A 20-year follow-up of a patient with Klippel-Trenaunay syndrome: case report.

INTRODUCTION: Klippel-Trenaunay syndrome (KTS) is a rare congenital malformation characterized by a clinical triad: capillary malformations, varicose veins, and hypertrophy of soft tissues or bones in one or more limbs. Differential diagnosis from Parkes Weber syndrome is crucial; the primary distinguishing feature is the presence of arteriovenous fistulas. KTS is progressive. Management of KTS is predominantly conservative, involving lifelong elastic compression therapy and regular courses of venotonics. This case report presents the clinical follow-up of a patient with KTS over a period of m

Achieving Accuracy and Gap Balancing in Fully Autonomous Robotic-Assisted Total Knee Arthroplasty with Functional Alignment in Valgus Knee Deformity.

INTRODUCTION: The robotic-assisted total knee arthroplasty (RA-TKA) facilitates real-time intra-operative balance assessment and accurate component positioning customized to the patient's ligamentous behavior, enhancing procedural accuracy and precision. Preliminary findings suggest RA-TKA, using fully autonomous computed tomography based systems, such as Cuvis, result in better short-term outcomes and improved patient-reported outcome measures. Coronal plane alignment of the knee classification aids to decide pre-arthritic phenotype of the knee and soft tissue balance judgment. MATERIALS AND

Microfibrillar-associated protein 5 suppresses adipogenesis by inhibiting essential coactivator of PPARγ.

Femoral head necrosis is responsible for severe pain and its incidence is increasing. Abnormal adipogenic differentiation and fat cell hypertrophy of bone marrow mesenchymal stem cells increase intramedullary cavity pressure, leading to osteonecrosis. By analyzing gene expression before and after adipogenic differentiation, we found that Microfibril-Associated Protein 5 (MFAP5) is significantly down-regulated in adipogenesis whilst the mechanism of MFAP5 in regulating the differentiation of bone marrow mesenchymal stem cells is unknown. The purpose of this study was to clarify the role of MAFP

Hyperplasia and accelerated hypertrophy of marrow adipocytes with knee immobilization were sustained despite remobilization.

Skeletal disuse can cause an accumulation of bone marrow adipose tissue (MAT) characterized by a combination of marrow adipocyte hyperplasia and/or hypertrophy. The malleability of MAT accumulation and of the hyperplasia and hypertrophy upon remobilization is unknown. In this study, we showed extensive hyperplasia and accelerated hypertrophy of bone marrow adipocytes in the proximal tibia epiphysis of rat knees immobilized for durations between 1 and 32 wk. Similar histomorphometric measures of adipocytes carried out in unoperated controls allowed distinguishing the effects of immobilization f

Port-wine stains associated with large vestibular aqueduct syndrome caused by mutations in GNAQ and SLC26A4 genes: A case report.

Port-wine stains (PWS) are capillary malformations associated with mutation in the GNAQ (NM_000441.1) gene. Large vestibular aqueduct syndrome (LVAS), caused by mutation in the SLC26A4 (NM_002072) gene, is an inner ear malformation that can lead to hearing loss. To our knowledge, LVAS in PWS patients has never been reported. Here, we describe a case of a 9-year-old female patient diagnosed with PWS on the face and neck, coexisting with large vestibular aqueduct syndrome. Further analyses revealed a somatic mutation in GNAQ and a compound heterozygous mutation in the SLC26A4 gene. Some PWS pati

Which plate results in better stability after segmental mandibular resection and fibula free flap reconstruction? Biomechanical analysis.

OBJECTIVE: This study investigated the biomechanical stability of 2 plate systems-mini-plates and reconstruction plates-in reconstruction with fibular free flaps. STUDY DESIGN: The reconstruction models were constructed by using 2 types of plates in representative cases with segmental mandibular defect (C, L, LC1, LC2). In each model, a masticatory simulation approximating 3 clenching tasks was conducted, using the muscle forces adjusted to the mandible structure used in this study. In addition, to evaluate the sensitivity of the 2 plate systems for masticatory load changes, a sensitivity ana

Infiltrative therapy as conservative treatment in hip osteoarthritis: a literature review.

Osteoarthritis (OA) is a very common disease, its prevalence increases with age and is a frequent cause of disability.Osteoarthritis is characterised by joint pain, stiffness and loss of range of motion. Overall, as many as 40% of those aged over 65 years in the community may have symptomatic OA of the knee or hip (1). OA results from a complex interaction of biomechanical and biochemical factors and is characterised by cartilage disruption and hypertrophy of bone. Intraarticular proinflammatory cytokines and proteinases in OA interfere with the synthesis of hyaluronic acid (HA), a complex gly

Transoral submucosal resection of the inferior turbinate: a novel approach to functional rhinoplasty.

BACKGROUND: In the presence of turbinate dysfunction, an inferior turbinectomy for persistent hypertrophy of bone and/or mucosa may be performed. We sought to explore anatomic feasibility of a transoral turbinectomy. METHODS: After transoral inferior turbinectomy in 12 cadavers, average distances from the external nasal valve to inferior turbinate and from pyriform aperture to inferior turbinate were compared. Average "area of access" was calculated. Preoperative and postoperative nasal length, tip projection, and alar-base width were also compared. RESULTS: Average distance from external na

[Anesthetic management of a patient with Klippel-Trenaunay syndrome undergoing caesarean section].

Klippel-Trenaunay syndrome (KTS) is characterized by capillary and venous malformation and hypertrophy of bone and soft tissues. A 29-year-old primigravida, who had been diagnosed of KTS by her hemangiomas and varicosities in the right leg, pubic area, rectum, vagina and lower abdominal area, was scheduled to receive caesarean section at 37 weeks gestation because vaginal delivery might cause hemorrhagic complications and extension of the venous lesions. Regional anesthesia was avoided because of the possible injuries of unknown venous malformations or varicose veins in the epidural or spinal

High frequency tympanometry in neonates with normal otoacoustic emissions: measurements and interpretations.

BACKGROUND: Drugs inhibiting cholesterol biosynthesis may affect bone metabolism through inhibition of the mevalonate pathway resulting in the inhibition of protein prenylation required for osteoclast activity. D-003 is a mixture of high molecular weight aliphatic primary acids purified from sugar-cane (Saccharum officinarum) wax, with cholesterol-lowering effects demonstrated in experimental and clinical studies. D-003 inhibits cholesterol biosynthesis through indirect regulation of HMG-CoA reductase activity. A previous study demonstrated that D-003 prevented bone loss and bone resorption on

Bilateral optic nerve drusen and gliomas in Klippel-Trenaunay syndrome.

Klippel-Trenaunay syndrome (KTS) consists of a vascular nevus involving an extremity, varicosities of that extremity, and hypertrophy of bone and soft tissue. When arteriovenous malformation is also present, it is called Klippel-Trenaunay-Weber syndrome (KTWS). Ophthalmic features of these syndromes include vascular anomalies of the orbit, iris, retina, choroid, and optic nerve. We report a case of a 16-year-old girl with KTS who was found to have bilateral optic nerve and chiasmal gliomas, optic disk drusen, and acquired myelination of the retinal nerve fiber layer. These findings have not be

Magnetic isolation of particles suspended in synovial fluid for diagnostics of natural joint chondropathies.

Millions of people are stricken with the degenerative joint disease known as osteoarthritis. Osteoarthritis is associated with biochemical and mechanical processes, and is characterized by loss of articular cartilage and hypertrophy of bone. As cartilage and bone particles are released into the synovial fluid, a variety of biomarkers have been suggested for the analysis of this fluid. Here we have developed a method for isolating bone and cartilage wear particles suspended in the synovial fluid of the hip, knee and ankle joints of humans, based on specific magnetization of collagens I and II.

Subcellular localization of IRS-1 in IGF-I-mediated chondrogenic proliferation, differentiation and hypertrophy of bone marrow mesenchymal stem cells.

Bone marrow derived mesenchymal stem cells (BM-MSC) can differentiate into chondrocytes. Understanding the mechanisms and growth factors that control the MSC stemness is critical to fully implement their therapeutic use in cartilage diseases. The activated type 1 insulin-like growth factor receptor (IGF-IR), interacting with the insulin receptor substrate-1 (IRS-1), can induce cancer cell proliferation and transformation. In cancer or transformed cells, IRS-1 has been shown to localize in the cytoplasm where it activates the canonical Akt pathway, as well as in the nucleus where it binds to nu

The response of bone to mechanical loading and disuse: fundamental principles and influences on osteoblast/osteocyte homeostasis.

Bone's response to increased or reduced loading/disuse is a feature of many clinical circumstances, and our daily life, as habitual activities change. However, there are several misconceptions regarding what constitutes loading or disuse and why the skeleton gains or loses bone. The main purpose of this article is to discuss the fundamentals of the need for bone to experience the effects of loading and disuse, why bone loss due to disuse occurs, and how it is the target of skeletal physiology which drives pathological bone loss in conditions that may not be seen as being primarily due to disus
