Human cytomegalovirus antagonizes SMC5/6 driven genome silencing via UL35 instituted proteasomal degradation of SLF2.

Recent studies demonstrated that the structural maintenance of chromosomes complex 5/6 (SMC5/6) acts as an important antiviral restriction factor that silences episomal DNA. Consequently, viruses have evolved strategies to antagonize this defence system. Interestingly, it was shown that SMC5/6-mediated silencing of extrachromosomal DNA depends on its recruitment to PML nuclear bodies which is mediated by a protein termed SMC5/6 localization factor 2 (SLF2). Since human cytomegalovirus (HCMV) dissociates PML nuclear bodies (PML-NBs) during the first hours of infection, we asked for the fate of

Recurrent Infections and Refractory Lymphoproliferation Associated With a Novel PRKCD Variant: A Case Report of Successful Haematopoietic Stem Cell Transplantation.

Lymphoproliferation is a common manifestation of inborn errors of immunity and may delay diagnosis when it is the initial presentation. A nine-year-old girl developed recurrent lymphadenopathy from two years of age, accompanied by recurrent infections, anaemia and growth retardation. Whole-exome sequencing identified a homozygous PRKCD missense variant. Lymph node biopsy revealed florid follicular hyperplasia with cytomegalovirus (CMV) activation, and CMV viremia was detected. Despite immunoglobulin replacement, valganciclovir and immunomodulatory therapy, lymphoproliferation persisted. Haemat

Cellular immune response during BK polyomavirus infection in living donor kidney transplant recipients.

BACKGROUND: BK polyomavirus (BKPyV) is a significant cause of graft impairment and loss following kidney transplantation. Early prediction of which patients are at risk of BKPyV replication and BKPyV-associated nephropathy (BKPyVAN) would be beneficial, preferably prior to transplantation. From an immunological perspective, determinants of susceptibility to BKPyV reactivation remain incompletely understood. The aim of this study was to evaluate the prognostic value of lymphocyte subset counts, CD4/8 T cell ratio, and changes in CD4 + T cell counts over time. Additionally, the concept of Immune

Letermovir Prophylaxis in Pediatric Allogeneic Hematopoietic Stem Cell Transplantation: Clinical Effectiveness and Economic Implications.

BACKGROUND: Cytomegalovirus (CMV) reactivation is a major infectious complication following pediatric allogeneic hematopoietic stem cell transplantation (HSCT), particularly in low- and middle-income countries (LMICs) with high CMV seroprevalence. Data on the clinical and economic impact of letermovir prophylaxis in pediatric HSCT recipients from resource-constrained settings remain limited. METHODS: We conducted a retrospective cost-effectiveness analysis of pediatric allogeneic HSCT recipients at a university-affiliated tertiary-care transplant center in India. Clinical outcomes and direct

Changing Burden of Opportunistic Infections in the New ART Era.

INTRODUCTION: Human immunodeficiency virus (HIV) remains a major global health challenge, characterized by progressive CD4⁺ T-cell depletion leading to opportunistic infections (OIs) and acquired immunodeficiency syndrome (AIDS). Despite widespread antiretroviral therapy (ART) coverage, significant disparities persist, particularly in resource-limited settings. This review critically appraises the evolving burden and spectrum of OIs in the pre- and post-ART eras. METHODS: A comprehensive literature search for this narrative review was conducted in PubMed, Embase, Web of Science, and Google Sc

Human filarial infection reshapes CD8+ T-cell transcription and function at homeostasis and following viral activation.

The role of CD8+ T cells in filarial infections remains poorly understood. Given the chronic nature of these infections, individuals living in endemic regions are frequently exposed to other pathogens including viruses. Because CD8 cells are essential for antiviral immunity, understanding how filarial infections shape the cytotoxic compartment is essential to elucidate their impact on bystander immunity. We evaluated the phenotype and function of CD8+ T cells from Loa loa-infected (Fil+) and -uninfected (Fil-) individuals at baseline and following cytomegalovirus (CMV) re-stimulation. Filarial

Herpesvirus reactivation in critical illness: Pathogenesis, clinical consequences, and diagnostic challenges.

Human herpesviruses (HHV) remain latent after primary infection and may reactivate during critical illness. HHV reactivation has been associated with adverse clinical outcomes, yet its pathogenic significance and optimal management remain unclear. This review summarizes the dysregulated immune mechanisms and risk factors for HHV reactivation in critical illness, treatment options, outcomes, current diagnostic challenges, and emerging precision medicine approaches. Critical illness induces profound immune dysregulation characterized by an initial hyperinflammatory response followed by immunosup

An Active Cytomegalovirus Infection and Epstein-Barr Virus Reactivation Complicated by Cytomegalovirus Meningitis and Virus-Associated Hemophagocytic Lymphohistiocytosis during Methotrexate and Upadacitinib Therapy in a Patient with Rheumatoid Arthritis.

Methotrexate (MTX) and Janus kinase (JAK) inhibitors are effective treatments for rheumatoid arthritis (RA); however, they may increase the risk of opportunistic infections. We herein report a case of a patient with RA receiving MTX and upadacitinib who developed an active cytomegalovirus (CMV) infection and Epstein-Barr virus (EBV) reactivation, complicated by CMV meningitis and virus-associated hemophagocytic lymphohistiocytosis (HLH). This case highlights that clinicians should consider not only herpes zoster, but also infection or reactivation involving non-VZV herpesviruses, including CMV

Healthcare resource utilization and costs of letermovir for cytomegalovirus-prophylaxis after allogeneic hematopoietic cell transplantation in a real-world, multi-centre study in Germany.

OBJECTIVES: Letermovir has demonstrated efficacy and tolerability as prophylaxis against clinically significant cytomegalovirus infection (csCMVi) in CMV-seropositive adult patients undergoing allogeneic hematopoietic cell transplantation (alloHCT) compared to alternative antivirals. Despite these advantages, associated costs remain substantial. This study addresses the evidence gap by evaluating the health economic impact of CMV prophylaxis with letermovir in a real-world setting for alloHCT patients. METHODS: This retrospective, multi-centre case-control study was conducted at six German te

Multi-omics analyses reveal key immunological and metabolic correlates of mortality in HIV-associated Pneumocystis pneumonia.

Pneumocystis jirovecii pneumonia (PCP) remains a major cause of life-threatening respiratory failure in people with advanced HIV, yet the biological factors associated with mortality are incompletely understood. We performed integrated transcriptomic, proteomic, and metabolomic profiling of bronchoalveolar lavage fluid from 42 adults with HIV-associated PCP, relating molecular signatures to mortality. We found that fungal burden, measured by sputum immunofluorescence, Pneumocystis qPCR, and 1,3-β-D-glucan, did not differ between survivors and non-survivors or correlate with baseline gas exchan

Disseminated Bartonella or Post-Transplant Lymphoproliferative Disease in Pediatric Transplant Patients: A Diagnostic Challenge.

BACKGROUND: Post transplant lymphoproliferative disease (PTLD) and infection remain the two most common complications of solid organ transplant and carry a risk of significant morbidity and mortality. PTLD is a highly variable disease with clinical signs and symptoms that are non-specific and require a high degree of suspicion to detect. The most common presenting symptoms of PTLD are malaise, fatigue, fever, and a mononucleosis-like picture; night sweats, weight loss, and lymphadenopathy are also frequent manifestations. An infection with Bartonella Henselae is another known cause of local ly

Immune signatures of common exposures through co-occurrence of T-cell receptors in tens of thousands of donors.

BACKGROUND: Memory T cells are records of clonal expansion from prior immune exposures such as infections, vaccines and chronic diseases. Some of the receptors of these expanded T cell clones in a typical immune repertoire are highly public (present in many individuals) because they respond to the same peptide from a prevalent immune exposure, presented by the same Human Leukocyte Antigen (HLA) allele. Only a tiny fraction of public T-cell receptor β sequences (TCRs) have known associations with exposures or specific peptides. METHODS: We mined the TCR repertoires of tens of thousands of dono
