Serum levels of osteocalcin, the major noncollagenous bone protein, are elevated in patients with certain metabolic bone diseases, but the effects of other illnesses on serum osteocalcin levels are not known. We measured serum osteocalcin concentrations in 250 patients in a rehabilitation hospital who suffered from various illnesses. Frequency of rare skeletal lesions in Paget's disease is compared with normal localisations. Radiological morphology of rare localisations of Paget's disease of the scapula, of the lower arm bones, of the hand bones, of the patella and the calcaneus are described. Thickening of trabeculae in the spongy bone and porosity of compact bone, deformation and enlargement of the bones are typical; this has to be differentiated and related to a small number of other bone diseases.

We have presented three patients who had bony expansion due to metastatic carcinoma of the prostate demonstrated by roentgenography and bone scintigraphy. Bony expansion must be differentiated from the blooming phenomenon which may occur in areas of high radioactivity. Bony expansion is not specific for Paget's disease, but also occurs in many other benign and malignant conditions.

The presentation of Paget's disease varies from a painful or deforming skeletal affliction to an asymptomatic disorder diagnosed on routine biochemical or radiological assessment. When involvement of the peripheral skeleton by Paget's disease is extensive, the clinical diagnosis is usually clear. Affected bones are thickened and deformed and the overlying skin is warm. Out of 21 900 cases filed at the Latin-American Registry of Bone Pathology between April 1940 and July 1981, there were 987 with Paget's disease (4.51 per cent); 62 of these (6.28 per cent) were complicated by sarcoma and two were associated with giant-cell tumours of bone (osteoclastoma) without signs of malignancy. Osteogenesis imperfecta (OI) and Paget's disease of bone occurred in a patient whose brother has Paget's disease. Several other relatives have the dominant variety of OI. The author presents a case of Paget's disease effecting a lumbar vertebral body that was seen on a computerized tomogram. It had an interesting appearance and showed no contrast enhancement. There was a moderate compression fracture to this vertebral body and there was encroachment on the spinal canal. The author knows of no other similar case presentation.

A spinal cord compression due to intradural meningioma appeared in two patients with vertebral X-Rays lesions of foreknown Paget's disease. These lesions were located at the same level that the meningioma in the second case and three vertebra below the meningioma in the first case. Sixty-nine patients with primary osteosarcoma of the bone are reviewed. There were 50 males and 19 female; a male predominance of 2.6:1. The patients presented in advanced stages. In agreement with other authors, we found a peak incidence in the second decade. No associated Paget's disease was found in any of the elderly patients. The long bones of the lower limbs were the most frequently affected. Treatment with amputation was not acceptable to the great majority of our patients.

The two most commonly used biochemical markers of bone turnover are the serum alkaline phosphatase and the urinary excretion of peptide-bound hydroxyproline, both of which are increased in Paget's disease. Osteitis deformans (Paget's disease of bone) may be complicated by osseous cancer. Of 3964 patients between 1927 and 1977, 38 had primary malignant bone tumors. This represents an incidence of sarcomatous change of 0.95%. The bones of the pelvis were most often involved; the humerus and the femur were next in frequency. Calcitonin is a peptide hormone secreted by the C-cells of the thyroid gland. A major physiological function of the hormone appears to be the protection of the skeleton against resorption in humans. It thus opposes the resorptive actions of parathyroid hormone and 1,25 dihydroxyvitamin D. This action is utilised pharmacologically in order to treat diseases where increased bone resorption is a major component. Paget's disease of bone is characterized by focal resorption of existing bone followed by the deposition of woven and lamellar bone in a characteristic pattern. Pain was present in 113 out of a total of 148 patients with Paget's bone disease who were evaluated. The symptom was originated by: a) bone, in 56.6 % of the cases; b) joints, in 32.7 %; c) skull compression in 7.1 % and spine compression in 3.5 % of the cases.
