Molecular Approaches to Diagnosis in Ewing Sarcoma: Targeted RNA Sequencing.

Molecular testing for pathognomonic gene fusions is mandatory for the diagnosis of small round cell tumors. In particular, in Ewing sarcoma, the identification of specific chimeric fusion genes is critical to distinguish it from other histologically similar subtypes. As the clinical relevance of gene fusion detection continues to grow, many laboratories have adopted targeted next-generation sequencing approaches to overcome the limitations of conventional singleplex assays. We were among the early adopters of targeted RNA sequencing using anchored multiplex PCR, a technique that enables the si

Fast Analysis of RNA from Transcription and Rho-Dependent Termination Assays Using Automated Capillary Electrophoresis.

Conventional analysis of RNA products synthesized by bacterial RNA polymerase (RNAP) in in vitro transcription assays relies on radioactive labeling, followed by separation of labeled RNAs by denaturing urea-polyacrylamide gel electrophoresis (PAGE). This approach provides excellent resolution across a wide range of RNA lengths (from 2 nt to several hundred nucleotides), but the use of radioisotopes requires specialized facilities, strict waste-disposal procedures, and is often restricted. To circumvent these limitations, we adapted a standard in vitro transcription assay for use with the Agil

Enumeration of Benign and Malignant Plasmacytoid Dendritic Cells in Peripheral Blood and Bone Marrow by Flow-Cytometric Analysis.

Dendritic cells (DCs) are powerful antigen-presenting cells involved in regulating immune response. Plasmacytoid dendritic cells (pDCs) are a subtype of DCs present in small quantities in the bone marrow, peripheral blood, and lymph nodes. They are an important component of our immune system's regulation and response to diseases. They activate T cells and play a critical role in immune tolerance. In this chapter, we review the immunophenotypic features of pDCs and provide a practical protocol for pDCs enumeration in the peripheral blood and in bone marrow samples.

State of the global tuberculosis epidemic: burden, progress towards WHO End TB Strategy targets and systemic challenges.

Mycobacterium tuberculosis remains the leading cause of death from a single infectious pathogen globally despite decades of effective chemotherapy. In 2024, an estimated 10·7 million people developed tuberculosis, including approximately 620 000 people living with HIV (PLHIV), and tuberculosis caused an estimated 1·23 million deaths overall, including approximately 150 000 deaths among PLHIV. Men accounted for more than half of the cases, and children represented a substantial burden, reflecting ongoing transmission and diagnostic gaps. Approximately a quarter of the world's population has bee

Minimal supply chain and forecasting testing volumes for clinical bacteriology laboratories in resource-limited settings for individual patient care: a costing study.

BACKGROUND: Inadequate access to clinical bacteriology testing in low-resource settings compromises individual patient care and contributes to antimicrobial resistance. Funders, policy makers, and health administrators working to expand bacteriology testing in sub-Saharan Africa require evidence-based guidance. The aim of this study was to assemble a costed consensus list of minimum equipment and consumables required to implement and operate a functional clinical bacteriology laboratory in secondary and tertiary care hospitals in sub-Saharan Africa, to guide funders and health administrators.

Advances in tuberculosis diagnostics and biomarkers, 2020-26: scientific evidence, recommendations, gaps, and developmental priorities.

Major advances in tuberculosis diagnostics and biomarker research from 2020 to 2025 have informed updated WHO policy guidance, consolidating recommendations for the detection of tuberculosis disease and drug resistance within a single framework. These recommendations align screening, diagnosis, and drug-resistance testing across populations and specimen types. Digital chest radiography, increasingly supported by computer-aided detection, is now a core WHO-recommended screening and triage tool, improving case detection and referral for confirmatory testing at scale. Decentralised molecular diag

Advances in development of tuberculosis vaccines and preventive drug therapies: current pipeline, evidence, and access.

Tuberculosis remains the leading cause of death from a single infectious agent globally. The WHO Global Tuberculosis Report 2025 indicates that progress towards the WHO End TB Strategy targets remains off track despite modest declines in incidence and mortality. In 2024, an estimated 10·7 million people developed tuberculosis and 1·23 million died from the disease. In this Series paper, we examine evidence published between Jan 6, 2020, and Jan 6, 2026, to provide an updated overview of tuberculosis vaccines and tuberculosis preventive treatment (TPT). As of June 4, 2026, 18 tuberculosis vacci

Advances in tuberculosis treatment: novel regimens, new drug pipelines, host-directed therapies, and updated management guidelines.

Tuberculosis treatment has undergone its most profound transformation since the launch of the standardised DOTS strategy in 1994. Since 2020, a series of pivotal randomised trials, including TB-PRACTECAL, ZeNix, Nix-TB, endTB, BEAT-TB, SHINE, and Study 31/A5349, have redefined the management of both drug-susceptible and drug-resistant tuberculosis. These studies have enabled shorter, fully oral regimens with improved efficacy and safety across adult and paediatric populations, including people with HIV, and have driven major updates to WHO treatment guidelines. Despite these advances, tubercul

Updated Protocols for 3D and 2D Culture and Histological Analysis of Intestinal Organoids.

The intestinal epithelium maintains continuous self-renewal and multi-differentiation capacities through the integrated regulation of multiple signaling pathways, including Wnt, bone morphogenetic protein, epidermal growth factor (EGF), and Notch signaling pathways. Based on this principle, a defined combination of EGF, Noggin, and the Wnt agonist R-spondin has been shown to support long-term expansion of mouse intestinal stem cells and the formation of organoids with indefinite self-renewal and full differentiation capacity. In human intestinal organoid cultures, the addition of small molecul

Diagnostic performance and testing costs of pooled testing for molecular detection of tuberculosis in seven countries: a prospective, observational, diagnostic accuracy study.

BACKGROUND: Globally in 2024, only 54% of people diagnosed with tuberculosis received an initial WHO-recommended rapid molecular diagnostic test. Pooled testing could expand access to molecular detection using existing laboratory capacity. We aimed to evaluate the performance of pooled tuberculosis testing in a multicentre diagnostic accuracy study. METHODS: In this prospective diagnostic accuracy study, we enrolled individuals with presumptive tuberculosis through facility-based case finding (district hospitals and primary health centres) and community-based case finding in Bangladesh, Brazi

Preclinical evaluation of 68Ga-labelled Pt(IV)-DFO complexes as PET-imaging agents for osteosarcoma.

Pt (IV) complexes have emerged as promising redox-activated prodrugs with improved pharmacological properties compared to Pt(II) and the potential for functionalising additional binding sites. In this study, four Pt(IV)-deferoxamine (DFO) complexes (A-D) were evaluated as gallium-68 radiolabelled imaging agents in healthy mice and in a murine osteosarcoma model using positron emission tomography (PET). All complexes presented quantitative 68Ga-labelling yields with high radiochemical purity (>95%) and excellent stability in aqueous buffer and human serum after 1h incubation. In vivo evaluation

Liposclerosing myxofibrous tumor of the proximal femur: a case report.

Liposclerosing myxofibrous tumor (LSMFT) is a rare fibro-osseous lesion with a strong predilection for the proximal femur, rarely seen in clinical practice and often diagnostically challenging. A 29-year-old woman who presented with left hip pain after a fall is reported. Multimodal imaging - plain radiographs, computed tomography, and magnetic resonance imaging (MRI) - was performed, followed by intraoperative frozen section analysis and curettage. The residual cavity was filled with a synthetic bioactive-glass bone substitute, and the proximal femur was stabilised with a dynamic hip screw (D

Population-based genomic screening of FGFR3 reveals variant spectrum, disease burden and clinical outcomes in 39,104 newborns.

BACKGROUND: Fibroblast growth factor receptor 3 (FGFR3) mutation is classically known to cause a spectrum of autosomal dominant skeletal dysplasias through gain-of-function mechanisms. However, because most genetic studies are limited to symptomatic individuals or high-risk families, the unbiased variant carrier frequency, variants spectrum, and disease prevalence in newborns remain largely uncharacterized. METHODS: We performed high-throughput targeted next-generation sequencing of the FGFR3 gene on 39,104 Chinese newborns from a multi-center cohort. Variants were classified according to the

Genetic evidence linking the promoter region and 5'-UTR deletion of GNAS with pseudohypoparathyroidism type 1 C.

BACKGROUND: Pseudohypoparathyroidism (PHP) is a heterogeneous disorder caused by genetic or epigenetic changes at the GNAS locus on chromosome 20q13.3 leading to impaired cyclic adenosine monophosphate (cAMP)-dependent signaling pathway via the alpha-subunit of the stimulatory G protein (Gsα). Unlike PHP type 1 A (PHP1A), which occurs due to GNAS variant and low Gsα activity, the activity of Gsα is normal in PHP type 1 C (PHP1C) despite the same phenotypic characteristics as PHP1A. However, the genetic cause of PHP1C is unknown. METHODS: The likely causative variant of PHP1C was investigated

MINDIN-Induced Attenuation of Primary Cilia Activates Parathyroid Hormone Receptor Type I (PTH1R) to Drive Osteomimicry and Bone Tropism in Prostate Cancer.

Advanced prostate cancer metastasizes primarily to bone, a process driven in part by the de novo expression of bone-associated factors (osteomimicry) that enables tumor cells to colonize and thrive in the skeletal environment. During the early stages of tumorigenesis in multiple cancer types, the primary cilium, a non-motile microtubule-based sensory organelle, is lost. However, the causes of primary cilium loss and the consequences of this process for tumor progression are still ill-defined. The aim of this study was to determine whether MINDIN, a protein secreted by prostate tumor cells, pro

Case Report: Distal tibial physeal injury secondary to conservative treatment of acute hematogenous osteomyelitis resulting in ankle deformity - highlighting the necessity of regular follow-up.

Ankle deformities most commonly result from physeal injuries following major trauma; those secondary to occult physeal injury without significant antecedent trauma are rarely documented. We report a case of ankle deformity in a 13-year-old boy resulting from distal tibial physeal injury following conservative treatment for acute hematogenous osteomyelitis(AHO) four years earlier; despite initial clinical improvement, the patient was subsequently lost to follow-up. At his return visit, significant ankle discomfort severely compromised daily activities. Following combined distal tibial osteotomy

PF127 hydrogel-assisted in-situ Staphylococcus aureus biofilm fabrication endowing pathological animal model for chronic osteomyelitis.

Chronic osteomyelitis driven by biofilms remains difficult to treat and thus the exploration of its clinical therapy and pathological mechanism largely depend on effective animal models. However, standardized and efficient methods for establishing stable and reproducible animal models for chronic osteomyelitis are still lacking. This study developed a Hydrogel Assisted In-situ Biofilm Fabrication (HAIBF) method by coupling thermosensitive Pluronic F-127 (PF127) hydrogel with biofilm fragments, which stably mimic the pathological process of biofilm-infected chronic osteomyelitis. Novel Insights into the Biochemistry of Citrate and Its Transporters: Relevance to Metabolic Rewiring, Tumor Microenvironment, and Bone Metastasis in Cancer.

Citrate intersects multiple biochemical pathways, yet its role in cancer has received limited attention. Similarly, the Na+-coupled citrate transporter (SLC13A5) has been investigated extensively in connection with epileptic encephalopathy, but its relevance to cancer remains understudied. This review focuses on novel insights into citrate biology and SLC13A5 that suggest a tumor-promoting role for SLC13A5/citrate. Cytosolic citrate is the building block for fatty acid and cholesterol synthesis necessary for membrane biogenesis and serves as a signaling molecule in regulating mitophagy and pro
