Rotation osteotomy of the tibia after poliomyelitis. A review of 51 patients.

After severe poliomyelitis, which is still relatively common in some developing countries, lateral rotation deformity of the tibia may occur. We have reviewed 51 patients treated by O'Donoghue's rotation osteotomy of the tibia. An average lateral rotation deformity of 57 degrees was fully corrected in all the patients, and in 38 of them the graft obtained during the osteotomy was used for a simultaneous Grice-Green subtalar arthrodesis in one or both feet. All the osteotomies united in an average of 11 weeks, some with relatively minor and unintentional posterior angulation. There was no poste

[Long-term follow-up of lung function and standing height following scoliosis surgery in children and adolescents].

Previous investigations have reported conflicting results on the development of lung function and body height after operative correction of scoliosis at young age. Partly the number of patients or the duration of observation were relatively small. 120 patients were followed for at least 10 years (10-14, average 12 years) after surgical correction of scoliosis at the age of 18.2 +/- 6.8 years. Standing height and vital capacity were measured 1, 2, 5 and 10 years after operation. Height was expressed as percentile for age and vital capacity as % expected for actual height. The primary diagnoses

Neuromuscular symptoms in patients with previous poliomyelitis: a New Zealand study.

Eighteen patients with old poliomyelitis were assessed in order to determine the incidence and severity of late complications. Sixty-one percent complained of new weakness, 83% fatigue and 17% muscle pain. After assessment 33% (six patients) were judged to have significant new weakness and muscle fatigue that could not be explained by other causes, and this group may have postpoliomyelitis progressive muscular atrophy or postpolio syndrome. Onset of symptoms was typically about 30 years after the acute illness; new weakness was relatively mild and progression was slow over many years. Clinical

Calf enlargement associated with neurologic disease: two uncommon cases.

Muscle enlargement and hypertrophy are rare findings in neurogenic lesions. The two in combination have been reported in cases of peripheral nerve lesions, polyneuropathy, and poliomyelitis. True and pseudo muscle hypertrophy are the two possible etiologies, whereas infiltration, stretch, or exercise of the muscle are the causative factors. We report two cases of unilateral calf enlargement, one occurring after surgery for S1 radiculopathy with associated cramping, and the other after poliomyelitis.

Failure of fixation after segmental spinal instrumentation without arthrodesis in the management of paralytic scoliosis.

Between April and October 1981, nineteen children who were between five and twelve years old (median age, seven years), and who had severe paralytic scoliosis secondary to poliomyelitis, were treated by segmental spinal instrumentation without arthrodesis. Three patients could not be followed, and the cases of the remaining sixteen patients were reviewed in April 1984. In fifteen patients, the implant system had failed to control the deformity. In six patients, the rods had fractured at the apex of the original deformity; in five, longitudinal shift of the rods had allowed recurrence of the de

The late sequelae of poliomyelitis.

Progressive functional deterioration following poliomyelitis has been reported after a prolonged period of stability. We present follow up data on 209 patients; the period from the original illness to December 1985 or death was between two and 73 years (mean 33.9). One hundred and sixty-three (78 per cent) patients developed late functional deterioration. This was due to purely respiratory factors in 99 cases, new neurological signs in 20 cases, and orthopaedic problems in 17 cases; 31 patients deteriorated due to a combination of factors. The commonest cause of respiratory deterioration was t

Longitudinal growth of the femur and tibia after diaphyseal lengthening.

Growth of the femur and tibia after lengthening of their diaphyses, which had been performed several years before skeletal maturity, was assessed in eighteen patients by serial orthoroentgenograms. The rate of growth of the lengthened femora, which had been congenitally short, increased after each of seven lengthening procedures, from an average preoperative rate of 82 per cent of the normal side to 90 per cent of the normal side. The amount of surgical lengthening was similar for the femora and the tibiae, averaging 18 per cent of the preoperative length of the bone in the femora and 20 per c

The Chiari osteotomy. A review of 58 cases.

Fifty-eight Chiari osteotomies performed in 51 patients were reviewed retrospectively. The indications for surgery were painful subluxation, instability, or poor acetabular coverage. The average age at surgery was 16 years (range, 7-45), and the average follow-up period was 40 months. The majority of the patients had congenital hip dislocation, but also included were patients with cerebral palsy, Legg-Calvé-Perthes disease, polio, infection, coxa vara, and epiphyseal dysplasia. Supplemental bone grafts were applied as needed. The functional results were unrelated to age, with 26 excellent, 28

[An unknown after surgical lengthening of the femur in children: spontaneous postoperative parasitic lengthening].

The authors have studied overgrowth or diminished growth seen in 54 femora lengthened surgically by a one-stage procedure between 1963 and 1979. Thirty six cases were of poliomyelitis. An overgrowth was regularly seen of the order of 2.5 p. 100- that is 11 mm on average. This stimulation of growth by lengthening was not related to age. It was more marked where there was severe involvement. In contrast, in 13 cases of congenital femoral abnormalities and 5 cases of miscellaneous diseases (Ollier's disease, neurofibromatosis, sequelae of septic arthritis) overgrowth or diminished growth have bee

Paralytic deformities of the lower limb.

Deformity may occur in any paralytic condition in childhood. It develops after birth in poliomyelitis, cerebral palsy, hereditary neuropathies and muscular dystrophies or before birth in spina bifida, spinal agenesis and arthrogryposis. Paralytic deformities arise primarily in muscles and their tendons, and are due to diminished musculo-tendinous growth associated with unbalanced muscle action. Their inexorable progress is only controlled effectively by surgical correction of tendon length and muscle imbalance. The paralysis needs to be assessed according to the specific type of paralytic lesi

Scoliosis and trunk muscles.

This study examines the role of muscular deficiencies in the origin and development of paralytic scoliosis. We studied spine films of 108 scoliotic curves of poliomyelitic children and results of muscle tests made during the growth phase. We investigated the relationship between spinal and abdominal muscle test values, determined 2 years after the child had experienced the acute phase of anterior polioimyelitis, and the convexity, evolution rate, effects of gravity, and reducibility of the scoliotic deformity. Although paralysis certainly is responsible for triggering scoliosis, it plays no ro

Genetic causes of chronic musculoskeletal disease in childhood are common.

We surveyed admissions to a pediatric orthopedics hospital for calender years 1957 (polio era) and 1982 (post-polio era). The relative importance of genetic causes of musculoskeletal disease increased after poliomyelitis immunization became universal. Admissions for musculoskeletal disease with a genetic cause (chromosomal, Mendelian, multifactorial, and familial) accounted for 45% of the total in 1957 and 57% in 1982 (p less than 0.02); non-genetic causes were 44% in 1957 and 28% in 1982 (p less than 0.001). The high probability of recurrent disease in the families of these patients implies t

Changes in respiratory function in disorders of the thoracic cage. With special reference to the ventilatory mechanism and the regulation in scoliosis.

After describing the clinical and functional effects of pathological changes in the thoracic cage ("mechanical syndrome" and "neuromuscular syndrome"), the authors discuss the disorders of respiratory function observed in a group of patients with poliomyelitic scoliosis. The aim was to establish the type and nature of these changes with a view to clarifying certain pathogenetic aspects. Two factors suggest that the altered geometry of the thoracic cage in scoliosis is responsible for the mechanical inefficiency of the thoracopulmonary apparatus, namely: 1. changes in respiratory function, sinc

Twenty-year review of tibial lengthening for poliomyelitis.

Twenty-four patients treated for tibial shortening secondary to poliomyelitis were reviewed at least 20 years after operation. All subjects were active and only one expressed doubt about the value of tibial lengthening, considering the period of hospitalisation to have been a significant drawback. Approximately five centimetres were added to the length of the treated tibiae. Only one of the nine patients with lengthening of over five centimetres was able to walk at a rate of more than five kilometres per hour, compared with seven of the remaining 15 whose tibiae had been lengthened five centim

Acute respiratory failure in scoliosis or kyphosis: prolonged survival and treatment.

Acute respiratory failure (ARF) in adults with severe thoracic spinal deformity is said to be a preterminal event with a median survival of one year. Twenty patients with ARF (mean +/- S.D., arterial oxygen tension [PaO2] 35 +/- 7 mm Hg, arterial carbon dioxide tension [PaCO2] 63 +/- 9 mm Hg, pH 7.34 +/- 0.08) due to severe scoliosis or kyphosis (spinal curve 113 +/- 28 degrees ) were seen between 1965 and 1980. All 20 survived the initial episode of ARF and during the follow-up period (median, six years) experienced 2.4 additional episodes of ARF. The age at presentation was 52 years (mean, r

Perthes' disease after poliomyelitis. Recognition and management of aseptic capital femoral necrosis.

Aseptic necrosis of the capital femoral epiphysis occurred in a polio-wasted leg and was undiagnosed for over one year. The diagnosis was overlooked because of a pre-existent limp. Exacerbation of an existent limp or persistent knee or leg pain should always prompt a thorough search to exclude chronic and acute causes of hip disease in young children. Aggressive and individualized care is indicated for Perthes' disease and may include non-surgical means such as recumbency and bracing in the very young or traction with abduction and various bracings in the older children. Surgical approaches ma

[Antimutagenic effectiveness of pyrimidine derivatives in lesions of the bone marrow cell chromosome apparatus in mice infected with measles or poliomyelitis viruses].

It is determined that the number of marrow cells with cytogenetic disturbances grows in mice infected with measles or polymyelitis viruses. The level of marrow cells with disturbances is much lower in the mice fed with methyluracilum or pentoxylum before infection. The level of deteriorated cells also decreases when the mice were fed with the mentioned drug after infection. In an analogous experiment with polymyelitis viruses the antimutagenic effect was not determined.

[Epiphysiodesis in the treatment of discrepancies in length of the lower extremities (author's transl)].

Thirteen children treated only by epiphysiodesis for inequality in the length of lower limbs were reviewed after maturity. The intention was to estimate the value of the predictions of growth used in this work and the effectiveness of the operation. The result of this procedure depends upon the accuracy in predicting the further growth in order to determine at what age the operation should be performed. The imperfect results of epiphysiodesis are attributable to prediction errors and to operation delay in some cases. Prediction can be made using only roentgenographic measurement of the femur a

Early arthrodesis for a flail shoulder in young children.

In seven children, five to nine years old, with a flail shoulder and a functional hand and elbow, arthrodesis of the shoulder in 80 to 90 degrees of abduction was performed by denuding the humeral head and glenoid of cartilage and fixing the humerus to the glenoid with Steinmann pins first inserted in the humerus in a proximal-distal direction and then driven in the reverse direction into the glenoid. Postoperatively each limb was immobilized in a spica cast, the pins were removed after a month, and the cast was removed after ten to twelve weeks when union was evident. All seven fusions united
