Osteomyelitis.

Osteomyelitis, defined as inflammation of bone, is a highly morbid disease that is most commonly caused by bacterial infection. Infectious osteomyelitis occurs across a spectrum of ages and affects previously healthy individuals as well as individuals with medical comorbidities. Establishment of osteomyelitis typically occurs following one of three general pathological mechanisms, including haematogenous seeding of healthy or injured bone, extension of a contiguous soft tissue infection, or direct inoculation of microorganisms into the bone following trauma. A wide spectrum of microbial pathog

Assessing Demographic Characteristics, Microbiology, and Mortality Outcomes Associated With Vertebral Osteomyelitis: A National Single-Center Cohort Study in Malta.

Background Vertebral osteomyelitis (VO) is an uncommon but potentially debilitating spinal infection associated with delayed diagnosis, prolonged hospitalization, and significant morbidity. General international consensus indicates that the incidence of VO is increasing, which likely reflects an aging population and an increase in use of advanced imaging. This paper aims to describe the demographic characteristics, risk factors, microbiology, and short-term outcomes associated with VO in Malta. Methods We performed a retrospective single-centre cohort study of adult patients (>18 years old) di

The Management and Outcomes of Staphylococcus aureus Osteoarticular Infections Associated With Prolonged Bacteremia in Children.

INTRODUCTION: Staphylococcus aureus is the predominant cause of acute hematogenous osteoarticular infection (AHOAI, osteomyelitis or bacterial arthritis) in children and is commonly associated with bacteremia. Current guidelines suggest that persistent bacteremia may warrant more aggressive treatment. We evaluated the outcomes of children with S. aureus AHOAI with respect to bacteremia duration. METHODS: Children ≤18 years old with AHOAI secondary to S. aureus from 2011 to 2021 were identified through prospective surveillance studies at Texas Children's and St. Louis Children's Hospitals. An Unexpected Cause of Diabetic Foot Ulcer Infection: Pasteurella multocida.

Diabetic foot infection (DFI) is a common complication of diabetes mellitus and a major cause of preventable morbidity and mortality in individuals with diabetes. Superficial DFI can lead to contiguous spread to soft tissue and bone as well as hematogenous spread and sepsis. It frequently involves typical pathogens such as Staphylococcus aureus and Pseudomonas aeruginosa, or it may be polymicrobial, involving Gram-negative bacilli and anaerobes. Atypical organisms can also be responsible, especially when environmental exposures are present. We report a rare case of Pasteurella multocida osteom

Comprehensive Analysis of the Spectrum of Osteoarticular Infections in Children.

BACKGROUND: Studies of pediatric osteoarticular infections (OAIs) mostly focus on acute hematogenous osteomyelitis (AHO) and acute bacterial arthritis (ABA). A comprehensive descriptive analysis of pediatric OAIs, including subacute, chronic, and non-hematogenous types, is lacking. METHODS: A detailed analysis of all pediatric OAIs was undertaken at 2 academic centers, Hasbro Children's Hospital, Providence, RI, and Nationwide Children's Hospital, Columbus, OH. Infections were classified as AHO (with or without suppurative arthritis), isolated ABA, subacute or chronic hematogenous osteomyelit

An Elderly Patient With Lumbar Spondylodiscitis and Psoas Abscess: Diagnostic Considerations in the Context of Aerococcus urinae.

Aerococcus urinae is an emerging Gram-positive uropathogen increasingly detected in elderly patients with structural abnormalities of the urinary tract. Although often dismissed as a contaminant, recent evidence highlights its potential to cause invasive infections, including infective endocarditis and, in rare cases, vertebral osteomyelitis or spondylodiscitis. Few such spinal infections have been reported to date. We describe the case of an 83-year-old man with a history of resected penile carcinoma (pT1a, R0; partial penectomy and bilateral inguinal lymphadenectomy in 2016), and chronic bla

A Rare Case of Disseminated Bacteroides fragilis Sternoclavicular Osteomyelitis Following Cardiopulmonary Resuscitation (CPR) in an Immunocompromised Patient: Is CPR the Missing Link?

Primary sternoclavicular osteomyelitis is uncommon and often misdiagnosed due to its nonspecific presentation, with anaerobic involvement, particularly by Bacteroides fragilis, being exceptionally rare and typically occurring in immunocompromised patients. We report the case of a 73-year-old man with multiple comorbidities, including diabetes mellitus, chronic kidney disease, ischaemic heart disease, and a chronic heel ulcer, who presented with fever and dyspnoea following a recent pulseless electrical activity arrest and successful cardiopulmonary resuscitation (CPR). Prevalence of radiographic findings in chronic osteomyelitis.

BACKGROUND: Simple radiography in conjunction with pertinent medical history and a comprehensive physical examination is typically adequate for diagnosing chronic osteomyelitis (CO). However, radiographic manifestations of CO lack specificity; therefore, the concordance among specialists in this regard has not been systematically assessed. This study aimed to compare and evaluate the proficiency of orthopedic surgeons and radiologists in identifying radiographic indicators present in simple radiographs for diagnosing CO. METHODS: This cross-sectional study was a correlational investigation ut

Haematogenous polyostotic osteomyelitis caused by Serratia marcescens in a cat.

A 2-year-old female Bengal cat was referred for acute right forelimb lameness one month after ovariectomy. Physical examination revealed multifocal pain on bone palpation and fever. Radiographs showed mixed lytic-proliferative polyostotic diaphyseal osteopathy of multiple bone segments. Histopathologic evaluation of bone biopsies showed severe chronic pyogranulomatous osteomyelitis and multidrug-resistant Serratia marcescens was cultured. Antibiotic therapy with piperacillin-tazobactam was administered for 60 days, based on susceptibility testing. Lameness and bone pain resolved within 15 days

Iliopsoas Abscess Related to Pseudomonas aeruginosa: A Case Report.

We describe a rare case of an 80-year-old male with an iliopsoas abscess (IPA) associated with Pseudomonas aeruginosa (P. aeruginosa). The patient had a history of diabetes mellitus and was admitted to our hospital due to aspiration pneumonia, where he was treated with ampicillin/sulbactam (ABPC/SBT). After admission, he experienced a recurrence of aspiration pneumonia, and ABPC/SBT was repeatedly used. The fever resolved by day 30 and antibiotic therapy was completed on day 33. Although the patient remained afebrile thereafter, anorexia persisted. On day 57, the patient experienced chills, fe

Fungal Musculoskeletal Infections: Comprehensive Approach to Proper Diagnosis.

Fungal musculoskeletal infections often have subacute or indolent manifestations, making it difficult to distinguish them from other diseases and infections, given that they are relatively uncommon. Fungal infections occur by hematogenous spread, direct inoculation, or contiguous extension and may be related to different risk factors, including immunosuppression and occupational activity. The infection can manifest in isolation in the musculoskeletal system or as part of a systemic process. The fungi may be endemic to certain regions or may be found throughout the world, and this can help to n

Assessing Risk for Complications in Acute Hematogenous Osteomyelitis in Children: Validation of 2 Predictive Scores.

BACKGROUND: Acute hematogenous osteomyelitis (AHO) can be associated with severe complications which can be difficult to predict in the clinical setting. The previously published predictive acute complication score ("A-SCORE") and chronic complication score ("C-SCORE") show promise, however, further external validation is needed. METHODS: We performed a retrospective study of 418 children with AHO and analyzed the performance of A-SCORE (variables included bone abscess, fever after 48 h of starting antibiotics, suppurative arthritis, disseminated disease, and delayed source control) to predic

Insights into S. aureus-Induced Bone Deformation in a Mouse Model of Chronic Osteomyelitis Using Fluorescence and Raman Imaging.

Osteomyelitis is an infection of the bone that is often difficult to treat and causes a significant healthcare burden. Staphylococcus aureus is the most common pathogen causing osteomyelitis. Osteomyelitis mouse models have been established to gain further insights into the pathogenesis and host response. Here, we use an established S. aureus hematogenous osteomyelitis mouse model to investigate morphological tissue changes and bacterial localization in chronic osteomyelitis with a focus on the pelvis. X-ray imaging was performed to follow the disease progression. Six weeks post infection, whe

Patellar Osteomyelitis in a 9-Year-Old Patient with Chronic Granulomatous Disease: A Case Report.

Hematogenous osteomyelitis is commonly reported in long tubular bones in the pediatric population. Acute osteomyelitis involving the patella is extremely uncommon in children, and its diagnosis is frequently delayed due to its rarity and variable clinical manifestations. Chronic granulomatous disease (CGD) is a rare genetic immunodeficiency disorder characterized by severe recurrent bacterial and fungal infections. The most commonly affected sites of infection are the lungs, lymph nodes, skin, liver, and gastrointestinal tract. Acute hematogenous osteomyelitis of the patella associated with CG

Modulation of Gut Microbiota Metabolism in Obesity-Related Type 2 Diabetes Reduces Osteomyelitis Severity.

Staphylococcus aureus is an opportunistic pathogen causing osteomyelitis through hematogenous seeding or contamination of implants and open wounds following orthopedic surgeries. The severity of S. aureus-mediated osteomyelitis is enhanced in obesity-related type 2 diabetes (obesity/T2D) due to chronic inflammation impairing both adaptive and innate immunity. Obesity-induced inflammation is linked to gut dysbiosis, with modification of the gut microbiota by high-fiber diets leading to a reduction in the symptoms and complications of obesity/T2D. However, our understanding of the mechanisms by

Osteoarticular Mycoses.

Osteoarticular mycoses are chronic debilitating infections that require extended courses of antifungal therapy and may warrant expert surgical intervention. As there has been no comprehensive review of these diseases, the International Consortium for Osteoarticular Mycoses prepared a definitive treatise for this important class of infections. Among the etiologies of osteoarticular mycoses are Candida spp., Aspergillus spp., Mucorales, dematiaceous fungi, non-Aspergillus hyaline molds, and endemic mycoses, including those caused by Histoplasma capsulatum, Blastomyces dermatitidis, and Coccidioi

Subtrochanteric femoral fracture with postoperative chronic osteomyelitis treated successfully by 1-stage operation: a case report.

Chronic osteomyelitis is a chronic infectious disease of bone tissue, which can cause necrosis of bone and surrounding soft tissue, and is a common complication of open fracture, internal fixation, diabetic foot and blood-borne bone infection. Traumatic osteomyelitis is caused by bone tissue infection after open fracture surgery or open reduction of fracture or other bone and joint surgery. The lesion is near the fracture end. Intramedullary infection is the most serious infection in acute stage, with high fever, chills and other toxemia symptoms, similar to acute hematogenous osteomyelitis. T
