Bilateral Cavernous Sinus Thrombosis and Clival Osteomyelitis Complicating Pediatric Otitis Media.

Central skull base osteomyelitis (SBO) and associated neurovascular complications represent critical clinical conditions that necessitate prompt diagnosis and aggressive therapeutic intervention. We report the case of a previously healthy four-year-old boy who presented with a six-day history of purulent left otorrhea, fever, headaches, and the acute onset of left convergent strabismus. Contrast-enhanced CT revealed left-sided acute otitis media complicated by bilateral cavernous sinus thrombosis, right ophthalmic vein thrombosis, left sigmoid sinus and internal jugular vein thrombosis, and an

Probable Tigecycline-Induced Acute Eosinophilic Pneumonia: A Case Report.

A 25-year-old man developed acute hypoxaemic respiratory failure after 21 days of tigecycline therapy for osteomyelitis caused by multidrug-resistant organisms. Chest computed tomography demonstrated bilateral consolidations, ground-glass opacities, interlobular septal thickening, small bilateral pleural effusions and mediastinal lymphadenopathy. Bronchoalveolar lavage (BAL) revealed marked eosinophilia (99%-100%) despite minimal peripheral eosinophilia. Alternative causes, including infectious, parasitic, autoimmune and other potential aetiologies, were carefully evaluated. Based on the tempo

Adjunctive hyperbaric oxygen combined with surgery and antifungal therapy for the management of soft tissue mucormycosis: a case report and review of the literature.

Soft tissue mucormycosis is a rare fungal infection that can develop on contaminated wounds. We report a case of Mucor circinelloides ankle infection in an immunocompetent man following a high-energy motorcycle accident, which has successfully been treated by surgery (multiple debridements, arthrodesis, soft tissue defect reconstruction with flap and skin graft), systemic antifungal therapy (initial liposomal amphotericin B followed by isavuconazole) and hyperbaric oxygen. This case highlights the therapeutic challenges and need for a multidisciplinary approach of such infection.

Calciphylaxis Mimicry in End-Stage Renal Disease: Acute Limb Ischemia From Calcific Peripheral Arterial Disease.

Calciphylaxis is a life-threatening complication of end-stage renal disease (ESRD) characterized by painful necrotic skin lesions resulting from small-vessel (arteriolar) calcification, thrombosis, and subsequent ischemic tissue necrosis. However, not all ischemic lesions in dialysis patients represent calciphylaxis. We present a 44-year-old man with ESRD on hemodialysis who developed progressive bilateral digital necrosis initially suggestive of calciphylaxis, given severe hyperphosphatemia and secondary hyperparathyroidism. Lesion morphology demonstrated well-demarcated distal gangrene witho

[Complications of acute otitis media].

Complications of acute otitis media (AOM) are rare but can occur intratemporally or intracranially. Clinically relevant mastoiditis usually presents with subperiosteal abscesses, whereas subclinical opacification of the mastoid cells is common but does not require specific treatment. Other potential complications include labyrinthitis, facial nerve palsy, intratemporal abscess, cerebral venous sinus thrombosis, or meningitis. Diagnosis relies on clinical examination, laboratory tests, and targeted imaging. Intratemporal complications are primarily treated with antibiotics. Surgical interventio

Rare Invasive Fusarium Sinusitis in a Transiently Immunosuppressed Patient.

A Florida cattle rancher undergoing treatment for lumbar osteomyelitis presented with severe neutropenia and febrile episodes after being discharged on a seven-week course of intravenous (IV) vancomycin and cefepime. Upon admission, antibiotics were changed, and micafungin and filgrastim were added. Several days later, he had a normalizing absolute neutrophil count (ANC) and improving fevers with an overall good clinical appearance. Discharge seemed near, but he continued to complain of nasal congestion and mild oromaxillary pain. In caution, a CT maxillofacial scan and ENT evaluation were per

Multisystemic Imaging Features of Coccidioidomycosis.

Coccidioidomycosis, a disease induced by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to the southwestern United States and occurs with a myriad of clinical and imaging manifestations, which range from limited pulmonary disease to disseminated multiorgan infections. Humans are infected when arthroconidia in the soil are disturbed and dispersed into the air. Once inhaled, arthroconidia form spherules, which trigger a host cell-mediated immune response. If this response is adequate, it results in granuloma formation, halting the infection. However, an insuffici

Shifts in unidentified-pathogen acute hematogenous osteomyelitis incidence after nonpharmaceutical interventions highlight the role of seasonal viruses: An interrupted time-series analysis.

OBJECTIVES: To analyse the temporal association of COVID-19-related nonpharmaceutical interventions (NPIs) with the incidence of unidentified-pathogen acute hematogenous osteomyelitis (UP-AHO) in children aged 1-5 years, and to assess the contribution of seasonal viral circulation. METHODS: We conducted an interrupted time-series analysis from 2015 to 2023. Monthly UP-AHO incidence per 100,000 children was analyzed using a quasi-Poisson regression model, considering seasonal viruses and their attributable fraction. RESULTS: Overall, 5619 children were hospitalized for UP-AHO (median age: 2 y

Paediatric septic thrombophlebitis secondary to an otogenic infection: a single-centre retrospective case series.

UNLABELLED: Paediatric septic thrombophlebitis secondary to head and neck infections is a rare but life-threatening disease. Although commonly associated with oropharyngeal infections, otogenic cases have also been described, yet paediatric literature on their disease course and thrombotic distribution remains sparse. To describe the clinical presentation, thrombotic distribution, management, and outcomes of paediatric septic thrombophlebitis secondary to primary otogenic infections. We conducted a retrospective case series of children aged 0-18 years admitted to a tertiary paediatric referral

The Management and Outcomes of Staphylococcus aureus Osteoarticular Infections Associated With Prolonged Bacteremia in Children.

INTRODUCTION: Staphylococcus aureus is the predominant cause of acute hematogenous osteoarticular infection (AHOAI, osteomyelitis or bacterial arthritis) in children and is commonly associated with bacteremia. Current guidelines suggest that persistent bacteremia may warrant more aggressive treatment. We evaluated the outcomes of children with S. aureus AHOAI with respect to bacteremia duration. METHODS: Children ≤18 years old with AHOAI secondary to S. aureus from 2011 to 2021 were identified through prospective surveillance studies at Texas Children's and St. Louis Children's Hospitals. Acute Necrotizing Pancreatitis Presenting With Extensive Unilateral Lower Limb Myositis, Fasciitis, and Sterile Reactive Knee Synovitis: A Rare Extra-pancreatic Manifestation.

Musculoskeletal manifestations of acute pancreatitis are uncommon and are most frequently described as pancreatic panniculitis or pancreatic panniculitis-polyarthritis (PPP) syndrome. Diffuse inflammatory myositis with fascial involvement is exceedingly rare and may mimic cellulitis, pyomyositis, septic arthritis, necrotizing fasciitis, or deep venous thrombosis, leading to diagnostic uncertainty and potentially unnecessary surgical intervention. A 44-year-old man presented with severe epigastric pain radiating to the back for 15 days, followed by fever and rapidly progressive painful swelling

Aeromonas hydrophila Necrotizing Soft-Tissue Infection With Acute-on-Chronic Calcaneal Osteomyelitis Following Freshwater Exposure in a Patient With Poorly Controlled Diabetes Mellitus.

Aeromonas hydrophila is a gram-negative bacterium found in freshwater environments that can cause rapidly progressive necrotizing soft-tissue infections (NSTIs), particularly in patients with predisposing host factors such as diabetes mellitus. The organism rarely affects deeper structures such as bone, which can make diagnosis and treatment more difficult. We report the case of a 68-year-old man with poorly controlled type 2 diabetes mellitus, heart failure with reduced ejection fraction, and prior calcaneal trauma who developed an NSTI with acute-on-chronic calcaneal osteomyelitis after stag

Effect of Institutional Guidelines on Equitable Management of Pediatric Osteomyelitis.

BACKGROUND: Acute hematogenous osteomyelitis (AHO) in children requires timely diagnosis and multidisciplinary management to optimize outcomes. Clinical practice guidelines aim to standardize care and reduce variation; however, their impact on health equity is not well understood. We evaluated whether implementation of institutional AHO guidelines improved care delivery and reduced disparities in sociodemographic groups. METHODS: We conducted a retrospective cohort study of children aged 90 days to 18 years hospitalized with AHO at a tertiary care children's hospital. Patients were stratified

Infection Relapse and Time-to-Healing After Conservative Surgery for Non-Acute Presentations of Diabetes-Related Forefoot Osteomyelitis: Does Site Matter?

Diabetes-related foot osteomyelitis (DFO) can present as an acute condition with soft tissue involvement or as a non-acute form characterized by long-standing ulcers without immediate limb-threatening features. This study evaluates infection relapse and healing times after conservative surgery in non-acute DFO, with a focus on osteomyelitis localization. A retrospective cohort of 60 patients treated for forefoot DFO without soft tissue involvement or necrosis was analyzed. Conservative surgery, defined as resection of infected bone without amputation, was performed in all cases. Postoperative

CD177, MYBL2, and RRM2 Are Potential Biomarkers for Musculoskeletal Infections.

BACKGROUND: Biomarkers of infection are measurable indicators that reflect the presence of an infection in the body. They are particularly valuable for detecting infections and tracking treatment responses. Previous transcriptome analysis of peripheral blood mononuclear cells (PBMCs) collected from patients during the active phase of diabetic foot infection identified the upregulation of several genes, including a neutrophil-specific cell surface glycoprotein, CD177, an Myb-related transcription factor 2 (MYBL2), and ribonucleotide reductase regulatory subunit M2 (RRM2).
