Hierarchical Targeting of TREM2+ Myeloid Cells via Acid-Triggered OMVs Reprogram Immunosuppression and Suppress Osteolysis in Bone-Metastatic TNBC.

Triple-negative breast cancer (TNBC) bone metastasis is characterized by an immunosuppressive microenvironment dominated by triggering receptor expressed on myeloid cells 2 (TREM2) + myeloid cells, which promote osteoclastogenesis, as well as T cell exclusion. To disrupt myeloid-driven immunosuppression, we developed a hierarchical targeting nanoplatform (siTREM2@ETP-PEOz-OMVs) that exploits pH-responsive outer membrane vesicle (OMV) exposure for selective myeloid cell uptake in acidic TNBC bone metastatic microenvironment, enabling TREM2 silencing-driven macrophage repolarization and osteocla

Ferroptosis in orthopedic implant integration after oncologic surgery.

Primary bone cancer treatment often requires large bone resections followed by reconstruction using orthopedic implants, which are usually made of titanium (Ti). In oncologic patients, the bone-implant interface (BII) microenvironment is compromised by adjuvant therapies, such as chemotherapy and radiotherapy, which impair tissue repair and disrupt redox homeostasis. Despite high biocompatibility of Ti implants, their long-term success is challenged by the continuous release of degradation products Ti4+ ions and titanium dioxide (TiO2) nanoparticles, which promote chronic inflammatory and oxid

6-Hydroxyflavone Attenuates Inflammatory Osteolysis by Inhibiting Osteoclast Activation via the Nrf2 and Calcium Signaling Pathways.

Inflammatory osteolysis (IO) is a pathological bone disorder characterized by excessive osteoclast activation and bone resorption driven by inflammatory mediators and oxidative stress. 6-Hydroxyflavone (6-HOF), a natural flavonoid with potent antioxidant and anti-inflammatory properties, has not been fully investigated in the context of IO. In this study, we explored the therapeutic effects and underlying mechanisms of 6-HOF in LPS-induced osteoclastogenesis and inflammatory bone loss. Network pharmacology analysis predicted that 6-HOF primarily targets oxidative stress and calcium signaling p

Dedifferentiated liposarcoma and 1,25 dihydroxyvitamin D-induced paraneoplastic hypercalcaemia.

Hypercalcaemia of malignancy (HM) can result from excess parathyroid hormone-related peptide (PTHrP), osteolysis from bone metastasis or overproduction of 1,25 dihydroxyvitamin D (1,25[OH]2D) by the enzyme 1-α hydroxylase. HM among sarcomas is rare, with the most common mechanism being overproduction of PTHrP. There are only three published cases of sarcoma-related HM caused by overproduction of 1,25[OH]2D. We herein report a patient who presented with abdominal pain and generalised weakness and was diagnosed with a large multifocal retroperitoneal dedifferentiated liposarcoma. His bloodwork s

STAT1 itaconation prevents macrophage cytolistic mtDNA -induced inflammation in wear particle-induced aseptic loosening.

Prosthetic wear particle-driven macrophage inflammation severely limits the long-term efficacy of total joint replacements by causing aseptic loosening. However, the specific mechanisms by which wear particles induce macrophage inflammation remain incompletely elucidated. Itaconic acid produced by the Krebs cycle is markedly up-regulated in TiPs-stimulated macrophages, which may modulate mitochondrial metabolism through protein itaconation or competitive inhibition. Here, using 4-OI, we demonstrate that itaconate functions as an endogenous metabolic regulator that suppresses SDH activity, ther

Friedelin suppresses osteoclastogenesis via sequential dual-targeting of p105 processing and STAT3-mediated metabolic reprogramming.

BACKGROUND: Pathological osteolysis, driven by excessive osteoclast activity, is a hallmark of Rheumatoid Arthritis (RA) and osteoporosis. Current therapies target inflammation but fail to prevent bone erosion, highlighting the demand for dual-action agents. Although Tripterygium wilfordii has shown therapeutic efficacy in RA, its clinical application is restricted by toxicity. Consequently, identifying novel bioactive monomers with favorable safety and deciphering their targets remains a critical need. METHODS: Through network pharmacology, we identified Friedelin as a core bioactive monomer

Periprosthetic Fractures of the Acetabulum: A Rare, but Serious Complication of Total Hip Arthroplasty.

BACKGROUND: Periprosthetic acetabular fractures in the postoperative period are a relatively rare complication of total hip arthroplasty (THA). There are limited data describing this patient population, their treatment, and outcomes. METHODS: A retrospective review was conducted using a prospectively gathered database of 3,595 revision THA procedures from 1990 to 2024. Of these, 54 periprosthetic acetabular fractures were identified. The review was performed to determine patient demographics, implant survivorship, mortality, and functional outcomes. RESULTS: A total of 54 periprosthetic frac

Conventional Titanium Acetabular Cups vs. Trabecular Titanium Acetabular Cups in Primary Total Hip Arthroplasty: Ten-year Follow-up Clinical and Radiological Results.

OBJECTIVES: Evidence supporting the use of trabecular titanium acetabular cups in primary total hip arthroplasty (THA) is generally favorable, especially in the short and mid-term. However, certain considerations remain, and long-term data are still unavailable. METHODS: A total of 53 THAs in 47 patients performed at our institution between 2011 and 2015 were included and divided into two groups to compare clinical and radiological outcomes. Group A included 15 patients with conventional titanium cups, while Group B comprised 38 patients who received trabecular titanium cups. RESULTS: There

Nanoparticle-Loaded Injectable Hydrogel Alleviates Titanium Particle-Induced Osteolysis by Disrupting GATA6/DDX3X-Mediated Macrophage Inflammation.

Aseptic loosening (AL) represents the primary cause of joint arthroplasty failure, which is predominantly triggered by chronic inflammatory reactions to prosthetic wear particles, with macrophages serving as the central effector cells. Accumulating evidence indicates that the crosstalk between endoplasmic reticulum stress and mitochondrial stress exacerbates macrophage-mediated inflammation; however, the core molecular regulators orchestrating this pathological cascade remain elusive. Herein, we investigated the functional role of the GATA6/DDX3X (DEAD-box helicase 3 X-linked) axis in titanium

Dexamethasone-loaded nanoparticles promote angiogenesis and pro-regenerative macrophages polarization in the presence of titanium debris: a rabbit critical defect model.

OBJECTIVE: Surgical treatment of peri‑implantitis sometimes encompasses bone regeneration procedures performed in junction with implantoplasty. Hence, the aim of this study was to evaluate if titanium debris (Tid) produced by abrasion of dental implants may alter the cellular response, the osteogenic capacity and vascularization in a critical-size calvarial bone defect model in rabbit. It was also assessed if dexamethasone-doped polymeric nanoparticles, combined with calcium phosphate, may help to overcome these impairments. METHODS: Four critical bone defects were performed on six New Zealan

Stress shielding in radial head arthroplasty: a comparative analysis of partially vs. fully grit-blasted press-fit stems.

BACKGROUND: Stress shielding is a known cause of periprosthetic bone loss in joint arthroplasty, but its clinical implications in radial head arthroplasty (RHA) remain unclear. Design modifications, such as partially grit-blasted stems, aim to mitigate proximal radial bone resorption while maintaining stability. We hypothesized that partially grit-blasted stems will have less stress shielding than fully grit-blasted stems. METHODS: A retrospective review of 28 patients who underwent press-fit RHA with either partially grit-blasted (n = 21) or fully grit-blasted (n = 7) short-stem implants bet

[Translated article] Survival of tantalum-coated stems in primary hip arthroplasty during long term follow-up: A retrospective study.

BACKGROUND AND OBJECTIVE: Porous tantalum offers excellent osteointegration and bone growth over the implant, in addition to a physiological load transfer with reduced bone resorption. However, there is limited literature on the survival of tantalum-coated femoral stems. The objective of this study is to evaluate the outcomes in terms of survival of a tantalum-coated stem in the context of primary total hip arthroplasty (THA). MATERIALS AND METHODS: A retrospective study was conducted on all patients who underwent primary THA between 2006 and 2009 at a single center, where a tantalum-coated f

Spautin-1 inhibits osteoclast differentiation and attenuates bone loss by blocking USP10/JIP1-mediated JNK pathway.

Osteoclast hyperactivity is a key factor in the pathogenesis of several skeletal disorders, including inflammatory bone loss, periprosthetic osteolysis, and rheumatoid arthritis. The ubiquitin-proteasome system (UPS) is pivotal in bone homeostasis and disease pathogenesis, modulating critical osteogenic and osteolytic signaling cascades. Our study identified Spautin-1, an inhibitor of ubiquitin-specific protease 10 (USP10), with the capacity to suppress osteoclastogenesis and the expression of genes associated with osteoclast activity, triggered by the nuclear factor-κB (NF-κB) ligand (RANKL)

MARS MRI Abnormalities in Asymptomatic Revision Total Hip Arthroplasty Patients With a Titanium Sleeved Ceramic Head.

BACKGROUND: Titanium sleeves are the gold standard to protect ceramic heads placed on a previously used trunnion. However, this introduces another articulation that may lead to local soft tissue reaction. The purpose of this study is to determine the prevalence of abnormalities on metal artifact reduction sequence magnetic resonance imaging (MARS MRI) in asymptomatic patients with a titanium sleeve in revision total hip arthroplasty (rTHA). METHODS: Sixteen asymptomatic patients (17 hips) with a minimum of 1-year follow-up after rTHA with titanium sleeves in a ceramic head were enrolled in th
