A rare case report: MRSA osteomyelitis after CRPP for pediatric distal both-bone forearm fracture.

BACKGROUND: Distal forearm fractures are common pediatric injuries. While Closed Reduction and Percutaneous Pinning (CRPP) is a standard treatment with excellent outcomes, subsequent poliomyelitis is a less commonly reported but serious complication. This case highlights the diagnostic and therapeutic challenges of managing Methicillin-resistant Staphylococcus aureus (MRSA) fracture-related infection (FRI), particularly in the presence of subtle initial signs. CASE PRESENTATION: A 9-year-old male underwent CRPP for a right distal forearm fracture (Fig. 1). Intraoperative radiographs confirmed

Bone biomarkers in post-polio clinic patients.

INTRODUCTION: Osteoporosis is common in post-polio clinic patients, and is reported in 30%to 50% of middle-aged individuals with previous polio. The levels of bone biomarkers (calcium regulating hormones, bone metabolism markers, and bone turnover markers), and the response of bone turnover markers to bisphosphonates is unknown in post-polio patients with osteoporosis. OBJECTIVES: 1) To describe serum levels of bone biomarkers in post-polio clinic patients with osteoporosis and compare these levels to those in controls with osteoporosis without neurological disease. 2) To examine the change i

Dual Mobility Acetabular Cup Utilization in Total Hip Arthroplasty: Mitigating Instability Risks.

BACKGROUND: With the increase in life expectancy and the rising number of total hip arthroplasty (THA) cases, the rate of complications is also expected to increase. One of the most challenging complications is dislocation, which is the leading cause of revision surgery within the first year after THA. This study aimed to assess hip instability rates in high-risk patients who underwent dual mobility cup (DMC) implantation. METHODS: This retrospective study reviewed all patients who underwent THA with DMC at Rasul-e-Akram Hospital between 2014 and 2021. DMC was used in high-risk patients for d

Vaccine hesitancy and bone health: Musculoskeletal sequelae of vaccine-preventable diseases.

Vaccine hesitancy threatens global health by diminishing herd immunity and permitting the return of vaccine-preventable diseases (VPDs) including poliomyelitis, measles and coronavirus disease 2019 (COVID-19). These infections carry short-term and long-term musculoskeletal consequences, such as paralysis, limb deformities, septic or reactive arthritis, osteomyelitis and chronic pain, that place an ever increasing burden on orthopaedic services. This narrative review synthesizes clinical and epidemiological evidence linking sub-optimal immunization to orthopaedic morbidity caused by major VPDs.

Acute Flaccid Myelitis: Mid-Term Clinical Course of Knee Extension Paralysis and Outcomes of Nerve Transfer.

BACKGROUND: Acute flaccid myelitis (AFM) is a rare debilitating poliomyelitis-like illness characterized by the sudden onset of flaccid palsy in the extremities. The purpose of this study was to report the mid-term clinical course of knee extension in AFM and the effect of contralateral obturator nerve-to-femoral nerve transfer (CONFNT) for restoration of knee extension in AFM. METHODS: Twenty-six patients with lower extremity palsy due to AFM were referred to our clinic for possible surgical reconstruction. Their median age was 4.0 years, and the first evaluation of the palsy was done at a m

Severe immune thrombocytopenia following diphtheria, tetanus, pertussis and polio vaccination in a 36-year-old Caucasian woman: a case report.

BACKGROUND: Immune thrombocytopenia (ITP) is a rare autoimmune disorder characterized by low platelet counts and increased bleeding risk. The disease may be induced by other disorders, including malignancies, autoimmune diseases, infectious agents or drugs. However, ITP has also been described following vaccinations, such as the measles-mumps-rubella vaccination. In rare cases, ITP may occur in children who received a DTaP-IP (diphtheria, tetanus, acellular pertussis vaccine and inactivated poliovirus) vaccine. Hereinafter, we report the first well-documented cases of ITP in an adult patient i

Long-Term Survival on Noninvasive Ventilation in Adults With Thoracic Scoliosis.

BACKGROUND: Subjects with thoracic scoliosis were an important group in early studies of noninvasive ventilation (NIV). The aim of this study was to describe current rates of initiation of NIV and survival after initiation in this population. METHODS: This study included patients identified as having thoracic scoliosis and established between 1993 and 2018 on home NIV. Patients with scoliosis secondary to neuromuscular disease (other than poliomyelitis) were excluded. Survival rates were calculated for various time intervals up to 25 y. RESULTS: A total of 53 subjects with thoracic scoliosis

Bone Mineral Density Among Individuals With Residual Lower Limb Weakness After Polio.

BACKGROUND: Literature indicates that individuals with long-term residual lower extremity (LE) weakness after polio have decreased bone mineral density (BMD) related to muscle weakness. Where weakness is asymmetrical, bone densitometry measured only on the stronger LE may misclassify BMD. OBJECTIVE: To determine (1) whether femoral neck BMD differed from side to side in individuals with asymmetrical LE muscle weakness, and (2) the proportion of individuals at risk for underdiagnosis of low bone density or osteoporosis given unilateral assessment of the femoral neck. DESIGN: Retrospective stu

Immune Priming and Long-term Persistence of Memory B Cells After Inactivated Poliovirus Vaccine in Macaque Models: Support for at least 2 Doses.

BACKGROUND: As a risk-mitigation strategy to minimize paralytic polio following withdrawal of Sabin type 2 from the oral poliovirus vaccine in April 2016, a single full dose or 2 fractional doses of inactivated poliovirus vaccine (IPV) are recommended. However, limited knowledge exists on long-term persistence of immune memory following 1- or 2-dose IPV schedules. METHODS: We examined induction and maintenance of immune memory following single- vs 2-dose IPV schedules, either full-dose intramuscular or fractional-dose intradermal, in rhesus macaques. Humoral responses, bone marrow-homing anti

Combined tibial lengthening and ankle arthrodesis for patients with certain type of sequelae of poliomyelitis.

PURPOSE: Following far advancement of modern medicine and technology, functional disability in a certain type of sequelae of poliomyelitis may be effectively improved. METHODS: Eight consecutive adult patients with unilateral sequelae of poliomyelitis were treated. These patients had shortened lower extremity of an average of 4.8 cm (range, 4.0-5.5 cm) in the lesion side. Muscle power of the ipsilateral knee was nearly intact (grade 4 or 5) but the ankle extension was completely flaccid. The tibia was osteotomized and lengthened with external fixation. Consequently, all external fixators were

Transjugular balloon mitral valvotomy in a patient with severe kyphoscoliosis.

Balloon mitral valvotomy (BMV) performed by the conventional transfemoral approach can be difficult or even impossible in the presence of structural impediments such as severe kyphoscoliosis, gross cardiac anatomic distortion and inferior vena caval anomalies. A 25-year-old woman with severe thoracolumbar kyphoscoliosis due to poliomyelitis presented with symptomatic rheumatic mitral valve stenosis. After the failure of transfemoral BMV, the procedure was attempted from the right jugular access, using a modified septal puncture technique. The left atrium was entered from the jugular access and

Bone erosion and subacromial bursitis caused by diphtheria-tetanus-poliomyelitis vaccine.

Revaxis(®) is a vaccine against diphtheria, tetanus and poliomyelitis (dT-IPV). This vaccine should not be administered by the intradermal or intravenous route. Poor injection techniques and related consequences are rare. We report a case of bursitis associated with reactive glenohumeral effusion complicated by bone erosion occurring after injection of the dT-IPV vaccine. A 26 year old patient was admitted for painful left shoulder causing functional impairment. Control magnetic resonance imaging showed bone oedema on the upper outer part of the humeral head, with a slight cortical irregularit

Foot and ankle function after tibial overlengthening.

Lengthening the tibia more than 25% of its original length can be indicated for proximal femoral deficiency, poliomyelitis, or femoral infected nonunion. Such lengthening of the tibia can adversely affect the ankle or foot shape and function. The present study aimed to assess the effect of tibial lengthening of more than 25% of its original length on the foot and ankle shape and function compared with the preoperative condition. This was a retrospective study of 13 children with severe proximal focal femoral deficiency, Aitken classification type D, who had undergone limb lengthening from June
