Bone and joint infections remain therapeutic challenges, usually requiring prolonged intravenous therapy and hospitalization. Dalbavancin, a long-acting lipoglycopeptide, offers a simplified alternative. We retrospectively analysed 83 patients treated with dalbavancin for osteomyelitis, spondylodiscitis, septic arthritis, or prosthetic joint infection in two tertiary Greek hospitals (2022-2024). Mean age was 69 ± 16 years; 56.6% were male; Charlson Comorbidity Index averaged 4 ± 2.25. Common comorbidities included diabetes (28.9%) and coronary artery disease (18.1%). Infections were vertebral

Necrotizing fasciitis (NF) is a rare, rapidly progressive soft-tissue infection associated with high morbidity and mortality. Diabetes, obesity, and immunosuppression are well-known risk factors that predispose to severe disease and atypical presentations. Complications such as pelvic osteomyelitis and fungal coinfection are exceptionally uncommon in NF and are associated with markedly worse outcomes. Early recognition and immediate surgical intervention are critical to improving survival. We report the case of a 64-year-old woman with poorly controlled type 2 diabetes, obesity, and baseline g

Osteoarticular tuberculosis (TB) is a form of extrapulmonary TB that affects bones and joints, with the spine being the most common site of involvement. Spinal TB, also known as Pott's disease, is therefore the most frequent manifestation of osteoarticular TB. Historically, Pott's disease has been more prevalent in developing countries; however, its incidence is increasing in developed nations, driven by global migration and a growing population of immunocompromised individuals. We report the case of a woman in her 50s who presented with a one- to two-month history of intermittent lower abdomi

Background Amputation in the lower limb can be a challenging procedure both for the surgeon and the patient. Common complications include revision of amputation and wound infection, increasing morbidity to patients and putting further pressure on stretched health services. We describe a simple technique used in lower limb amputation using an antibiotic-loaded calcium sulphate bone graft placed in the medullary canal and soft tissues to control haemostasis and deliver a high dose of antibiotics locally into the wound bed. Methodology We describe a series of 17 lower limb amputations in 15 patie

Sickle cell disease (SCD) is a chronic, inherited hemoglobinopathy associated with multisystem complications, most commonly vaso-occlusive pain crises and stroke; however, rarer manifestations such as acute soft head syndrome (ASHS), characterized by painful scalp swellings from subgaleal hemorrhage, are often overlooked. We report the case of a 15-year-old Kuwaiti male with SCD who presented with a two-day history of headache, multiple scalp swellings, and low-grade fever, alongside a significant history of prior complications, including acute chest syndrome, stroke, splenectomy, and osteomye

BACKGROUND: It is unclear whether coexisting septic arthritis and osteomyelitis (CSAO) differs from septic arthritis (SA) alone in terms of susceptible age groups, clinical and paraclinical presentations, and prevalence. This study aimed to compare patients with isolated SA with those presenting with CSAO, determine the efficacy of different parameters used to distinguish those diagnoses, and investigate the prevalence of CSAO due to Kingella kingae.
METHODS: The study retrospectively included all patients treated for SA over a 17-year period at Geneva University Hospitals. Clinical, biologic

BACKGROUND: Studies of pediatric osteoarticular infections (OAIs) mostly focus on acute hematogenous osteomyelitis (AHO) and acute bacterial arthritis (ABA). A comprehensive descriptive analysis of pediatric OAIs, including subacute, chronic, and non-hematogenous types, is lacking.
METHODS: A detailed analysis of all pediatric OAIs was undertaken at 2 academic centers, Hasbro Children's Hospital, Providence, RI, and Nationwide Children's Hospital, Columbus, OH. Infections were classified as AHO (with or without suppurative arthritis), isolated ABA, subacute or chronic hematogenous osteomyelit

We report a case of sternoclavicular joint (SCJ) septic arthritis in a previously healthy non-smoker 63-year-old male following a direct blunt impact to the anterior chest, which served as the only identifiable predisposing factor. The patient presented with a two-month history of left SCJ pain, swelling, and a chronic draining sinus tract. Laboratory findings demonstrated leukocytosis and elevated inflammatory markers, while aspiration cultures grew methicillin-sensitive Staphylococcus aureus. Surgical management included irrigation and debridement of the SCJ with saucerization of the necroti

Group B Streptococcus (GBS) is a well-recognized pathogen in neonates and older adults but is rare in adolescents, particularly as a cause of musculoskeletal infection. We describe the case of a previously healthy 16-year-old female patient with GBS sacroiliac septic arthritis and osteomyelitis presenting with progressive hip and back pain complicated by acute urinary incontinence. Initial evaluation was confounded by nonspecific symptoms, under-recognized inflammatory markers, and incidental spinal imaging findings, resulting in diagnostic delay. Blood cultures ultimately identified GBS, and

Charcot neuroarthropathy (CNA) is a progressively debilitating condition characterized by joint destruction and deformity due to neuropathy and mechanical trauma. It is most seen in patients with uncontrolled diabetes mellitus and resulting secondary peripheral neuropathy. Misdiagnosis is common, as the initial presentation of erythema, swelling, and warmth can mimic conditions such as cellulitis or osteomyelitis, and radiographic images may resemble that of severe trauma. Early recognition is crucial to prevent complications, including chronic deformity, ulceration, and amputation. In this ca

Vaccines in the pediatric population are crucial in preventing infectious diseases, but rare inflammatory responses can arise, particularly in patients with suspected immunodeficiency. This case highlights a unique inflammatory reaction mimicking septic arthritis following closely spaced pneumococcal vaccines (PPSV23) in a child undergoing immunodeficiency work-up. A previously healthy five-year-old Caucasian female with a history of recurrent bacterial infections presented with a two-day history of left knee swelling and pain after receiving two pneumococcal vaccines within a five-week interv

OBJECTIVE: Purulent infectious myositis, previously known as tropical pyomyositis, is a bacterial disease characterised by abscess formation deep in the extremity muscles, usually seen in the pediatric population. As this disease has been attempted to be defined in a limited number of previous studies in literature, the aim of this study was to present a case series in the light of current diagnostic methods.
METHODS: A total of 52 children (38 males and 14 females) with a mean age of 9.0±5.30 years were diagnosed with primary purulent infectious myositis with extremity localization and under

Osteomyelitis is a severe bone infection that requires early diagnosis and targeted treatment, mainly in pediatric populations where clinical presentations can be complex and nonspecific. This case report presents a seven-year-old male from Los Ríos, Santo Domingo, Dominican Republic, diagnosed with methicillin-resistant Staphylococcus aureus (MRSA)-associated osteomyelitis. The child presented with systemic signs, including fever, vomiting, and abdominal pain, along with localized trauma, limb pain, and functional limitations. Clinical examination revealed hemodynamic instability and gastroin

PURPOSE: The management of pathologic fractures (PF) following osteomyelitis (especially the acute subtype) has not been widely investigated. Therefore, this study aims to assess a stepwise treatment plan for the acute incidence of PF in long bones following pediatric acute Hematogenous osteomyelitis (AHO).
METHODS: This case series was conducted between 2011 and 2023 in a tertiary pediatric center. Patients with fracture incidence within the first 10 days after AHO diagnosis were included. Patients' characteristics were retrospectively reviewed. Our stepwise treatment plan was as follows: 1.

BACKGROUND: An epidural abscess is a rare but serious medical condition where a pocket of pus forms in the epidural space - the area between the outer covering of the spinal cord (the dura mater) and the bones of the spine. It's usually caused by a bacterial infection, most commonly Staphylococcus aureus. The infection can spread to this area from other parts of the body, through the bloodstream, or it may be introduced directly during spinal procedures like epidural injections or surgery. Symptoms often include severe back pain, fever, and neurological deficits like weakness or numbness, whic

Gastric Crohn's disease is a rare manifestation of Crohn's disease affecting the stomach, unlike its more common forms that primarily affect the ileum and colon. This is a case of an 18-year-old female who was referred to gastroenterology for investigation of iron deficiency anaemia, which was first noted when she was 15 years old. Investigation for persistent iron deficiency anaemia (IDA) revealed high faecal calprotectin and a positive faecal immunochemical test (FIT). Endoscopy was done, which revealed mild haemorrhagic gastritis and a few superficial gastric erosions. Biopsies taken from t

Avian pseudotuberculosis infection usually presents as well-demarcated visceral necrotic foci, typically affecting the gastrointestinal tract, liver and spleen. This case series describes an atypical presentation of Yersinia pseudotuberculosis (Yptb) characterized by severe chronic myositis, arthritis and osteomyelitis in five village weavers (Ploceus cucullatus), and acute osteomyelitis and myositis associated with septicaemia in an oriental magpie robin (Copsychus saularis) from a zoological collection. Clinical signs of the weavers included lethargy, poor flying ability and focally extensiv

Pulmonary infiltrates, arising from diverse etiologies such as infections, cardiac conditions, or parenchymal diseases, present a diagnostic challenge. Drug-induced pneumonitis, although less common, should be considered, especially when symptoms develop after medication initiation. This case report highlights a rare yet significant complication of antibiotic therapy, daptomycin-induced eosinophilic pneumonia (DIEP). A 56-year-old male with a history of type 2 diabetes mellitus, hypertension, and renal insufficiency presented with pleuritic chest pain and a productive cough for two days. Chest
