Odontogenic Sinusitis: The Selective Role of a Modified Caldwell-Luc Approach in Two CT/CBCT-Documented Cases.

Tooth-bearing structures are commonly involved in the spread of various odontogenic lesions. Radicular and dentigerous cysts are among the most common odontogenic cysts, exhibiting significant variations in size and morphology within the jawbones. On the other hand, the influence of dental caries, periodontal diseases, and periapical lesions extends beyond tooth-bearing bone and also affects adjacent anatomical structures, especially the maxillary sinuses. In addition to untreated odontogenic lesions, treatment-related complications in the posterior maxilla may also disturb the sinus environme

Aggressive Juvenile Trabecular Ossifying Fibroma in a 4-Year-Old: Case Report Bringing Diagnostic Challenges and Surgical Management.

Juvenile trabecular ossifying fibroma (JTOF) is an uncommon benign fibro-osseous neoplasm of the craniofacial skeleton that predominantly affects children and may clinically and radiographically resemble other aggressive lesions. We describe the case of a 4-year-old boy with a rapidly enlarging, painless maxillary mass. Imaging revealed a well-defined expansile lesion with internal radiopaque foci and ground-glass attenuation, extending into the nasal cavity and orbit. Histopathology showed immature trabecular bone with osteoblastic rimming in a fibroblastic stroma, along with aneurysmal bone

An Uncommon Cause of Cystic Lung Disease.

A 59-year-old female with Sjögren's syndrome presented with a three-year history of dyspnea and cough beginning after COVID-19 infection. Serial chest computed tomography scans over a course of three years revealed multiple pulmonary cysts and nodules, including one that enlarged over time. Pulmonary function was largely preserved, aside from a mildly reduced diffusing capacity, while laboratory studies showed hypergammaglobulinemia and elevated kappa light chains. After a nondiagnostic bronchoscopic transbronchial biopsy, she underwent thoracoscopic wedge resection. Pathology revealed eosinop

Proximal Tibial Giant Cell Tumor in a Skeletally Immature Child: A Diagnostic Dilemma and Review of the Literature.

INTRODUCTION: Giant cell tumor (GCT) of bone is a benign, locally aggressive neoplasm that primarily involves the epiphysis of long bones in skeletally mature individuals. It is extremely rare in skeletally immature patients. We report a rare case of proximal tibial GCT in a 7-year-old girl with a pathological fracture and metadiaphyseal extension, highlighting diagnostic considerations and long-term outcomes. CASE REPORT: A 7-year-old girl presented with pain, swelling, and deformity of the left upper leg following trivial trauma. Radiographs and non-contrast computed tomography scan demonst

Diagnostic and Therapeutic Approach to External Frontal Lesions: A Report of Two Cases.

Benign frontal masses represent a diagnostic and therapeutic challenge due to their aesthetic impact and the wide range of possible differential diagnoses. We present a comparative case series of two patients with benign frontal masses: a frontal lipoma and a frontal osteoma. Both cases highlight the importance of careful clinical evaluation, appropriate use of imaging studies, and tailored surgical planning to achieve optimal functional and aesthetic outcomes. Frontal osteomas are benign, slow-growing bone tumors arising from the cranial vault and represent the most common benign tumors of th

An Odontogenic Cutaneous Sinus Tract Mimicking a Mandibular Neoplasm: Diagnostic Challenges and Radiologic Correlation.

Odontogenic cutaneous sinus tracts (OCSTs) are an uncommon consequence of chronic dental infection in which inflammatory drainage extends from a periapical source through bone and soft tissues to the skin surface. Although odontogenic infections typically present with intraoral symptoms such as pain, swelling, or vestibular drainage, chronic lesions may occasionally manifest as extraoral nodules, cysts, or draining sinuses. Because many patients experience little or no dental discomfort, these lesions are frequently misdiagnosed as dermatologic, infectious, or neoplastic conditions. The report

Large Pleomorphic Adenoma of The Upper Lip in a Middle-Aged Woman With Facial Deformity, Tooth Displacement, and Bone Loss: Uncommon Clinical Presentation of a Benign Tumor.

Pleomorphic adenoma (PA) is the most common benign neoplasm of the salivary glands, occurring in both major and minor glands, with a predilection for the intraoral palate. Its presentation in the upper lip is rare and, when large, may result in facial deformity, alveolar bone resorption, and tooth displacement, especially in long-standing lesions. Clinically, PA presents as a solid, painless, slow-growing, and well-circumscribed mass, which may cause functional morbidity depending on its size and location. The differential diagnosis includes canalicular adenoma, other benign minor salivary gla

Radiographic reduction following decompression of a dentigerous cyst and an odontogenic keratocyst: A comparative case report.

Odontogenic cysts are well-known maxillofacial pathologies with variable biological behaviour. Decompression has been widely used as a conservative, structure-preserving management strategy. This report presents a radiographic comparison of reduction patterns following decompression in two individual cases, one dentigerous cyst and one odontogenic keratocyst. Two patients presenting with cystic jaw lesions - one detected during radiographic investigation and the other presenting with progressive swelling - were managed using decompression as the initial treatment. Standardised radiographic mea

"Dry Tap" Fine-Needle Aspiration Biopsy as a Diagnostic Clue in Cyst-like Juvenile Jaw Lesions Mimicking Dentigerous Cysts on Panoramic Radiography and Cone-Beam Computed Tomography.

Pediatric odontogenic tumors are rare but are frequently overlooked because they often mimic simple cysts on routine radiographic examinations. The radiographic appearance on panoramic imaging and cone-beam computed tomography (CBCT) frequently does not correlate with the true biological nature of these lesions. On CBCT, classic odontogenic tumors often demonstrate mixed radiolucent-radiopaque patterns with ill-defined borders, internal calcifications, septations, or other structural features. The diagnostic challenge arises when an odontogenic tumor mimics a unilateral, well-defined radioluce

Decompression Tube Fixation on Adjacent Teeth for Cystic Lesions in the Jaws: A Long-Term Follow-Up Analysis of Predictive Factors.

Introduction Decompression is an effective conservative treatment for large odontogenic cysts; however, conventional tube stabilization often encounters issues such as device dislodgement or mucosal trauma. This study aimed to analyze the therapeutic efficacy and clinical outcomes of a novel decompression tube fixation method secured directly to adjacent teeth using stainless steel wire. Materials and methods We retrospectively reviewed 20 patients (11 males, 9 females; mean age: 41.5 years) with jaw cysts treated between 2013 and 2023. Quantification of cyst dimensions and two-dimensional (2D

Diagnosis of nasopalatine duct and nasopalatine duct cyst in CBCT images: a radiomics-based machine learning approach.

OBJECTIVES: This study aimed to evaluate the diagnostic performance of machine learning (ML) algorithms based on radiomic features extracted from cone-beam CT (CBCT) images in differentiating the nasopalatine duct (NPD) from the nasopalatine duct cyst (NPDC), and to compare their performance with that of a dentomaxillofacial radiologist. METHODS: CBCT scans from 101 histopathologically confirmed NPDC cases and 101 age- and sex-matched controls with normal NPD were retrospectively analysed. Manual segmentation was performed to extract 1037 radiomic features (original, Laplacian of Gaussian, an

Clinical and genetic findings in 103 individuals in Norway with basal cell naevus syndrome.

BACKGROUND: Basal cell naevus syndrome (BCNS) is a rare autosomal dominant condition characterized by early onset and multiple basal cell carcinomas (BCCs), skeletal and ocular abnormalities, and extracutaneous tumours and cysts. In most cases, BCNS is caused by a pathogenic variant in the tumour suppressor gene PTCH1. Pathogenic variants in SUFU account for a minority of cases. No clear genotype-phenotype associations between different PTCH1 variants have been established. OBJECTIVES: To identify all individuals with BCNS in Norway, characterize their clinical and genetic profiles, and inves

Two secondary aneurysmal bone cysts associated with mandibular fibrous dysplasia: a rare case and literature review.

INTRODUCTION AND IMPORTANCE: The aneurysmal bone cyst (ABC) is an expansive, destructive, non-odontogenic, and benign bone lesion characterized by spaces filled with blood, typically affecting the elongated bones and spine, with a low incidence in the maxillofacial region. The pathogenesis may occur due to prior or pathological trauma, arteriovenous malformation, secondary association with a pre-existing bone process, such as giant cell tumor, ossifying fibroma, and bone dysplasia, or even genetic alterations. Managing an ABC involves different methods. The preferred treatment is surgical rese

Double-way drainage device for cystic decompression: a technical note.

Decompression is a conservative treatment approach, either definitive or adjuvant, to jaw cysts (of odontogenic and non-odontogenic origin). It relieves intracystic pressure, which allows size decrease of the pathology concomitant to bone neoformation. Several devices have been described with the purpose of decompressing a lesion, which attempted to fulfil ideal characteristics, but many have displayed disadvantages, such as the impossibility of use in edentulous patients, being prone to contamination, clinical discomfort, etc. Thus, the objective of this technical note is to present a new app

Pediatric nasopalatine vs. median palatine cysts: differential diagnosis and endoscopic management - a comparative case report.

BACKGROUND: Median palatine cysts (MPCs) and nasopalatine duct cysts (NPDCs) are rare non-odontogenic cystic lesions in the pediatric population. Due to their low incidence and often asymptomatic nature in children, these cysts are frequently overlooked or misdiagnosed. To the best of our knowledge, few comparative pediatric cases have been reported. The novelty lies in the detailed histopathological differentiation and the analysis of their impact on maxillofacial development, providing a crucial clinical reference for early intervention. CASE PRESENTATION: In this report, two pediatric pati

Analysis of CD4+ and CD8+ T Lymphocytes and Expression of Immune Checkpoint Proteins (PD-1 and PD-L1) in Primary and Persistent Periapical Lesions.

INTRODUCTION: Persistent periapical lesions (PePL) represent a clinical challenge because of their inadequate response to conventional endodontic therapies. Studies indicate differences in immunoinflammatory mechanisms between primary periapical lesions (PrPL) and PePL but the findings remain conflicting. This study aimed to evaluate CD4+ and CD8+ T lymphocytes and the expression of immune checkpoint proteins (programmed cell death protein 1 [PD-1] and its ligand [PD-L1]) in PrPL and PePL, correlating the findings with the intensity of the inflammatory infiltrate and pattern of the cystic epit

A Novel Approach: Tube-Based Bridging Drainage for Fenestration Decompression of Cysts at the Posterior Border of the Mandibular Ramus.

Odontogenic jaw cysts are common intraosseous lesions with a high incidence and insidious onset, often resulting in delayed diagnosis and presentation as large cysts requiring a two-stage approach: initial fenestration decompression to reduce cyst volume, followed by secondary surgical enucleation. However, cysts in anatomically challenging locations are not always amenable to conventional fenestration. This article reports a case of an odontogenic cyst located at the posterior border of the mandibular ramus, a site posing significant surgical challenges. The cyst was deeply situated, making c

Atypical Nasopalatine Duct Cyst in a Grafted Alveolar Cleft: Radiographic Diagnostic Challenges.

Nasopalatine duct cysts may present with atypical radiographic features when arising in grafted alveolar cleft regions. A patient with cleft lip and palate presented with a well-defined radiolucent lesion in the anterior maxilla following alveolar bone grafting. Imaging demonstrated a unilocular lesion associated with displacement of the incisive canal, mimicking other odontogenic and nonodontogenic cysts. The unusual location and surgical history posed diagnostic challenges. Local trauma related to grafting may have contributed to cyst development. Careful radiographic assessment is essential
