Sensing the skeleton: sensorineural regulation in bone.

The traditional view of bone tissue as an inert mechanical scaffold has been fundamentally revised over time. Perhaps nowhere is this more clear than in the emerging field of 'skeletal neuroscience', which has revealed the skeleton to be a densely innervated organ that senses and integrates mechanical, immune and metabolic stimuli. Here, we review recent evidence for bidirectional crosstalk between somatosensory neurons and the bone microenvironment, and highlight how techniques such as whole-tissue clearing, genetic tracing and single-cell transcriptomics have mapped osteo-neural interactions

Molecular mechanisms and therapeutic prospects of short-chain fatty acid-mediated regulation of rheumatoid arthritis via the gut-joint axis.

Rheumatoid arthritis (RA) is characterized by chronic synovial inflammation and progressive joint destruction, and approximately 30-40% of patients respond inadequately to currently available disease-modifying antirheumatic drugs. Growing evidence suggests that immune abnormalities driving joint inflammation may arise at mucosal sites, including the gut, lung, and periodontium, before the onset of clinical arthritis. Short-chain fatty acids (SCFAs), principally acetate, propionate, and butyrate, are major metabolites generated by anaerobic gut bacteria through dietary-fiber fermentation and pa

Megakaryocytes in myelofibrosis: mechanisms of fibrotic niche remodeling and therapeutic implications.

Myelofibrosis is a clonal myeloproliferative neoplasm characterized by progressive remodeling of the bone marrow microenvironment. Megakaryocyte (MK) abnormalities are defining histological features of prefibrotic and overt primary myelofibrosis and contribute to disease pathogenesis. Dysplastic MKs can directly modify the pericellular extracellular matrix (ECM), but their main fibrogenic influence comes from coordinating stromal, immune, and endothelial responses. This Review synthesizes evidence linking abnormal MK biology to fibrotic progression and examines its implications for biomarkers

Diagnostic Updates for Polyuria-Polydipsia Syndromes.

OBJECTIVE: Subclinical macrovascular impairment, which has been evaluated with various arterial stiffness (AS) parameters, has been defined in patients with systemic sclerosis (SSc). However, studies investigating the relationship between AS and clinical endpoints in SSc are lacking. This study aims to determine the prognostic value of AS parameters to predict all-cause mortality in SSc patients. PATIENTS AND METHODS: AS parameters [carotid-radial pulse wave velocity (PWV) and augmentation index (AIx)] were assessed via applanation tonometry. The prognostic value of these parameters was quant

Immune Involvement in the Development and Maintenance of Oral and Craniofacial Bones.

The oral and craniofacial bones are essential for determining individual appearance and enabling mastication, respiration, and phonation. The cranium exhibits a complex architecture composed of multiple small bones. The development of this structured organization is a consequence of the well-coordinated functions of various bone cells, such as chondrocytes, osteoblasts, osteoclasts, and osteocytes. These cells contribute to both the development and maintenance of bone, known as the modeling and remodeling phases, respectively. The differentiation and function of these bone cells can be influen

IRE1 signaling in osteoprogenitors augments β-catenin activity and physiologic bone accrual.

The endoplasmic reticulum (ER) orchestrates the folding of the large amounts of membrane and secretory proteins that are synthesized during the process of osteogenesis. The unfolded protein response (UPR) resulting from accumulation of misfolded proteins in the ER lumen either promotes or inhibits osteoblast differentiation in vitro depending on magnitude and duration. All three transducers of the UPR, namely, IRE, PERK, and ATF6 proteins, have been implicated in skeletal biology, yet their specific contribution to osteoblast differentiation and function in vivo has not been investigated syste

The role of osteomimicry factors in prostate cancer progression and metastasis.

Prostate cancer progression and metastasis are complex steps that are controlled by various molecular and cellular mechanisms. Here, the process of osteomimicry has a vital role in the context of bone metastasis. Osteomimicry phenomenon refers to the ability of cancer cells to acquire bone-like properties, thus enabling them to adapt to and survive in their bone microenvironment. This phenomenon promotes cancer cell and bone microenvironment interactions and contributes directly to tumor survival, growth, and the development of bone metastatic lesions. In this review, we discuss the role of di

GOLGB1 deficiency accelerates intervertebral disc degeneration by activating the MAPK pathway.

Intervertebral disc degeneration (IDD) is a multifactorially regulated age-related condition. Golgin B1 (GOLGB1) is a large Golgi-related transmembrane protein encoded by golgb1 gene. Since its discovery, GOLGB1 has been proved to be related to many diseases. However, its role in IDD is still unclear. The aim of this research was to examine whether GOLGB1 deficiency accelerates disc degeneration and to study its mechanism. We selected C57BL/6 male mice aged 8 weeks to isolate, culture and identify nucleus pulposus (NP) cells, and used a variety of experimental techniques, including plasmid tra

Targeting the Autophagy-Apoptosis Axis in Osteosarcoma: Therapeutic Potential of Biocompounds: A Review.

Osteosarcoma is a prevalent form of cancer that develops in growing bones. The precise cause of this multifaceted disease remains uncertain. Yet, genetic, genomic and proteomic research has enhanced our understanding of the disruption in its molecular signaling pathways. In this review, we attempted to highlight the regulation of autophagy pathways by a range of biocompounds (chemical compounds of biological origin) in osteosarcoma. Autophagy is a degradation mechanism, maintaining cellular homeostasis under basal and stress conditions. However, the role of autophagy is variable in cancer. The

Functionalized osteogenic extracellular vesicles derived from BMP9-stimulated mesenchymal stem cells (MSCs) effectively induce bone regeneration.

Effective healing of large bone defects is a clinical challenge. Extracellular vesicle (EV)-mediated cell-free therapy has great potential in bone defect repair; however, unmodified EVs generally exhibit limited osteogenic potential. Bone morphogenetic protein 9 (BMP9) is one of the most potent osteogenic cytokines reported to date. Here, we modified the EVs from BMP9-stimulated adipose-derived mesenchymal stem cells (iMAD), designated as B9-EVs. Subcutaneous injections of B9-EVs in immune-competent mice elicited no detectable host immune response. B9-EVs effectively induced in vitro osteogeni

MEK inhibitor mirdametinib promotes fracture healing in osteofibrous dysplasia RASopathy.

Osteofibrous dysplasia (OFD) is a skeletal RASopathy presenting with periosteal bone lesions that may progress to fracture and delayed healing (pseudarthrosis). MET gene mutations reducing ubiquitin-mediated protein degradation via loss of the juxtamembrane domain (METΔJMD) were previously identified in patients with OFD, resulting in ligand-dependent gain of function. The effect of METΔJMD expression on skeletal progenitor cell differentiation and the potential efficacy of targeted therapies remain unclear. We engineered MetΔJMD mice and showed that MetΔJMD expression inhibited osteogenic dif

SPIN4-related X-linked overgrowth in a family.

Spindlin Family Member 4 (SPIN4) is an epigenetic reader gene on the X chromosome. Its loss-of-function variant altering the WNT/β-catenin pathway was recently reported to cause a SPIN4-associated overgrowth syndrome in an extended family. The index case is a 14-year-old male with tall stature (+2.0 SD) as the only growth-related finding, accompanied by protruding joints, splenomegaly, low bone mineral density, and normal intelligence. Exome sequencing identified the same loss-of-function variant of SPIN4 (NM_001012968.3:c.312_313del:p.(Arg104Serfs*24)), which was identified in the original fa

Investigating the Role of MicroRNAs in Mesenchymal Stem Cell Osteogenic Differentiation.

Mesenchymal stem cells (MSCs) are multipotent progenitors that give rise to osteoblasts, which are essential for bone formation, remodeling, and skeletal homeostasis. MicroRNAs (miRNAs), small non-coding RNA molecules, have emerged as critical post-transcriptional regulators of gene expression that fine-tune MSC commitment, osteoblast proliferation, and matrix mineralization. In this review, we first summarize the physiological mechanisms of bone remodeling and the differentiation of MSCs into osteoblasts, with particular attention to the coordinated roles of osteoblasts, osteoclasts, and oste

The Association of Bone Stress Injuries With Body Mass Index Percentile Drop and Eating Disorder Diagnoses in Adolescent Athletes.

CONTEXT: Adolescence is a critical period for bone development. Due to an elevated risk for low body mass index (BMI) and/or low energy availability, certain adolescent athletes are at increased risk of bone stress injury (BSI) and eating disorders (EDs). Despite this risk, the incidence of EDs in patients with BSI is unclear. OBJECTIVE: To compare the incidence of a premorbid drop in BMI percentile in a sample of adolescent athletes diagnosed with a BSI between 2005 and 2016. We also evaluated the relative risk of developing an ED for this sample, compared with a sex-matched, age-matched (±6

Established and emerging non-cellular therapies in inherited bone marrow failure syndromes.

Inherited bone marrow failure syndromes (IBMFs) are a molecularly heterogeneous group of genetically-determined syndromes with a wide spectrum of clinical abnormalities and risk of hematopoietic neoplasia and reduced life expectancy. The prevalence is not well established but is growing due to the advent of genetic diagnostics. Allogeneic hematopoietic stem cell transplantation (HSCT) remains the only curative modality in eligible IBMF patients but the indications are individualized due to significant transplant-related morbidity, including secondary cancer. HSCT indications and practices are

A cartilage-targeted IGF-1-antibody fusion protein as a new therapeutic approach for IGF-1 deficiency.

Growth hormone (GH) insensitivity syndrome (GHIS) is a childhood growth disorder characterized by an inability to generate insulin-like growth factor-1 (IGF-1) in response to GH. Consequently, GH therapy is ineffective in patients with GHIS. Patients are often treated with recombinant IGF-1 instead, which requires twice-daily injections and is associated with adverse effects including hypoglycemia. In the current study, we evaluated CV1623-1, a cartilage-targeted antibody-like IGF-1 fusion protein as a potential new treatment for GHIS and for other disorders of linear growth involving IGF-1 de

Dose-dependent effects of estrogen-enriched bone cement on membrane quality in the Masquelet technique: experimental rat model.

The Masquelet technique is a 2-stage surgical method used for the reconstruction of critically sized bone defects due to various etiologies. Estrogen is known to have anabolic effects on bone metabolism. In this study, based on literature data, the aim is to systematically evaluate the histological and immunohistochemical effects of adding different doses of estrogen to polymethylmethacrylate cement on induced membrane tissue.  Methods: Twenty-eight male rats were included in the study and divided into 4 groups. Three experimental groups received different doses of estradiol, a subtype of estr

Bone transport accelerates diabetic foot ulcer healing via TGF-β1-mediated angiogenic and osteo-immune coupling.

Diabetic foot ulcer (DFU), often complicated by peripheral artery disease and local ischemia, remains a major clinical challenge. Bone transport (BT) is a surgical technique known to promote both osteogenesis and angiogenesis, and has shown promise in facilitating DFU healing. However, the underlying molecular mechanisms, especially the role of the TGF‑β1 signaling pathway, are not yet fully understood. Seventy-five Sprague-Dawley rats with ischemic diabetic foot ulcers (DFUs) were randomly assigned to three groups: sham (osteotomy without distraction), BT, and BT with TGF-β1 pathway inhibitio
