data were enrolled for retrospective analysis. SIJ-MRI exams were retrospectively analyzed by two musculoskeletal radiologists. Patients were separated into BME group and non-BME group according to whether there was BME of SIJ demonstrated as high signal intensity o

MRI Findings of the Sacroiliac Joints in Patients with Low Back Pain: Alternative Diagnosis to Inflammatory Sacroiliitis.

BACKGROUND: Magnetic resonance imaging (MRI) is the most sensitive imaging modality for the detection of sacroiliitis. Diagnosing sacroiliitis on MRI is not always straightforward and can be challenging in some cases. OBJECTIVES: To evaluate the prevalence of alternative diagnoses suggested by MRI and characterize the MR appearance of the most common ones. METHODS: Consecutive MRI examinations of the sacroiliac joints (SIJ) performed between 2005 and 2012 were retrospectively evaluated for the presence of structural and active sacroiliitis findings according to the Assessment of SpondyloArth

Prevalence and awareness of sacroiliac joint alterations on lumbar spine CT in low back pain patients younger than 40 years.

Background Computed tomography (CT) examinations of the lumbar spine are commonly performed in patients aged ≤40 years due to low back pain (LBP). Purpose To investigate the prevalence and awareness of radiologists for the presence of structural post-inflammatory/other sacroiliac joint (SIJ) alterations on lumbar spine CTs of young patients with LBP. Material and Methods A total of 484 lumbar spine CT examinations (272 men, 212 women; average age, 31 years; age range, 18-40 years) of patients with LBP in which the entire SIJs were visualized were retrospectively reviewed. SIJs were scored (con

Refractory osteitis condensans ilii: outcome of a novel mini-invasive surgical approach.

PURPOSE: Osteitis condensans Ilii (OCI) is an orthopaedic mystery until now and the refractory type poses a great challenge in its management. Surgical resection and sacroiliac arthrodesis are major procedures with no guarantee of success for an unknown disease entity with a normal sacroiliac joint. The aim of this retrospective study was to evaluate results of a novel mini-invasive surgical approach for the refractory type after failure of conservative management. MATERIALS AND METHODS: Fourteen females were included with an average age 35.5 ± 5.8 years. Nine cases were multiparous and five

FDG uptake in sacroiliac joint due to osteitis condensans ilii shown on PET/CT in a patient with breast cancer: the value of coregistered CT in avoiding misinterpretation.

A 56-year-old woman with breast cancer underwent FDG PET/CT at follow-up. The PET images showed increased FDG uptake along right sacroiliac joint. The coregistered CT images showed diffuse sclerosis around the sacroiliac joints, but no bony destruction, periarticular erosion, or joint space narrowing. She had been complaining of intermittent lower back pain since her last pregnancy. The radiologic pictures and history of postpartum back pain were considered as typical characteristics for osteitis condensans ilii. This case reminds us that careful inspection of the coregistered CT images is imp

SPECT/CT of osteitis condensans ilii: one-stop shop imaging.

A 16-year-old, nonpregnant, healthy, and sportive teenager suffers from intermittent low back pain. Pelvic x-ray complemented by bone-SPECT/CT demonstrated an uncommon benign condition called osteitis condensans ilii. In the early phase, it is of paramount importance to distinguish osteitis condensans ilii from sacroiliitis or ankylosing spondylitis. This case report highlights the incremental value of performing one-stop shop hybrid SPECT/low-dose CT bone imaging in diagnosing and managing this rare benign skeletal condition.

Osteitis condensans ilii in differential diagnosis of patients with chronic low back pain: a review of the literature.

Osteitis condensans ilii (OCI) is a benign pathology causing chronic back and hip pain. Although the definitive cause is uncertain, mechanical stress is a significant factor in the development of the disease. Bilateral involvement of the sacroiliac joint is typical. We describe a case of unilateral OCI with unilateral sclerosis observed at radiography in a 34-year-old patient presenting with chronic back and hip pain, together with a review of the literature.

Comparative study on low back pain misdiagnosed as spondyloarthropathy.

This study aims to investigate features of different diseases with low back pain misdiagnosed as spondyloarthropathy so as to improve the accuracy of diagnosis for spondyloarthropathy. The clinical and laboratory data of 24 cases misdiagnosed as spondyloarthropathy in recent 3 years were comparatively and retrospectively analyzed. The diagnostic accuracy of the European Spondyloarthropathy Study Group (ESSG) criteria, Amor criteria, and the combination of them in these misdiagnosed cases was also analyzed. The final diagnoses of these 24 cases were listed below: four malignant tumors (retroper

Osteitis Condensans Ilii.

Osteitis Condensans Ilii (OCI) is a benign cause of axial low back pain. Although no clear etiology has been identified, the prevailing theory is that mechanical strain affects the auricular portion of the ilium and causes premature arthritis. The location of the sclerosis has been traditionally confined to the ilium and may give the false impression of sacro-iliac joint involvement. Clinicians must be guided by history, radiographic findings, and laboratory studies in differentiating OCI with other disorders; furthermore additional causes of low back pain including metastatic disease and anky

Sternoclavicular steroid injection for treatment of pain in a patient with Osteitis condensans of the clavicle.

Osteitis condensans of the sternoclavicular joint was first described by Brower et al in 1974. It is a rare benign disorder primarily affecting women of child-bearing age. Persistent pain and swelling in the medial part of the clavicle are the most common presenting symptoms and may represent an inflammatory process in the joint among other proposed etiologies. Radiological findings include sclerosis of the medial part of the clavicle and a normal sternoclavicular joint. Diagnosis is usually confirmed by biopsy. Pain management can be challenging in these patients. Multiple treatments have bee
