Uncovering the hidden costs of free tuberculosis care: economic narratives of informal treatment supporters in rural Ghana.

BACKGROUND: Tuberculosis (TB) control strategies in Ghana emphasize free diagnosis and treatment, often overlooking the economic burden borne by informal treatment supporters typically family members, friends or community members who provide day-to-day care remains largely invisible in health policy discourse. These supporters play a crucial, yet underrecognized role in TB management, especially in rural areas where health system limitations intensify reliance on unpaid informal treatment supporters. OBJECTIVES: This study explores the direct and indirect costs incurred by informal TB treatme

Combined posterior-anterior surgery with autologous bone grafting for multi-segmental spinal tuberculosis.

BACKGROUND: Multi-segmental spinal tuberculosis is a severe infectious disease of the spine affecting multiple vertebral bodies, often leading to spinal instability, progressive kyphosis, and irreversible neurological impairment. Its diagnosis and treatment are notably challenging due to the extensive distribution of lesions and severe bone destruction. METHODS: A retrospective analysis was conducted on 95 patients with multi-segmental spinal tuberculosis involving four or more vertebrae, who were treated at the Department of Orthopedics, General Hospital of Ningxia Medical University, from J

Cold war of strains: the 'Bulgarian' BCG vaccine between Paris, Copenhagen, and Moscow (1940s-1950s).

This article deals with the tuberculosis policy in Communist Bulgaria from the 1940s to the end of the 1950s. The focus is on the BCG vaccination as the major preventive tool. The article's reconstruction of decision-making draws on evidence from archive records produced by the Bulgarian Ministry of Health. The main question guiding the research is how past Bulgarian experiences on the one hand, and international traditions, on the other, influence medical opinion and state policy towards tuberculosis and patients with tuberculosis. How did the Cold War context shape BCG vaccination policy? Th

Interleukin-37 in respiratory diseases: molecular mechanisms and immune modulation.

Interleukin-37 (IL-37) is a potent anti-inflammatory cytokine that plays a crucial protective role in cancer, autoimmune diseases, and inflammatory diseases though its unique dual intracellular and extracellular action pathways. This review highlights the significance of IL-37 in common respiratory diseases. Specifically, IL-37 can alleviate asthma by inhibiting Th2/Th17 immune responses, inhibiting the release of epithelial-derived alarmins (TSLP and IL-33), and attenuating airway remodeling. In pulmonary infections, IL-37 modulates host responses by mitigating virus-induced hyperinflammation

Triple-synergy bactericidal action: a mannose-decorated bioinspired coating on silver-mesoporous silica for eradicating multidrug-resistant Mycobacterium tuberculosis.

Tuberculosis, the world's leading cause of death from a single infectious agent in 2023, is exacerbated by the rising prevalence of multidrug-resistant (MDR) Mycobacterium tuberculosis, underscoring an urgent need for novel therapeutic agents. In this study, we developed a green in situ synthesis strategy for silver nanoparticles (Ag NPs) utilizing polydopamine (PDA) as a biomimetic reducing and coating agent on mesoporous silica (SBA-15). The material was further co-coated with the antimicrobial polymer epsilon-poly-l-lysine (EPL) and functionalized with mannose via a Schiff-base reaction. Th

Socio-environmental determinants of tuberculosis in South Africa.

Correlation analysis and trend analysis were employed to study the interaction of tuberculosis (TB) incidence with other socio environmental variables in South Africa from 2000 to 2020. The data were obtained from the United Nations database and World Development Indicators. Malnutrition, air pollution (PM2.5), population density, and urbanization were vital variables under study. Findings showed population density and malnutrition to be strongly and positively correlated with case detection of TB, and both correlations were statistically significant. Urbanization was weakly negatively correla

Clinical evaluation of the diagnostic performances and treatment monitoring of the new PATHFAST TB LAM Ag assay in sputum specimens of patients with tuberculosis or with nontuberculous mycobacteria pulmonary disease.

UNLABELLED: Diagnosing pulmonary tuberculosis (TB) and monitoring treatment remain challenging. New tools such as the PATHFAST TB LAM assay (PHC Corporation, formerly LSI Medience Corporation, Tokyo, Japan; distributed by BioSynex, France; PF-LAM) may complement microscopy, culture, and NAAT. PF-LAM is an immunoassay quantifying lipoarabinomannan (LAM), a mycobacterial cell wall component, in sputum within 1 hour using an automated chemiluminescent reader. Diagnostic performance was first assessed on 100 sputum samples: 40 culture-positive for Mycobacterium tuberculosis complex (MTBC), 40 cult

The osteoclast blind spot and other unresolved mechanisms in the exosomal theory of tuberculosis-induced osteoporosis.

The recent study by Zhao et al. ( Inflamm Res. 2025) proposed a novel paradigm in which exosomal miR-125b-5p from Mycobacterium tuberculosis-infected macrophages impairs osteoblast function by targeting IGF2, thereby linking pulmonary infection to systemic osteoporosis. While this work provides a valuable mechanistic insight, our letter offers a critical appraisal to contextualize its findings and highlight pivotal unanswered questions. We posit that the proposed pathway, though compelling, requires further validation to establish direct causality in vivo, independent of the well-established r

Bilateral Parotid Tuberculous Lymphadenitis Mimicking Warthin Tumor in an Elderly Male: A Case Report.

Tuberculosis involving the parotid lymph nodes is rare and can clinically and radiologically mimic benign neoplasms such as Warthin tumor, particularly when bilateral. We present the case of a 72-year-old male with bilateral parotid swellings, initially suspected to be Warthin tumors based on magnetic resonance imaging (MRI) findings showing well-defined, cystic lesions with focal solid components and central necrosis. Histopathological examination following ultrasound-guided biopsy revealed caseating granulomas consistent with tuberculous lymphadenitis. This case highlights the importance of

Cell envelope maintenance by PhoP is essential for Mycobacterium tuberculosis methylglyoxal resistance.

During Mycobacterium tuberculosis infections bacteria are engulfed by macrophages, a main line of defense against invading pathogens. Upon activation, macrophages increase glycolysis, producing the antibacterial aldehyde methylglyoxal. To test whether bacterial methylglyoxal resistance is required for robust infections, we sought to identify M. tuberculosis defense mechanisms against methylglyoxal. We identified phoP mutants were among the most highly sensitive strains to methylglyoxal in vitro. phoP mutants are highly attenuated in mice but a phoP mutant was even more attenuated in mice that

Nationwide longitudinal evaluation of a machine learning approach for enhanced interpretation of Xpert MTB/RIF ultra rifampicin-resistance results in low bacterial load tuberculosis specimens.

BACKGROUND: The World Health Organization (WHO) has identified tuberculosis (TB) as the leading cause of death from a single infectious agent. False-positive rifampicin (RIF) resistance results from the Xpert MTB/RIF Ultra assay are common in TB patients with low bacterial loads, especially among HIV-coinfected individuals. Hence, to distinguish genuine RIF resistance from false-positive results, this study developed and validated an artificial intelligence clinical decision support system (AI-CDSS). METHODS: Between January 2021 and March 2025, Taiwan's national TB reference laboratory recei

NucS Thr74Ile confers mutator phenotype in a globally disseminated clinical Mycobacterium abscessus subsp. abscessus clone.

OBJECTIVE: The emergence of mutations conferring resistance to first-line drugs, macrolides and amikacin, is a major cause of treatment failure in Mycobacterium abscessus complex (MABC) infections; however, mutator strains predisposed to developing drug-resistance mutations remain uncharacterized. NucS, a key DNA mismatch repair protein, maintains mycobacterial genomic stability. This study investigated whether naturally occurring NucS polymorphisms contribute to mutator phenotypes in clinical MABC strains. METHODS: The whole genome data of clinical MABC isolates obtained from public database

Bedaquiline-induced eosinophilic pneumonitis in treatment-refractory mycobacterium avium complex (MAC) lung disease.

We report the first known case of eosinophilic pneumonitis induced by bedaquiline, a drug increasingly used off-label for non-tuberculous mycobacterial infections. While bedaquiline is generally well tolerated, its potential to cause pulmonary hypersensitivity reactions has not been previously reported. This case highlights the need for heightened clinical awareness of novel adverse effects as bedaquiline use expands beyond its original indication for multidrug-resistant tuberculosis. Clinicians should consider eosinophilic pneumonitis in patients developing new respiratory symptoms and periph

ddPCR-based profiling of plasma EV-miRNA identifies a noninvasive biomarker panel for tuberculosis.

BACKGROUND: Tuberculosis (TB) is a top infectious disease killer; underdiagnosis fuels its spread. Thus, finding perspective non-invasive biomarkers for early tuberculosis (TB) is very essential. Extracellular vesicles (EVs) microRNAs (miRNAs) are promising non-invasive biomarkers for tuberculosis (TB) detection. METHODS: To investigate the feasibility of early diagnosis of TB, we examined plasma-derived EV-miRNAs obtained from patients with active TB (n = 25), asymptomatic tuberculosis (aTB) (n = 25), and latent tuberculosis infection (LTBI) (n = 25). Initially, EVs were isolated from plasma

Rv1899c, an HDAC1-ZBTB25-Interacting Protein of Mycobacterium tuberculosis, Promotes Stress Resistance and Immune Evasion in Infected Macrophages.

Rv1899c, a previously identified HDAC1-ZBTB25-interacting protein of Mycobacterium tuberculosis, plays a crucial role in bacterial adaptation and immune modulation. Recombinant M. smegmatis-expressing Rv1899c (MS_ Rv1899c) showed enhanced survival under acidic and oxidative stress compared to vector controls, along with improved early intracellular growth in THP1-derived macrophages. This was accompanied by reduced reactive oxygen species (ROS), diminished cytokines associated with inflammation and downregulation of autophagy proteins ATG5, Beclin, and LC3, which ultimately skewed the immune r

3-Gene-TB-SCORE Accuracy for Tuberculosis Disease Diagnosis Is Not Affected by Immune-Mediated Inflammatory Disease Comorbidity.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a major global health threat. Approximately one-quarter of the world's population has an Mtb-specific immune response and are classified as having tuberculosis infection (TBI), with a lifelong 5-10% risk of developing TB disease. This risk is elevated in individuals with immune-mediated inflammatory diseases (IMID) undergoing immunosuppressive therapies. To evaluate the diagnostic accuracy of the 3-gene TB-SCORE for TB disease in individuals within the TB spectrum, including those with TBI-IMID in a low TB endemic country,
