This review focuses on experimental models developed to study myeloma bone disease (MBD), a major cause of morbidity in multiple myeloma (MM). Under physiological conditions, bone remodeling is regulated by osteoclasts (OCs) and osteoblasts (OBs); in MM, this balance is disrupted, resulting in enhanced bone resorption and suppressed bone formation. Myeloma cells alter the bone marrow (BM) microenvironment by increasing the RANKL/OPG ratio and secreting Wnt pathway inhibitors such as DKK-1 and sclerostin, thereby promoting osteoclastogenesis and inhibiting osteoblast differentiation. To dissect

Nanoscale wear particles generated over time in the implant-bone interface induce profound periprosthetic inflammatory osteolysis, the most common complication after total joint arthroplasty, while macrophages serve as key initiators of this response. Ferroptosis represents a recently identified mode of regulated cell death distinguished by its nonapoptotic nature and reliance on iron-driven lipid peroxidation, which is strongly linked to inflammatory processes within macrophages. However, the contribution of macrophage ferroptosis to the development of wear particle-induced periprosthetic ost

INTRODUCTION: Total hip arthroplasty (THA) remains one of the most effective surgical interventions for advanced hip pathology, providing reliable pain relief and functional restoration. Despite widespread use, outcomes from different regions and clinical settings continue to be valuable for benchmarking the quality of care.
MATERIALS AND METHODS: A prospective observational study was conducted in the department of orthopedics at a tertiary care center in eastern India. Thirty adult patients undergoing uncemented THA for various hip pathologies were enrolled. All procedures were performed usi

INTRODUCTION: Metallosis is a rare but recognized complication following total knee replacement (TKR). It is characterized by metallic particle deposition within periprosthetic tissue, triggering chronic inflammation, progressive osteolysis, and implant loosening.
CASE REPORT: A 63-year-old female with bilateral TKRs, performed 12 and 15 years earlier, presented with pain, swelling, and restricted flexion of the right knee following a fall. She had a history of recurrent instability and locking for 3 years. Imaging revealed extensive osteolysis, polyethylene liner dissociation, and loosening

BACKGROUND: Despite the increasing use of MARS (metal artifact reduction sequence) MRI to investigate painful total hip arthroplasties (THA), no validated criteria exist for diagnosing femoral stem loosening.
PURPOSE: To evaluate MARS MRI for the diagnosis of aseptic stem loosening and determine its diagnostic accuracy.
MATERIALS AND METHODS: One hundred fourteen consecutive cases with THA revision surgery and MARS MRI of the hip were retrospectively included. Two blinded musculoskeletal radiologists independently assessed periprosthetic bone resorption (PPBR), bone marrow edema (BME), perio

BACKGROUND: Condylar stress fracture of the third metacarpal bone (MC3) is a common catastrophic injury in Thoroughbred racehorses and is associated with parasagittal groove (PSG) subchondral osteolysis. Standing computed tomography (sCT) imaging enables sensitive identification of this fatigue-induced early subchondral bone injury (SBI), but there is no objective method for identifying racehorses at heightened risk of condylar stress fracture.
OBJECTIVES: To estimate PSG first principal strain in elite Thoroughbred racehorses that have undergone subjective risk assessment using sCT fetlock s

Inflammatory osteolysis represents a critical complication following orthopedic interventions such as total joint replacement, primarily triggered by persistent inflammatory responses induced by prosthetic wear debris or bacterial components like lipopolysaccharides (LPS). Inflammatory osteolysis, a severe complication of orthopedic interventions like total joint replacement, is driven by prosthetic wear debris or lipopolysaccharides (LPS)-induced persistent inflammation and osteoclast activation. Current therapeutic strategies are limited by significant side effects and their inability to sim

Rho guanine nucleotide exchange factor (Rgnef/p190RhoGEF), a RhoA-specific guanine nucleotide exchange factor, has been implicated in cancer and amyotrophic lateral sclerosis, but little is known about its role in bone. Here we investigate the roles of Rgnef in bone metabolism using Rgnef-deficient and overexpressing mice. Compared with littermate wildtype mice, Rgnef-deficient mice had increased bone mass owing to lower osteolysis and higher osteogenesis, and Rgnef-overexpressing transgenic mice had the opposite bone phenotype. Rgnef deficiency inhibited osteoclast formation and resorptive fu

BACKGROUND: Reoperation rates in clavicular fractures and acromioclavicular (AC) joint dislocations are high, often indicated for plate removal due to plate prominence and poor cosmesis. The recently designed 2.7 mm variable angle locking compression plate (VA-LCP®) Clavicle Plate and 2.7 mm VA-LCP® Clavicle Hook Plate (J&J MedTech) are intended to have an improved plate-to-bone fit, are precontoured, and have a low profile. Additionally, the Clavicle Hook Plate has a more anatomically adapted hook design to reduce the risk of painful osteolysis leading to earlier plate removals. All screw hol

Bone metastases mark a critical and often terminal phase in cancer progression, where disseminated tumor cells (DTCs) manage to infiltrate and exploit the complex microenvironments of the bone marrow. While most current therapies focus on the well-known late-stage "vicious cycle" of osteolysis, they often overlook the earlier stages, namely, tumor cell colonization and dormancy. During these early phases, cancer cells co-opt hematopoietic stem cell (HSC) niches, using them as sanctuaries for long-term survival. In this review, we bring together emerging insights that highlight a trio of undera

Bone metastasis in breast cancer remains a major therapeutic challenge because current osteoclast-targeted therapies do not fully disrupt the tumor-bone vicious cycle. Osteocytes, the most abundant bone cells, are increasingly recognized as key regulators of bone-tumor crosstalk. Previous work has shown that osteocyte-specific overexpression of the Wnt co-receptor LRP5 inhibits breast cancer-induced osteolysis and generates conditioned medium (CM) with tumor-suppressive activity. Proteomic analysis identified LIM domain and actin-binding protein 1 (LIMA1) as a central mediator that interacts w

BACKGROUND: Total joint arthroplasty is an effective treatment for end-stage joint diseases, with approximately 1.5 million procedures performed globally annually and a 25%-30% annual growth rate in China. However, 10%-15% of patients develop prosthetic loosening or subsidence within 15-20 years postoperatively, predominantly due to aseptic loosening (incidence >10%) caused by wear particle-induced aseptic inflammatory osteolysis. The role of autophagy in this pathogenesis remains incompletely understood.
METHODS: A 61-year-old female patient developed aseptic loosening 11 months after left t

Talaromyces marneffei (T. marneffei), an opportunistic pathogen, mainly infects immunocompromised individuals and often causes osteolytic bone destruction. We designed the current study to investigate the therapeutic efficacy and intrinsic mechanism of luteolin (Lut) on T. marneffei-associated osteolysis. The potential therapeutic targets of luteolin were identified by RNA sequencing (RNA-Seq) and network pharmacology methods. GO (gene Ontology), KEGG (Kyoto Encyclopedia of Genes and Genomes), and GSEA (Gene Set Enrichment Analysis) enrichment analyses were used to reveal the molecular mechani

Non-coding RNAs, including piwi-interacting RNAs (piRNAs), are known to regulate osteogenic differentiation in bone marrow-derived mesenchymal stem cells (BMSCs); their role in mesenchymal stem cells (MSCs) from diverse origins remains unclear. In this study, we identified piR48444 as a key regulator that is downregulated during the osteogenic differentiation of stem cells from exfoliated deciduous teeth (SHED) but is upregulated in inflamed and aged BMSCs. Functionally, piR48444 inhibited, while its knockdown enhanced osteogenic differentiation across MSCs from multiple sources. Notably, piR4

BACKGROUND: Gorham-Stout disease (GSD), also known as vanishing bone disease, is a rare osteolytic disorder characterized by progressive bone resorption and proliferation of lymphatic and vascular channels. Thoracic involvement often leads to life-threatening respiratory complications, yet clinical recognition remains delayed due to its rarity and heterogeneity. OBJECTIVE: This review aims to synthesize recent advances in the understanding of GSD pathogenesis, highlight the disproportionate burden of respiratory complications-particularly chylothorax-and propose an integrated diagnostic and th

BACKGROUND: Periodontal ligament stem cells (PDLSCs) are essential for periodontal tissue regeneration, but their osteogenic differentiation is significantly impaired in the inflammatory microenvironment of periodontitis. This study aimed to investigate the regulatory role of the SENP6-SMAD5-SOX2 axis in PDLSC osteogenesis.
METHODS: Transcriptomic analysis of public datasets was used to identify key regulators. In vitro functional experiments, including siRNA-mediated knockdown, co-immunoprecipitation, and luciferase assays, were performed on PDLSCs isolated from healthy donors. The expressio

Osteosarcoma (OS) presents formidable challenges due to its aggressive progression and resistance to conventional therapies. Recent advancements in multimodal strategies, such as combining gene therapy with mild-temperature photothermal therapy (mPTT), referred to as GT-mPTT, have shown promise in enhancing therapeutic efficacy while minimizing side effects. However, current systems face limitations in therapeutic response, efficiency, and biosafety. Herein, we report a reactive oxygen species (ROS)-responsive polymeric amphiphile for imaging-guided combinational GT-mPTT therapy, achieving a h

Plantar puncture wounds are a common entity among children, particularly in areas where walking barefoot is common, but the detection of radiolucent foreign bodies (FBs), like those made of wood, remains elusive, with the risk of delayed diagnosis. The patient was a four-year-old girl who presented with chronic foot pain, limping, and a localized dorsal mass six months following a plantar puncture injury. Initial management at another facility consisted of exploration for a suspected FB and abscess, guided by an ultrasound report and an attempt at removal guided by ultrasound; however, her sym
