Hypertrophical osteortropathy syndrome, which consists of arthritis, periostitis, growth of the periarticular soft tissue and fingers like drumsticks, is associated to several cardiopulmonary sufferings, its relation to cyanotic congenital cardiopathies has been rarely informed. Two cases of this rare association are described in this article, and the possible patogenic mechanisms are discussed. To distinguish this syndrome from other rheumatic sufferings such as Rheumatic Fever, Juvenile Rheumatoide Arthritis or Gout is an important point, because its treatment and prognostic are different.

A patient who, eight years prior, had a Heller operation for achalasia developed hypertrophic osteoarthropathy possibly secondary to a leiomyoma of the esophagus with sarcomatous changes. No local invasion or metastases were found. Total esophagectomy completely cured the esophageal symptoms and relieved the swelling and pain of her knees and other joints. This case illustrates the hypertrophic osteoarthropathy syndrome, its marked diagnostic significance and its occurrence secondary to an esophageal leiomyoma and leiomyosarcoma.

A study of a unique case of unilateral digital clubbing secondary to a true posttmaumatic aneurysm of the ulnar artery in the palm is presented. The clubbing of the digits resolved following resection of the aneurysm. Detailed evaluation of the patient included Doppler flow studies, differential pulse pressure recordings, measurement of reactive hyperemia, angiography, and differential capillary blood gas determinations. The contralateral hand of this otherwise healthy young patient was employed as a control. Findings of increased blood flow, decreased peripheral resistance, and increased oxyg

A case of an osteoblastoma of the proximal femur with a unique local, massive reactive periostitis mimicking osteosarcoma or osteomyelitis and unique systemic manifestations is reported. The severe toxic manifestations included: massive weight loss, chronic fever, anemia, systemic periostosis, and other signs. Due to confusion as to diagnosis, lack of response to numerous antibiotic regimens, and severe cachexia with clinical signs of impending death, an amputation was performed. Pathologic study revealed an osteoblastoma. A thorough review of the case suggests that the signs and symptoms were

Finger clubbing, measured objectively by using the hyponychial angle, was present in 75 out of 200 (38%) patients with Crohn's disease, 15 out of 103 (15%) with ulcerative colitis, and two out of 24 (8%) with proctitis. In Crohn's disease and ulcerative colitis the hyponychial angle was significantly correlated with both disease activity and the extent of fibrosis in the resected specimens from 47 surgically treated patients. The prevalence of finger clubbing in patients with macroscopic disease within the area of the gut innervated by the vagus nerve was significantly higher than that in pati

Of 485 cases of NPC collected from the files of our institute between March 1969 and December 1974, 82 (17%) were in children and adolescents (0--19 years old). This relatively high frequency of NPC in young people was not suggested by reports from high-incidence areas (Southern China, for example) but appears to be a characteristic feature of areas of intermediate incidence (Uganda, Kenya, Sudan, Tunisia). NPC is the tumour that occurs most frequently in young people between 1 and 20 years old in Tunisia, showing a peak in those 16 years of age. The male:female sex ratio was 3:1. Advanced sta

Nine patients with cystic fibrosis but without digital clubbing (Group A) were matched, prospectively, by sex and approximate age to 9 cystic fibrosis patients with digital clubbing (Group B) and to 9 normal persons (control subjects). Patients in Group B had significantly (P less than 0.05) lower clinical scores and forced vital capacity than did those in Group A, indicating more severe pulmonary disease in the former; however, other spirometer tests of pulmonary function revealed no differences between Groups A and B. The degree of digital clubbing had significant (P less than 0.05) linear r

The bone scan appearance of hypertrophic pulmonary osteoarthropathy (HPO) in patients with pulmonary neoplasms and a variety of other lesions has been described by several investigators. It is well known that after therapy, the bone scan changes resolve. This report deals with a patient who had no therapy for a pulmonary neoplasm. In a study done three months later, changes in his initial bone scan that had suggested pulmonary osteoarthropathy had resolved completely, even though the patient had no treatment for his carcinoma.

Based on a review of the embryology, genetics, and anatomy of radial club hand, it is suggested that damage to the apical ectoderm on the anterior aspect of a developing limb bud leads to the deformity. Study of the families of thirty-five children with radial club hand suggested that the condition is not genetically patterned. The anatomical findings and associated congenital abnormalities in the cases known to be related to thalidomide and in those in which thalidomide was not a factor were similar except that the incidence of other skeletal deficiencies was higher in the thalidomide group.

Detailed radiological and histological features of alpha-chain disease are presented against a background of the clinical and biochemical findings. The separation of this syndrome from other varieties of "Mediterranean" lymphoma is emphasized. The radiologist should in some instances be able to suggest the diagnosis in his differential diagnosis of abnormalities seen on examination of the small bowel. The histopathologist should be able to provide a diagnosis from examination of a peroral jejunal biopsy. One patient with this relatively uncommon disease had evidence of a hypertrophic osteoarth

A case of nodular sclerosis Hodgkin's disease, stage IVB, with lung involvement and hypertrophic pulmonary osteoarthropathy (HPOA), was treated with quadruple cytotoxic chemotherapy. The pulmonary lesions, which were thin walled cavities, and the lymphadenopathy resolved completely after two courses of chemotherapy. The clubbing and all evidence of periosteal new bone formation disappeared after six courses of treatment. Complete reversal of the syndrome by chemotherapy has not been previously described. A literature review revealed 13 more cases of Hodgkin's disease associated with HPOA. The

Finger clubbing, protein-losing enteropathy, and iron deficiency were documented in three children with severe gastroesophageal reflux. One patient had Sandifer syndrome and the other two had the rumination syndrome. In each case, surgical repair of the gastroesophageal reflux resulted in immediate clearing of signs of the Sandifer syndrome, gastroesophageal reflux, and anemia and the return of serum protein levels to normal. There was definite regression of the finger clubbing during the ensuing year. It is suggested that finger clubbing, protein-losing enteropathy, Sandifer syndrome, and rum

Mast cell counts were made, in tissue samples of normal and clubbed fingers obtained at autopsy, to investigate the possibility that mast cells act as an intermediary in the development of finger clubbing. Quantitative measurements of the composition of the tissues of the normal and clubbed nail bed were also made. The only significant difference in mast cell numbers in clubbed fingers was a decrease in mast cells in the tissues just superficial to the nail. The appearance of the mast cells was normal. The porportion of blood vessels and other tissues was not significantly different from norma

Three patients with cystic fibrosis were noted to have swelling of knee and ankle joints during exacerbation of their lung disease. Synovial fluid was analyzed in one patient and the synovium underwent biopsy in another. These studies excluded other causes of arthritis but did not contribute any new information on the nature of secondary hypertrophic osteoarthropathy. Radiological examination of long bones confirmed the diagnosis of hypertrophic osteoarthropathy in all three patients. Since many patients with cystic fibrosis survive longer, more instances of hypertrophic osteoarthropathy are e

A case of Fallot's tetralogy associated with hypertrophic osteoarthropathy in a young Nigerian female is described. The clinical spectrum of hypertrophic osteoarthropathy is reviewed. The rarity of this syndrome is stressed. Some other aspects of the clinical manifestation of cyanotic congenital heart disease which may mimic the skeletal syndrome are mentioned.

In a prospective study of 170 patients with various types of chronic liver disease, a pulmonary gas transfer defect was found in 20% and mottled shadowing in the chest radiograph in 6%. The presence of these abnormalities was not related to the cause of the liver disease. Reduction of transfer factor in liver disease was not accompanied by a restrictive ventilatory defect and was found in most cases with mottled radiographs. The incidence of finger clubbing and the levels of both venoarterial admixture and cardiac output were higher in patients with mottling than in those with normal radiograp

The recognition of morphological stigmata other than cardiac, which are now known to be associated with congenital heart disease, coupled with a familial occurrence, may permit diagnosis of specific cardiac lesions at a very early age. Eleven such morphological associations have been reviewed.

In a retrospective study of 399 patients with ulcerative colitis, 27 patients had colitic arthritis, 17 had ankylosing spondylitis, and 20 had clubbing of the fingers. Colitic arthritis and ankylosing spondylitis were not related to severity, extent of involvement, or duration of colitis. A significant association between colitic arthropathy and other complications of ulcerative colitis, such as pseudopolyposis, perianal disease, eye lesions, skin eruptions, aphthous ulceration, and liver disease has been demonstrated. The outcome of the first referred attack of colitis in the presence of coli

In an unselected series of 185 patients with histologically confirmed bronchial carcinoma 16 had endocrine disturbances attributable to the tumour (excluding pulmonary osteoarthropathy). Of these, 11 patients had hypercalcaemia; three inappropriate secretion of antidiuretic hormone; one Cushing's disease; three hypertrophic osteoarthropathy; and one gynaecomastia. Cushing's disease and inappropriate antidiuresis are specifically associated with oat-cell tumours, and hypercalcaemia occurs most frequently with squamous carcinoma. A negative correlation exists between gynaecomastia and osteoarthr
