Computed Tomographic Findings of Ischemic Necrosis of the Accessory Carpal Bone in a Dog.

A 1-year-and-9-month-old Golden Retriever presented with a 9-month history of right thoracic limb lameness. Radiographs revealed a monostotic, expansile lesion of the right accessory carpal bone (ACB). A computed tomography (CT) confirmed the aggressive nature of the lesion, demonstrating multifocal moth-eaten osteolysis, sclerosis, periosteal reaction, and flexor carpi ulnaris tendinopathy. Differential diagnoses included the ischemic necrosis of the ACB, osteomyelitis, and primary bone neoplasia. Histopathology confirmed osteonecrosis. This case highlights the value of CT in evaluating aggre

Infections and the Opioid Epidemic: Changing Epidemiology and Treatment Options.

The opioid epidemic has led to an infectious diseases syndemic, with injection drug use responsible for driving rising rates of HIV, viral hepatitis, and serious bacterial and fungal infections. Opioid use continues to cause devastating mortality, with over 80,000 opioid-involved overdose deaths in the USA in 2022. Despite the proven effectiveness of opioid agonist therapy, specifically methadone and buprenorphine, treatment coverage remains inadequate, with profound gaps in opioid use disorder (OUD) continuum of care. Injection-related infections, particularly skin and soft tissue infections,

Thoracoabdominal CT Angiography Detection of Anterior Mitral Leaflet Perforation in Recurrent Infective Endocarditis After Serial Echocardiograms: A Case Report.

Infective endocarditis (IE) can cause destructive valvular complications, including leaflet perforation and vegetation. Echocardiography remains the primary imaging modality for valvular assessment; however, computed tomography angiography (CTA) may reveal clinically important cardiac findings when performed for other urgent indications. We report the case of a 70-year-old male with recurrent IE, prior prosthetic aortic valve endocarditis first diagnosed in 2022, multiple aortic valve and aortic root operations, thoracic endovascular aortic repair with a left subclavian stent, prior aortic gra

Modality-specific quantitative radiologic characterization of diffuse sclerosing osteomyelitis of the mandible: a pilot study.

OBJECTIVES: Diffuse sclerosing osteomyelitis (DSO) of the mandible is a rare chronic inflammatory disease characterized by recurrent painful swelling and trismus. While advanced stages are known to show radiographically evident bone sclerosis, objective quantitative imaging criteria remain poorly defined. This study aimed to characterize modality-specific radiological features of DSO using intra-individual side-to-side comparison of bone density and cortical thickness. MATERIALS AND METHODS: This single-center retrospective study included 22 patients treated for DSO between 2010 and 2021. Pre

Body mass index and type 2 diabetes liability in osteomyelitis and cellulitis: A cross-biobank Mendelian randomization study.

Previous Mendelian randomization (MR) studies have linked obesity and type 2 diabetes (T2D) liability to osteomyelitis and skin or soft-tissue infection, but the reproducibility of these associations across independent outcome biobanks remains uncertain. We conducted cross-biobank univariable MR and multivariable MR analyses using European-ancestry genome-wide association study summary statistics. FinnGen Release 13 osteomyelitis (2519 cases and 471,012 controls) was designated as the primary clinical outcome, whereas cellulitis (10,223 cases and 468,418 controls) served as a comparative infec

The perforator-based axial adipofascial turnover flap combined with juncturae tendinum grafting for complex dorsal phalangeal defects.

INTRODUCTION: Complex dorsal finger soft tissue defects remain a challenging reconstructive problem, particularly when associated with exposure of bone and extensor apparataus. Advances in the anatomical understanding of the dorsal cutaneous vascular system of the digits have allowed adipofascial flaps in this region to evolve from random-pattern to axial-pattern designs, improving vascular reliability. However, a structured and comprehensive approach for managing these combined defects has not been clearly defined in the literature. This study aims to present a holistic loco-regional strategy

Area Deprivation Index as an Additive Marker of Postdischarge Care Vulnerability in Pediatric Acute Hematogenous Osteomyelitis.

BACKGROUND: Acute hematogenous osteomyelitis (AHO) in children requires prolonged antibiotic therapy and structured follow-up. However, successful treatment depends not only on medical management but also on consistent adherence to antibiotics and outpatient care. Socioeconomic disadvantage may influence a family's ability to complete treatment and attend follow-up appointments. The Area Deprivation Index (ADI) is a validated measure of neighborhood-level socioeconomic disadvantage and may help identify disparities in treatment continuity. This study sought to determine whether higher ADI scor

Concordance of Microbiological and Histopathological Examination of Bone Biopsies in Diagnosing Diabetic Foot Osteomyelitis, a Systematic Review.

BACKGROUND: Diabetic foot osteomyelitis (DFO) is a serious complication of diabetic foot ulcers and is associated with increased risk of amputation, morbidity, and mortality. Bone biopsy remains the most definitive diagnostic modality, yet controversy persists over its diagnostic utility This study reviews the diagnostic concordance of histopathology and microbiology in bone biopsy specimens from patients with DFO and examines the clinical implications of the findings. METHODS: A systematic review was conducted using PubMed, Medline, CINHAL, Cochrane Library, and Google Scholar. Studies inclu

Neutrophil-Driven Inflammation in Pediatric Chronic Recurrent Multifocal Osteomyelitis.

OBJECTIVE: Chronic recurrent multifocal osteomyelitis (CRMO) is a pediatric autoinflammatory bone disease with poorly defined pathophysiology and limited biomarkers. Murine models implicate neutrophils, neutrophil-derived IL-1β, and the adaptor protein PSTPIP2, but direct evidence in human CRMO is lacking. This study aims to translate findings from mouse models to human disease by investigating the role of neutrophils and PSTPIP2 in CRMO. METHODS: Blood samples from 34 patients with CRMO and 20 controls were analyzed by flow cytometry, biochemical assays, and transcriptomic profiling of purif

Systemic lupus erythematosus complicated by refractory Mycobacterium avium complex infection involving bilateral lower extremities: a case report and literature review.

BACKGROUND: Patients with systemic lupus erythematosus (SLE) receiving long-term glucocorticoids and immunosuppressants are at markedly increased risk of opportunistic infections. Mycobacterium avium complex (MAC) is a common nontuberculous mycobacterial pathogen, but MAC osteomyelitis is extremely rare in non-HIV SLE patients, posing diagnostic and therapeutic challenges. CASE PRESENTATION: We report a 52-year-old female with SLE on long-term prednisone, azathioprine, leflunomide, and hydroxychloroquine, who developed bilateral tibial and femoral osteomyelitis complicated by soft tissue absc

Low-Field Magnetic Resonance Imaging Features of Axial Proximal Sesamoid Osteomyelitis and Intersesamoidean Ligament Rupture in a Horse.

A 7-year-old Warmblood mare used for show jumping was evaluated for progressive left hindlimb lameness of 5 weeks' duration, localized clinically to the metatarsophalangeal region. Radiography revealed ill-defined osteolysis of the axial apical regions of both proximal sesamoid bones (PSBs). Due to rapid clinical deterioration, low-field magnetic resonance imaging was performed and demonstrated aggressive axial osteomyelitis of both PSBs, partial rupture of the intersesamoidean ligament and proximal scutum with herniation of synovium of the plantar joint recess, and an occult penetrating tract

Advanced Eumycetoma of the Right Foot Resulting in Below-Knee Amputation in Somalia: A Case Report.

INTRODUCTION: Mycetoma is a chronic, progressively destructive neglected tropical disease that presents with localised swelling, multiple sinus tracts and a discharge containing grains, and that may extend into deep tissue and bone. Somalia lies within the recognised mycetoma belt, but the national burden is poorly documented, and few patients reach specialist care early enough for the limb to be saved. CASE PRESENTATION: A 33-year-old man from Qoryooley District, Lower Shabelle, Somalia, presented with a seven-year history of progressive swelling of the right foot. Examination showed massive

Reconstruction of Severe Forearm Osteomyelitis.

This article reviews the surgical management of severe chronic forearm osteomyelitis using free vascularized fibular flaps (VFFs), with or without the one-bone forearm (OBF) technique. Based on a 22 patient retrospective series, outcomes show that VFF enables reliable reconstruction of extensive bone and soft tissue defects following radical debridement. In selected cases, OBF offers a salvage solution for both radius and ulna bone loss. The approach led to high infection control rates, bone healing, and functional recovery. Microsurgical reconstruction, particularly using VFF, remains the gol

Reassessing the Kocher Criteria: A Prospective Evaluation of Diagnostic Performance in Pediatric Musculoskeletal Infection.

BACKGROUND: The Kocher criteria are widely used to differentiate septic arthritis (SA) from transient synovitis (TS) in children. However, diagnostic performance has since been proven to be inconsistent. Furthermore, the criteria were originally developed to distinguish SA from TS and have not been validated for differentiating osteomyelitis (OM), which frequently presents with overlapping clinical features. The purpose of this prospective study was to evaluate the diagnostic performance of the Kocher criteria and other clinical variables in differentiating SA from both TS and OM. METHODS: A

Clinical, etiological, and microbiological differences between foot and non-foot osteomyelitis.

INTRODUCTION: Osteomyelitis presents with heterogeneous clinical, etiological, and microbiological characteristics depending on anatomical location. While foot osteomyelitis is commonly associated with diabetic foot infections and contiguous spread, non-foot osteomyelitis often arises from post-traumatic, hematogenous, or postoperative causes. However, direct comparative analyses between these two entities remain limited. MATERIALS AND METHODS: A retrospective analysis was conducted on 202 adult patients who underwent surgical treatment for osteomyelitis between 2011 and 2026. Patients were d
