Case Report: Distal tibial physeal injury secondary to conservative treatment of acute hematogenous osteomyelitis resulting in ankle deformity - highlighting the necessity of regular follow-up.

Ankle deformities most commonly result from physeal injuries following major trauma; those secondary to occult physeal injury without significant antecedent trauma are rarely documented. We report a case of ankle deformity in a 13-year-old boy resulting from distal tibial physeal injury following conservative treatment for acute hematogenous osteomyelitis(AHO) four years earlier; despite initial clinical improvement, the patient was subsequently lost to follow-up. At his return visit, significant ankle discomfort severely compromised daily activities. Following combined distal tibial osteotomy

PF127 hydrogel-assisted in-situ Staphylococcus aureus biofilm fabrication endowing pathological animal model for chronic osteomyelitis.

Chronic osteomyelitis driven by biofilms remains difficult to treat and thus the exploration of its clinical therapy and pathological mechanism largely depend on effective animal models. However, standardized and efficient methods for establishing stable and reproducible animal models for chronic osteomyelitis are still lacking. This study developed a Hydrogel Assisted In-situ Biofilm Fabrication (HAIBF) method by coupling thermosensitive Pluronic F-127 (PF127) hydrogel with biofilm fragments, which stably mimic the pathological process of biofilm-infected chronic osteomyelitis. Non-union of Bone Plating in a Mandibular Fracture With Osteomyelitis: A Case Report.

Mandibular fractures are a common occurrence, often associated with complex complications such as non-union and osteomyelitis. Non-union, the failure of a fracture to heal, can be particularly challenging in the mandible due to the limited soft tissue coverage and compromised blood supply in atrophic areas. Additionally, the presence of osteomyelitis, a bone infection, further complicates the healing process and increases the risk of implant failure. This case report aims to provide an overview of the management strategies for non-union of bone plating with mandibular fracture associated with

GaSex Nanoparticles Modulate Neutrophil Function and Mitigate NET-Driven Osteogenic Impairment in Osteomyelitis Therapy.

Osteomyelitis remains a major clinical challenge; however, current therapeutic strategies do not sufficiently address host immune defense, inflammatory microenvironment remodeling, or bone regeneration. Here, we show that excessive accumulation of neutrophil extracellular traps (NETs) in infected bone tissues contributes to osteomyelitis-associated apoptosis and impaired osteogenesis. NETs disrupt PINK1/Parkin-mediated mitochondrial quality control in osteoblast lineage cells, thereby promoting apoptosis, whereas inhibition of NET formation alleviates these pathological changes and improves bo

Nutritional support in bone infections: mechanisms, strategies, and clinical practice.

OBJECTIVE: Bone infections, including osteomyelitis and periprosthetic joint infection (PJI), are characterized by prolonged treatment, high recurrence, and functional impairment. Persistent inflammation induces hypercatabolism, negative nitrogen balance, and micronutrient depletion, impairing immune defense and bone repair. Systematic nutritional assessment remains insufficient. This review elucidates metabolic mechanisms under infectious stress, evaluates macronutrients, micronutrients, and immunomodulatory nutrients in infection control and bone regeneration, and proposes a clinical nutriti

Sepsis Prior to Total Knee Arthroplasty is Associated With Elevated Rates of Myocardial Infarction and Deep Vein Thrombosis.

BACKGROUND: Patients who have sepsis often experience persistent physiological instability, which may increase vulnerability to postoperative complications. However, the impact of preoperative sepsis on outcomes following elective total knee arthroplasty (TKA) remains poorly defined. Thus, the purpose of this study was to investigate the association of preoperative sepsis with 90-day and two-year postoperative complications following TKA. METHODS: A retrospective study was conducted utilizing an insurance database. Patients who had a preoperative diagnosis of sepsis within two years prior to

Natural polysaccharide-engineered POP@ZIF-67 nanoparticles coordinate antibacterial activity, macrophage polarization, and osteogenesis for chronic osteomyelitis therapy.

Chronic osteomyelitis is difficult to treat due to the interplay of bacterial colonization, excessive inflammation, and impaired bone regeneration. Here, we developed an engineered natural polysaccharide POP@ZIF-67 nanoplatform to modulate these interrelated pathological processes. ZIF-67 is an antimicrobial framework with porous properties, peroxidase-like catalytic activity, and acid-responsive release behavior; purslane polysaccharide (POP) was incorporated as an immunomodulatory component to mitigate the excessive inflammatory stimulation induced by ZIF-67. POP@ZIF-67 exhibits effective an

Recurrent severe pyogenic infections revealing neutrophil-specific granule deficiency caused by a novel CEBPEsplice-region variant: a case report.

Neutrophil-specific granule deficiency is an exceptionally rare autosomal recessive primary immunodeficiency caused by pathogenic variants in the CCAAT/enhancer-binding protein epsilon (CEBPE) gene. Defective neutrophil maturation results in impaired bactericidal activity and recurrent pyogenic infections, predominantly affecting the skin and soft tissues. We describe two affected siblings from a family of four carrying a previously unreported CEBPE missense substitution in the splice region (p.K170N; c.510G>C). A 37-year-old woman presented with lifelong recurrent pyogenic skin infections, in

[Maximum limb reconstruction: concept and implementation strategies].

Thanks to the rapid development of modern science and medicine, limb reconstruction can and should be taken to the next level. The author proposes the concept of "maximum limb reconstruction", which pursues not merely function but the maximal normalization of structure, morphology, and function, leveraging advances in prosthetics, artificial intelligence, and exoskeleton technology to enhance limb preservation. For the three pathological states-defect, infection, and deformity-sequential strategies are outlined: ① mutilating injuries are managed in three phases (early revascularization, mid-te

Susceptibility-guided use of gentamicin-loaded calcium sulfate after surgical debridement for complex diabetes-related foot infection: a single-centre case series.

BACKGROUND: Complex diabetes-related foot infections (DFIs) frequently involve tissue necrosis, impaired perfusion, bacterial persistence, and residual dead space, which may complicate infection control despite surgical and systemic antimicrobial treatment. Bioabsorbable calcium sulfate provides a potential means of delivering high local antibiotic concentrations after surgical source control. This study describes the clinical, microbiological, wound, and safety outcomes of susceptibility-guided gentamicin-loaded calcium sulfate used as an adjunct to surgical debridement and systemic antimicro

Biofilm-associated genes in Staphylococcus aureus: role in chronic infections and therapeutic challenges.

One of the major contributors to persistent healthcare and community-associated infections is S. aureus, which has a remarkable ability to form biofilms that protect it against the host immune system and antimicrobial therapy. In recent years, it has become clear that biofilm development is a highly orchestrated and dynamic process, controlled by complex genetic networks, transcriptional regulators, two-component systems (TCSs), and environmental signals. The present review covers the molecular mechanisms involved in the initiation, maturation, maintenance, and dispersal of biofilms in general

Imaging of primary vertebral tumors and tumor-like lesions in skeletally immature patients: a pictorial review.

Primary vertebral tumors and tumor-like lesions in skeletally immature patients are uncommon yet clinically consequential causes of pain, deformity, and neurological compromise. Compared with adults, pediatric lesions arise in a spine with open physes, evolving marrow composition, and substantial remodeling capacity-features that shape tumor biology and imaging appearance. This pictorial review summarizes malignant tumors, benign tumors, and tumor-like lesions of the pediatric spine, adopting a modality-first, compartment-aware approach. Useful highlights include vertebra plana in Langerhans c
