We describe three patients in whom at the age of 4-7 yrs the diagnosis adrenogenital syndrome (21-hydroxylase deficiency) was made. There was no salt-loss. Two boys presented with precocious pubic hair development and increased growth velocity and bone maturation. The third patient was a severely virilized girl raised as a boy until she was 4 yrs old. Following an extensive psychiatric evaluation, it was decided to raise her as a girl and feminoplasty was performed. A hematologic malignancy occurred simultaneously with a malignant mediastinal germ cell tumor in two men. In one instance the blood disorder was acute nonlymphocytic leukemia, and in the other, malignant histiocytosis. We subsequently identified nine additional patients, all male (age range 12-48 years), with a mediastinal germ cell tumor and either acute nonlymphocytic leukemia or malignant histiocytosis. Anthropometric data were collected on groups of seven-year-old Dunedin (New Zealand) boys, identified as hyperactive-only, aggressive-hyperactive, aggressive-only and non-aggressive/non-hyperactive. Measures taken included stature, weight, skeletal maturity, mid-upper arm circumference, triceps and subscapular fatfolds and body-mass index. The hypothesis that hyperactive-only boys would show delayed maturation, as indicated by lower bone-age, was not confirmed. However, these boys had significantly smaller mid-upper arm circumference, triceps fatfold and body-mass index than the other groups.

Clinical pictures of 21 cases with I-cell disease patients, 12 males and 9 females, were analyzed. Characteristic coarse facial features and shortness of stature were observed in all cases. Several specific disorders have been discussed which show the effects of endochondral and membranous defects in the craniofacial complex. Posterior choanal atresia has a significant incidence of associated defects. Recently a constellation of defects, bearing the acronym of CHARGE, has been described.
