BACKGROUND: Advanced Charcot neuroarthropathy complicated by diabetic foot ulceration is a limb-threatening condition associated with progressive foot deformity, recurrent ulceration, infection, impaired wound healing, and a high risk of major amputation. Evidence describing long-term limb preservation following multimodal conservative management remains limited.
CASE PRESENTATION: A 43-year-old man with poorly controlled type 2 diabetes mellitus and diabetic peripheral neuropathy presented with advanced Charcot neuroarthropathy of the left foot, complicated by a chronic, non-healing dorsal d

Chronic post-craniotomy draining fistulas are rare complications, particularly in patients who have received prior antibiotic therapy. Individuals with neurofibromatosis type 2 (NF2) may face heightened risk due to repeated cranial interventions, impaired wound healing, radiation exposure, and tumor burden. Persistent fistulas in this population are infrequently reported. We describe an unusual case of a chronic post-craniotomy draining fistula persisting for more than seven years in a patient with NF2 who had undergone multiple cranial surgeries and prior radiation therapy for tumor managemen

Odontogenic cutaneous sinus tracts (OCSTs) are an uncommon consequence of chronic dental infection in which inflammatory drainage extends from a periapical source through bone and soft tissues to the skin surface. Although odontogenic infections typically present with intraoral symptoms such as pain, swelling, or vestibular drainage, chronic lesions may occasionally manifest as extraoral nodules, cysts, or draining sinuses. Because many patients experience little or no dental discomfort, these lesions are frequently misdiagnosed as dermatologic, infectious, or neoplastic conditions. The report

Tuberculous osteomyelitis is an uncommon manifestation of extrapulmonary tuberculosis in children, with diaphyseal involvement being particularly rare. Its insidious course and nonspecific imaging often lead to misdiagnosis as pyogenic osteomyelitis or Brodie's abscess, resulting in delayed treatment. This report highlights a diagnostically challenging case of paediatric tibial tuberculosis osteomyelitis mimicking Brodie's abscess. A 15-year-old girl presented with an 18-month history of chronic pain, swelling, and a draining sinus on her left lower leg. Despite undergoing multiple surgeries a

We report a case of sternoclavicular joint (SCJ) septic arthritis in a previously healthy non-smoker 63-year-old male following a direct blunt impact to the anterior chest, which served as the only identifiable predisposing factor. The patient presented with a two-month history of left SCJ pain, swelling, and a chronic draining sinus tract. Laboratory findings demonstrated leukocytosis and elevated inflammatory markers, while aspiration cultures grew methicillin-sensitive Staphylococcus aureus. Surgical management included irrigation and debridement of the SCJ with saucerization of the necroti

Penetrating injuries frequently result in retained foreign bodies, which are often overlooked during initial examination. Delayed detection can lead to complications such as infections, tissue reactions, and functional impairment. Early diagnosis and intervention are essential to optimizing patient outcomes. This report described a case of chronic osteomyelitis caused by a retained wooden foreign body in the hand, highlighting the challenges in diagnosis and management. A 42-year-old male laborer presented with a 12-year history of recurrent hand infections despite multiple treatments. Physica

Skull base osteomyelitis (SBO) is a rare disease that often has delayed presentation in clinical settings. SBO is often misinterpreted as skull base malignancy, thus posing a challenge in diagnosis. We submit a case that presented with chronic otitis media with multiple cranial nerve palsies. His diagnosis was challenged in view of nonspecific imaging findings and odd presentation, which caused a delay in diagnosis. Methicillin-resistance coagulase-negative Staphylococcus (MRCONS) was isolated from the drained pus. MRCONS usually presents with an indwelling medical device or in severe burn cas

Osteopetrosis (OP) is a heterogeneous group of rare, heredity bone disorders with variable clinical features involving the bones of the body. OP is characterized by increased bone density, which is caused by aberrant osteoclast-mediated bone resorption. This syndromic disorder comes with a series of problems and, unless recognized and treated early, can lead to a multitude of further grave complications. We report a rare case of a female patient who reported chronic unhealed extraoral draining sinus present over the left submandibular region with pathologic fracture of the left mandibular angl

Myxoid spindle cell squamous cell carcinoma is a rare variant of squamous cell carcinoma that can pose diagnostic challenges because of its unusual morphology. In this article, we report the case of a 68-year-old man who presented with a slow-growing, fungating mass on the right tibia at the site of his long-standing draining sinus tract. Biopsy revealed a malignant spindle cell tumor with prominent myxoid stroma and areas containing thin-walled blood vessels with a curvilinear appearance. The immunohistochemical profile indicated that the neoplastic cells were positive for a variety of kerati

INTRODUCTION: Osseous hydatidosis is a rare orthopedic condition. Osseous hydatidosis leading to chronic osteomyelitis is rarer with very few articles published on it. This presents a challenge in diagnosis and treatment. Here, we report a patient with chronic osteomyelitis secondary to Echinococcal infection.
CASE REPORT: A 30-year-old lady with fracture left femur operated elsewhere presented with a draining sinus. She underwent a debridement and sequestrectomy. The condition was quiescent until 4 years later when symptoms recurred. She again underwent debridement, sequestrectomy, and sauce

Abscess related to an infected tooth is mostly associated with pyogenic infection, but sometimes, it can be asymptomatic and indicate a chronic condition. This case report shows cold abscess with a draining sinus due to dental origin. A 7-year-old female patient complained of pain with respect to grossly decayed tooth and recurrent swelling with no response to medications. After investigations and management of the lesion, it was concluded as abscess due to chronic granulomatous infection. Cold abscess is a classical manifestation of tuberculosis with no signs of inflammation. More than 60% of

Cyst-like lesion in the coronoid process of the mandible is a challenging diagnosis to make, as it may present with a range of non-specific symptoms. A middle-aged woman reported a one-year history of non-bloody, pus discharge from the right angle of the mandible. There was a history of prior surgery comprising teeth removal two years ago for a painful swelling on the right side of her face, following which her symptoms regressed but in the due course, she developed a chronic sinus with draining abscess. Radiographic findings, in combination with clinical symptoms, are critical in the diagnosi

BACKGROUND: Infection caused by Actinomyces species is a rare cause of head and neck infection in children. This chronic cervicofacial infection can present with localized swelling, abscess formation, sinus drainage and can be complicated by osteomyelitis.
METHODS: Presented are 2 pediatric cases of secondary actinomycosis in the context of congenital lesions: 1 patient with a previously excised preauricular sinus and another with a persistent sublingual mass. A comprehensive literature search was conducted for reported cases of pediatric actinomycosis in the cervicofacial region.
RESULTS: B

Chronic chest wall infections may occur in soft tissue, cartilage, and bone. They may present as localized chest wall pain, a discrete mass initially mistaken for neoplasm, a superficial infection, or a draining sinus. Chronic chest wall infections are typically non-necrotizing and associated with lower morbidity than their more acute and necrotizing counterparts. Effective management of chest wall infections ranges from antimicrobial administration to wide surgical resection and subsequent reconstruction.

Osteomyelitis occurs either as a result of hematogenous seeding, contiguous spread of infection to bone from adjacent soft tissues and joints, or direct inoculation of infection into the bone as a result of trauma or surgery. Hematogenous'osteomyelitis is usually monomicrobial, while osteomyelitis due to contiguous spread or direct inoculation is, usually polymicrobial. Staphylococcus aureus, coagulase-negative staphylococci and aerobic grani-negative bacilli are the most common organisms; other pathogens including streptococci, enterococci, anaerobes, fungi and mycobacteria have also been imp

BACKGROUND: Most recently we reported that RHB‑104 triple antibiotics combination in culture is bactericidal and should be effective for treatment of Crohn's disease (CD)-associated with Mycobacterium avium subspecies paratuberculosis (MAP) (Alcedo et al. in Gut Pathog 14:32, 2016). The combination exhibited unique synergistic antimicrobial growth activity. The proprietary RHB-104 capsule formulation contains active ingredients (63.3 % Clarithromycin (CLA), 6.7 % Clofazimine (CLO) and 30 % Rifabutin (RIF)). In our earlier study, we could not dissolve the proprietary RHB-104 capsule formulation

BACKGROUND: Cutaneous odontogenic fistulas or sinus tracts are frequently misdiagnosed and incorrectly treated, leading to unnecessary procedures and patient suffering. An understanding of the draining of cutaneous sinus tracts will lead to more appropriate treatment. Most cases respond to conservative, nonsurgical root canal therapy. Our objective is to report 2 cases of cutaneous sinus tract secondary to chronic periapical dental infection that were recently observed at our hospital.
METHODS: We present 2 cases of recurrent suppurative facial lesions that were initially misdiagnosed and tre

Squamous cell carcinoma is a rare, but well documented complication of chronic osteomyelitis. Squamous cell carcinoma arising in a sinus tract following chronic osteomyelitis of the pelvic bone is a very rare occurrence. The patient presented with multiple draining sinuses and an ulcerative growth at the mouth of one of the sinuses in the peri-anal region. X-ray findings and sinogram showed de- struction of part of pubic bone. Biopsy from the growth confirmed a well differentiated squamous cell carcinoma. Therefore it is imperative that these lesions be biopsied for accurate diagnosis, adequat
