33 monostotic and 27 polyostotic manifestations were found in 60 patients with Paget's disease of bone, 30 of whom were females (mean age 65.7 years) and 30 males (mean age 62.8 years). Malignant change is to be expected in about 1% of patients with Paget's disease. Usually these are osteosarcomas. Light and electron microscope studies of 64 biopsies from patients with Paget's bone disease reveal the structure of osteoblasts and osteoid tissue. The osteoblasts observed do not differ morphologically from those described in other bone diseases and the woven bone shows no specificity. Thirteen tibiae and seven femora suffering from bowing due to Paget's disease have been streightened operatively. In the tibiae, the deformities were of metaphysial genu varum with bowing in the sagittal plane of an average of 30 degrees combined with some medial rotation. In the femora, the main curve was anterolateral with some medial rotation and coxa vara. Seventeen cases united uneventfully after osteotomy. Experiences in sue of skeletal scintimaging, primarily with 99mTc EHDP, is reported in 108 patients with Paget's disease. Supplemental hand scintigrams with abnormal features were obtained from 29% of patients (134 of 463) who were referred for routine minified bone imaging with 99mTc-Sn-polyphosphate. A wide spectrum of normal activity distribution ranging from well-defined to "wash-out" images is described in 329 cases (71%). Twenty-eight patients with symptomatic Paget's disease of bone were treated with synthetic salmon calcitonin for periods of 9 to 42 months (average, 23 months). Serum alkaline phosphatase concentration and urinary hydroxyproline excretion, which had been elevated before treatment, were decreased by calcitonin treatment in all patients, and some decrease was sustained in 23 in association with variable decreases in pain, heat and stiffness of major joints. The problem of 'Gundu' in primates is discussed. Cases of so-called 'Gundu' in the literature, two cases in Gorilla and one in Lagothrix, are dealt with. Histological and X-ray techniques are used. One case (Lagothrix) represents the so-called simian bone disease, which is metabolic disorder and is due to a secondary nutritional hyperparathyroidism. The other two cases (Gorilla) show symmetrical bony tumors of the maxilla. The origin is uncertain. Gundu, Paget's disease and primary hyperparathyroidism Recklinghausen have not been observed in primates so far.

A temporal bone report is presented of a patient with progressive sensorineural hearing loss and advanced Paget's disease involving the skull. The histopathology reveals Pagetic bone invading the internal auditory canal and compressing the cochlear division of the VIIIth cranial nerve resulting in severe neural degeneration. There are no other histopathologic abnormalities present in the cochlea to explain the hearing loss. A low-frequency conductive hearing loss was also present, but no histopathologic correlate could be identified.

Archeological bone often lends itself to histological analysis. Sections from bone samples approximately one thousand years old may show as much structural preservation as those only a few hundred years old; hence, it appears that the degree of preservation is not necessarily affected by time. Enough structure may be preserved to permit the diagnosis of metabolic disorders of bone which might go undetected by other methods. This type of analysis can be utilized to accept or reject individual remains suspected of being pathological on the basis of other less precise diagnostic techniques.

The ultrastructure of bone cells was examined in biopsies from 18 patients with Paget's disease of bone and from 60 patients with a variety of other bone diseases. A characteristic nuclear inclusion was found in the osteoclasts of each patient with Paget's disease. The nuclear inclusion most closely resembles viral nucleocapsids of the measles type.
