A case is reported of dermatomyositis developing in a woman with staphylococcal osteomyelitis. After treatment for the infection, the dermatomyositis resolved and no other cause for it has been found. Dermatomyositis has not previously been described in conjuction with a staphylococcal infection.

In eight children with primary subacute osteomyelitis of a femoral or tibial epiphysis, the only complaints were pain and limp. Some 80 - 90% of all acute haematogenous osteomyelitis is caused by staphylococcus aureus. We suggest that this is due to the special ability of staphylococci to synthesise prostaglandins from appropriate precursors such as arachidonic acid. Prostaglandins, especially of the E series, are potent stimulators of bone resorption. Establishment of osteomyelitis could thus result from minor bone trauma and local release of prostaglandins and their precursors, followed by adventitious infection. A case of metastatic bronchogenic carcinoma to the distal phalanx of the ring finger was initially misdiagnosed as osteomyelitis. The presence of constitutional symptoms, past history of malignancy, the milder and relatively protracted clinical course, the negative culture and lack of response to surgical drainage and antibiotics should make the diagnosis of hand metastasis a distinct possibility. Amputation of the involved digit is an effective palliative treatment and the other foci of metastases should also be treated with systemic chemotherapy.

Several investigators have stressed the usefulness of the technetium bone scan in the early diagnosis of acute osteomyelitis. Seven patients with acute osteomyelitis had initial bone scans that were reportedly normal. The diagnosis was made as a result of either an abnormal gallium scan, positive cultures, or changes in follow-up roentgenograms. The explanation for the normal technetium scans is conjectural but may be related to compromised vascularity. A normal scan does not rule out acute osteomyelitis. If this diagnosis is strongly suspected, other diagnostic tests should be used.

The safety and efficacy of treatment with cefamandole were evaluated in 77 patients (from 33 institutions) with serious bone and joint infections. The antibiotic was given intramuscularly or intravenously in doses ranging from 1.5 to 12 g/day for 6 to 58 days. A model of posttraumatic osteomyelitis was developed in 120 Duncan-Hartley guinea-pigs. For this purpose the right femura of all animals were fractured surgically. Bone scans detect osteomyelitis earlier, sometimes several days earlier, than x-rays. Prompt institution of therapy, before significant bone damage has developed, thus becomes possible. A positive scintigram is, however, nonspecific, and firm diagnosis requires radiologic and other confirmation. Moreover, because the technique is so new, many open questions remain both about its optimal utilization and about its potential.

Smallpox osteomyelitis (osteomyelitis variolosa) with gross clinical and radiologic deformities of the elbows, hands and feet occurred in a 53-year-old Vietnamese refugee. This disease, once common in Asia and Africa, may cause permanent damage to joints and growth centers in the course of the acute infection in childhood. The roentgenographic features are sufficiently diagnostic to exclude congenital dysplasia of the skeletal system, leprosy, or sequelae of thermal injury, and other joint disorders.

Seventy episodes of Staphylococcus aureus sepsis occurring over a nine-year period in pediatric cancer patients are reviewed. Prominent findings at the time of diagnosis included fever, granulocytopenia, and active malignancy. Probable or suspected sites of primary infection were present in 40 episodes (57%). Antimicrobial regimens consisting of a brief initial period of parenteral therapy followed by oral therapy were investigated in infants and children with suppurative bone and joint disease. Electrically generated silver ions have been shown previously to be a potent antibacterial agent with an exceptionally broad spectrum as indicated by in vitro testing.
