Managing multidrug-resistant gram-positive infections is challenging, especially when prolonged therapy is needed for conditions like bacteremia, endocarditis, osteomyelitis, or other deep-seated infections. Oritavancin, a long-acting lipoglycopeptide, is increasingly used in these scenarios and offers an alternative to central catheter placement. However, despite its growing use for extended treatment, optimal dosing strategies and pharmacokinetic targets remain unclear. An in silico pharmacokinetic/pharmacodynamic simulation was performed to assess the predicted oritavancin concentration res

Acute transverse myelitis (ATM) is a rare, acquired neuroimmune disorder of the spinal cord that manifests with acute-onset weakness, sensory alterations, and autonomic dysfunction. We report a case of a 56-year-old immunocompetent female patient who presented with fever, flaccid quadriparesis, nuchal rigidity, and markedly elevated inflammatory biomarkers. Blood cultures came back positive for methicillin-sensitive Staphylococcus aureus, and magnetic resonance imaging (MRI) of the cervical spine demonstrated increased T2 signal between C4 and C7, inflammation of the anterior perivertebral sof

Doxycycline is a widely used antibiotic for commonly encountered infections and is generally well tolerated. However, it is associated with rare, drug-induced severe thrombocytopenia, which, if not promptly recognised and managed, could prove fatal. We present a case of doxycycline-induced severe thrombocytopenia in a 63-year-old man with a background of type 2 diabetes mellitus, left ventricular systolic dysfunction, and hypothyroidism. The platelet count dropped to a critical level of 6 x 10⁹/L following a one-week course of doxycycline, which improved with drug withdrawal, platelet transfus

Staphylococcus aureus is one of the main causes of osteomyelitis. The problem of antibiotic resistance is particularly acute in osteomyelitis because, in most cases, S. aureus forms biofilms in which the antibiotic sensitivity of bacteria is significantly reduced, and more than 50% of osteomyelitis cases are associated with methicillin-resistant S. aureus (MRSA). One of the promising approaches for surgical treatment of osteomyelitis caused by S. aureus may be one-stage replacement of the affected fragment with a spongy scaffold that provides both a bactericidal effect, particularly in relatio

A 74-year-old man with type 2 diabetes presented with fever, urinary retention, and urinary difficulties. Initial abdominal Computed Tomography (CT) suggested acute pyelonephritis, but a low-density area in the prostate was overlooked. Following the confirmation of methicillin-resistant Staphylococcus aureus (MRSA) in blood and urine cultures, comprehensive screening for metastatic abscesses was necessitated. Diffusion-weighted whole-body imaging with background suppression (DWIBS) was utilized and clearly identified a prostatic abscess (PA), nephritis, urethritis, and subcutaneous cysts. Thes

Deep bone abscesses are a rare complication following a pin tract infection. This article shows the importance of early detection, surgical management, and microbiology-guided antibiotic therapy to treat the abscess. We aim to highlight the key presenting factors, the radiological signs, and intraoperative features of a deep bone abscess, to facilitate detection in these rare circumstances. Closed or open reduction and internal fixation with Kirschner wires (K-wires) has become the standard of care for displaced supracondylar fractures. Although pin tract infection (PTI) is the most common com

INTRODUCTION AND AIM: Osteomyelitis is an infection of the bone that typically requires hospitalization for diagnosis and initial treatment. For many years, it has been assumed that osteomyelitis must be treated with intravenous (IV) antibiotics for the full course of therapy. However, recent data have demonstrated the efficacy of oral (PO) antibiotics. In addition, studies comparing the efficacy of IV versus PO antibiotics in children with osteomyelitis have found that there was no increased incidence of treatment failure when patients were transitioned to PO antibiotics. The Infectious Disea

Septic arthritis of the sternoclavicular joint (SCJ) is an uncommon location of septic joint infection and is often a diagnostic challenge. Patients may present with insidious, vague symptoms, such as chest pain or shoulder pain, and imaging modalities are often nonspecific, leading to a delay in diagnosis and treatment. We present a case of a young male patient initially presenting with right shoulder pain, but who then returned days later and was diagnosed with SCJ septic arthritis. This patient was treated successfully with SCJ resection and intravenous antibiotics. This case illustrates th

UNLABELLED: Seymour fractures are pediatric distal phalanx physeal injuries of the fingers and toes associated with disruption of the nail bed. Despite increasing recognition, they are still frequently misdiagnosed, leading to delayed treatment and preventable morbidity. This review synthesizes contemporary literature on the definition, clinical and radiographic diagnosis, differential diagnoses, management, and outcomes of Seymour fractures. Diagnosis relies on maintaining a high index of suspicion in skeletally immature patients presenting with distal interphalangeal (DIP) joint flexion, ble

Osteomyelitis, defined as inflammation of bone, is a highly morbid disease that is most commonly caused by bacterial infection. Infectious osteomyelitis occurs across a spectrum of ages and affects previously healthy individuals as well as individuals with medical comorbidities. Establishment of osteomyelitis typically occurs following one of three general pathological mechanisms, including haematogenous seeding of healthy or injured bone, extension of a contiguous soft tissue infection, or direct inoculation of microorganisms into the bone following trauma. A wide spectrum of microbial pathog

Septic arthritis is an orthopedic emergency requiring prompt diagnosis and treatment to prevent irreversible joint damage. Staphylococcus aureus is the most common causative organism in pediatric septic arthritis. In contrast, Salmonella species are rare etiological agents, particularly in immunocompetent children without predisposing conditions. We report two cases of Salmonella-induced septic arthritis in healthy children. The first case involved an 11-month-old girl with acute right ankle septic arthritis. The second case involved a 15-month-old boy with subacute right hip septic arthritis

BACKGROUND: Chronic nonbacterial osteomyelitis (CNO) is a rare inflammatory disorder that commonly causes bone destruction in the metaphysis of long bones, pelvic and vertebral bones, the clavicle, and the mandible. CNO is often misdiagnosed and underreported. This presentation of debilitating back pain caused by CNO showcases diagnostic challenges and potential for novel therapeutic treatment.
CASE DESCRIPTION: A 14-year-old male with Crohn's disease presented with acute, worsening lower back pain and two months of hematochezia after recently stopping infliximab. Physical examination showed

Treatment of avian malocclusion, or "scissor beak," is well-described in the literature, but reports have focused on early intervention of acute traumatic and congenital cases, and surgical correction of chronic mandibular malunion are rare. This report describes the diagnostic investigation, surgical correction, and long-term follow up of a 3-month-old Ameraucana rooster (Gallus gallus domesticus) with chronic beak malocclusion. Computed tomography was used to confirm the unilateral mandibular fracture malunion and guide surgical planning, which involved creation of a custom osteogenic distra

AIM: Despite status as a high-income nation, New Zealand (NZ) experiences the second highest rate of childhood bone and joint infection (BJI) globally with a disproportionate burden in Māori and Pacific children. This study aims to identify potentially modifiable risk factors for childhood BJI.
METHODS: An age and ethnicity matched case-control design was utilised to compare acute cases of BJI with healthy controls recruited from orthopaedic fracture clinics. Cases were </= 15 years hospitalised because of acute haematogenous osteomyelitis or septic arthritis across multiple centres in the No

Puncture wounds are common injuries that may lead to infection. Diabetic patients are at higher risk for serious infections, including osteomyelitis. Osteomyelitis is an infection of the bone that requires a combination of surgical and antibiotic therapy. X-ray findings of osteomyelitis include demineralization, periosteal reaction, and bony destruction. This case follows a 62-year-old woman with a history of uncontrolled type 2 diabetes mellitus who presented to the emergency department (ED) for left foot pain after she stepped on a nail that punctured her foot through her shoe. Four days bef

Staphylococcus lugdunensis is a virulent coagulase-negative Staphylococcus that can cause invasive bloodstream and deep-seated infection, behaving more like Staphylococcus aureus than commensal skin flora. A 73-year-old patient with chronic low back pain underwent bilateral lumbar medial branch and sacroiliac joint radiofrequency ablation. Over seven weeks, they developed progressively worsening low back pain with bilateral radicular symptoms, saddle anaesthesia, constipation, and acute urinary retention, without fever. C-reactive protein was markedly elevated with a normal white cell count an

Transverse myelitis is a rare, acquired inflammatory neurologic disorder that can result in chronic disability and secondary complications, including decubitus ulcers. These ulcers may serve as portals for infection and can progress to osteomyelitis, particularly when involving high-pressure areas. We present the case of a 45-year-old female with chronic transverse myelitis who developed a stage 4 perineal, ischial, and femoral decubitus ulcer complicated by acute-on-chronic osteomyelitis of the pelvis and femur. The patient underwent a left lower extremity amputation with preservation of the

Vertebral osteomyelitis and spondylodiscitis are uncommon but potentially life-threatening infections that may present with non-specific symptoms, resulting in delayed diagnosis. We report the case of a 65-year-old man who presented with acute low back pain, high-grade fever, confusion, and transient neurological symptoms. Initial investigations demonstrated markedly elevated inflammatory markers and persistent methicillin-sensitive Staphylococcus aureus (MSSA) bacteremia. A CT of the head and thorax/abdomen/pelvis, transthoracic echocardiography, and an early whole-spine MRI study failed to i
