Hepatoid adenocarcinoma of the lung is a rare primary pulmonary malignancy that exhibits morphological traits resembling those seen in hepatocellular carcinoma and is associated with high alpha-fetoprotein expression. It follows a very aggressive course and is often diagnosed in advanced oncologic stages. We present our experience with this malignancy and report on a case with mandibular metastasis, an uncommon metastatic site. The patient, a 64-year-old male smoker with an estimated 50-pack-year history, was evaluated for a solid lesion in the left upper lobe, accompanied by hypertrophic pulm

BACKGROUND: Primary hypertrophic osteoarthropathy (PHO) is a rare hereditary clinical syndrome characterized by digital clubbing, periostosis, and pachydermia. It is mainly caused by mutations in SLCO2A1 or HPGD, leading to impaired degradation and elevated levels of prostaglandin E2 (PGE2). In addition to the typical skeletal and skin manifestations, some patients may present with gastrointestinal or hematologic abnormalities, including anemia and myelofibrosis. This report describes a case of PHO with myelofibrosis and compound heterozygous SLCO2A1 mutations and review the literature.

BACKGROUND: Osteoporosis is a major global health concern, increasing the risk of fragility fractures, morbidity, and mortality. Conventional diagnostic tools such as dual-energy X-ray absorptiometry (DXA) have limitations in accurately predicting fracture risk. Recent studies suggest that microRNAs (miRNAs) play a crucial role in bone metabolism and may serve as potential biomarkers for osteoporosis and fracture susceptibility.
OBJECTIVE: This study aimed to identify differentially expressed miRNAs in osteoporotic and non-osteoporotic trauma patients with proximal femur fractures and assess

CONTEXT: Pachydermoperiostosis (primary hypertrophic osteoarthropathy, PHO), usually due to biallelic loss of function variants in HPGD and SLCO2A1, has some features overlapping with acromegaly and is often referred to endocrinologists. A detailed endocrine assessment is not available for these patients.
OBJECTIVE: To assess the genetic and endocrine characteristics of patients with PHO referred to endocrine centers with a possible diagnosis of acromegaly.
METHODS: Seventeen patients from 14 families in which acromegaly was excluded based on lack of elevated insulin-like growth factor (IGF)

A 48-year-old male displayed pain along with swelling of small joints of the hand and feet, along with low back pain, for the last 7-8 years. The joint pain was not associated with early morning stiffness or constitutional symptoms. On physical examination, all fingers and toes had bulbous enlargement and clubbing (Figs 1A and B). There were no skin changes and no motion restriction of any joint. Other systemic examination was normal. The routine investigations along with inflammatory markers and immunological tests were normal. A radiograph of bones showed periosteal changes in long bones as

Primary autosomal recessive hypertrophic osteoarthropathy (PHOAR) type 1 is caused by the failure of the HPGD gene product to break down prostaglandins. We report the case of a two-year-old male patient diagnosed with PHOAR1 due to a previously unreported homozygous intragenic deletion. Upon retrospective review of the patient's history, his clinical course proved to be typical of this syndrome, although it had been unrecognized. This case report presents an opportunity to review the natural history, clinical signs, diagnosis, and treatment of PHOAR1, with the aim of raising awareness of this

SLCO2A1 is a member of the organic anion transporting polypeptide (OATP) family, which preferentially transports prostaglandins (PGs) into cells and plays a vital role in regulating PGs inactivation and distribution. Dysregulation or genetic mutation of SLCO2A1 is associated with primary hypertrophic osteoarthropathy (PHO) and chronic enteropathy associated with the SLCO2A1 gene (CEAS). Although the biophysical and biochemical properties of SLCO2A1 have been characterized, the precise mechanism by which SLCO2A1 recognizes and transports PGs remains unclear. Here, we present the cryo-electron m

Digital clubbing, excruciating polyarthritis, and periosteal thickening of long bones are all features of the uncommon clinical syndrome known as hypertrophic osteoarthropathy (HOA). The most frequent association is lung cancer, which often presents as a paraneoplastic syndrome. We report the case of a 54-year-old male smoker who presented with joint pain and digital clubbing. Further investigations revealed a stage IV primary lung adenocarcinoma. Bone scintigraphy, performed after the diagnosis of the primary tumor, demonstrated increased uptake in the cortical bone of the lower limbs, with h

PURPOSE OF REVIEW: Cancer cachexia (CC) is a paraneoplastic syndrome (PNS) that is characterised by anorexia, weight loss, fatigue and reduced function. This review explores the molecular drivers of CC and other PNS, identifying shared pathways and highlighting unexplored gaps in research.
RECENT FINDINGS: Recent studies have provided further evidence of pro-inflammatory cytokines, such as interleukin-6 and tumour necrosis factor-α, as central players in both CC and PNS, emphasising their role in systemic effects like muscle wasting, lipolysis and pyrexia. Despite these overlaps between syndr

Hypertrophic pulmonary osteoarthropathy (HPOA) is a paraneoplastic syndrome characterised by digital clubbing, periostitis, and arthralgia, most commonly secondary to lung malignancy. This case report presents a man in his 60s with a long history of smoking and asbestos exposure who developed progressive wrist and ankle pain, limb swelling, and enlargement of the hands and feet over eight months. He was initially investigated for rheumatoid arthritis due to positive rheumatoid factor (RF) and anti-citrullinated peptide (anti-CCP) antibodies, but repeated chest X-rays were normal. Pachydermoperiostosis (PDP), also known as primary hypertrophic osteoarthropathy, is a rare genetic disorder characterized by digital clubbing, periostosis, and pachydermia. It typically presents during adolescence or early adulthood and is commonly associated with mutations in the HPGD or SLCO2A1 genes. Due to its nonspecific clinical features, PDP is often misdiagnosed, resulting in delayed treatment. We report the case of a 26-year-old Arab male with an eight-year history of progressive facial skin thickening, hyperhidrosis, and digital clubbing. Laboratory investigations were unremarkable,

Pachydermoperiostosis (PDP), or primary hypertrophic osteoarthropathy, is a rare genetic disorder characterized by digital clubbing, skin thickening, and periostosis. Due to its overlapping clinical features with secondary hypertrophic osteoarthropathy, it often poses a diagnostic challenge. We report a case of a 20-year-old male patient who presented with progressive digital clubbing, joint pain, and coarse facial features over four years. The patient also exhibited gynecomastia, an uncommon feature in PDP. Despite no history of underlying pulmonary, cardiac, or gastrointestinal disease, his

BACKGROUND AND OBJECTIVE: Data on file with the US FDA, and other published studies, suggest that the selective cyclo-oxygenase (COX)-2 inhibitor NSAID rofecoxib has a greater hypertensive adverse effect than other NSAIDs, including celecoxib. In this study we describe a pharmacoepidemiologic analysis of spontaneous adverse event reports of acute, clinically serious hypertension (as defined by hospitalisation) reported in association with rofecoxib, celecoxib, nabumetone and oxaprozin. The objective of this analysis is to assess whether postmarketing data are consistent with results of clinica

UNLABELLED: Chronic arthritis in children is widely associated with juvenile idiopathic arthritis (JIA); however, several rare monogenic disorders may mimic its clinical presentation. Misdiagnosis can result in unnecessary immunosuppressive treatment and delay appropriate care. This study aimed to share our experience with monogenic disorders presenting as chronic arthritis and highlight their distinguishing clinical and imaging features. This retrospective cohort study included patients initially suspected of having JIA who were later diagnosed with a monogenic disorder. Clinical, laboratory,

Paraneoplastic rheumatologic syndromes encompass a range of clinical conditions that mimic primary rheumatic diseases and occur in the context of malignancy. Hypertrophic osteoarthropathy (HOA) is a notable example of such a syndrome, which is frequently associated with intrathoracic malignancies, including primary lung tumors and metastases. This report presents a case of a patient diagnosed with HOA secondary to lung adenocarcinoma, who was admitted with symmetric polyarthritis that resembled elderly-onset rheumatoid arthritis. Anti-cyclic citrullinated peptide (anti-CCP) antibodies are prim
