Integrating whole-genome sequencing and epidemiology to characterise Mycobacterium bovis transmission in Ireland: a proof of concept.

BACKGROUND: In the Republic of Ireland, the herd-level incidence of bovine tuberculosis (bTB), caused by Mycobacterium bovis, reached 6.40% by June 29th 2025, highlighting bTB’s risk to animal health, biosecurity and the economy. The complexity of bTB epidemiology, driven by multiple host species, undetected transmission and incomplete diagnostic sensitivity, makes surveillance and control challenging. Pathogen whole-genome sequencing (WGS) can clarify transmission dynamics but is constrained by the slow, variable mutation rate of M. bovis. This pilot case study integrates WGS with epidemiolog

AI-Powered Chest X-Ray for Diagnosing Pulmonary Tuberculosis in County and Township Health Care Facilities in Yichang: Retrospective, Real-World Study.

BACKGROUND: In resource-limited areas, severe shortages of radiologists contribute to high rates of missed pulmonary tuberculosis (PTB) cases when relying solely on conventional chest X-ray (CXR). Although artificial intelligence-powered computer-aided detection (CAD) has proven effective in PTB diagnosis, its real-world performance remains underexplored. OBJECTIVE: This study aimed to evaluate the real-world diagnostic yield of CAD technology as a triage tool for detecting PTB in primary health care facilities in high-burden areas. METHODS: We conducted a retrospective paired-design diagnos

Potential Limitations of the World Health Organization Criteria for Nonsevere Tuberculosis in Children in a Low Prevalence, High-resource Setting.

BACKGROUND: In 2022, the World Health Organization (WHO) adapted its pediatric tuberculosis disease (TBD) treatment guidelines, recommending a shortened 4-month treatment regimen for nonsevere TB, based on a single study in high TB prevalence, low-resource settings. This study investigated to what extent the recommendations apply to a high-resource setting. METHODS: Retrospective cohort study of patients with TBD <16 years of age treated at the Austrian Reference Centre for Paediatric TB from 2010 to 2023. RESULTS: One hundred fifty-nine patients were included in the final analysis. Based on

Using a consensus machine learning approach to identify host biomarkers associated with treatment response in pulmonary tuberculosis.

The lack of reliable biomarkers to monitor treatment response constitutes a major challenge in tuberculosis (TB) management, often leading to under- or overtreatment with the standard six-month regimen. Here, we integrated amino acid and trace element profiles, along with four machine learning-based feature selection methods, to develop an evaluation model. Additionally, we investigated the dynamic expression patterns of genes associated with the identified features during TB treatment to gain functional insights. The consensus feature selection from the four methodologies identified 34 potent

JSTMapp: A web-based joint spatiotemporal modelling and mapping application for epidemiologists.

Disease mapping models help create disease risk maps, which public health policymakers can use to design disease control and monitoring programmes. These models are now routinely implemented using spatial statistical software packages that use frequentist estimation methods, such as SaTScan and HDSpatialScan, and Bayesian estimation methods, such as the Windows version of Bayesian inference using Gibbs sampling (WinBUGS) and R integrated nested Laplace approximation (INLA). We aimed to develop a user-friendly joint disease spatiotemporal modelling and mapping application (JSTMapp) for epidemio

Human alveolar macrophage function is impaired in tuberculosis contacts with diabetes.

BACKGROUND: Tuberculosis (TB) remains a leading cause of morbidity and mortality worldwide. Individuals with type 2 diabetes (T2D) are at increased risk of developing active TB, tend to present with more severe disease and experience adverse treatment outcomes compared to individuals without T2D co-morbidity. However, the underlying mechanisms responsible for increased susceptibility of patients with T2D to TB remain poorly understood. METHODS: We performed bronchoscopies and collected bronchoalveolar lavage fluid (BAL) from TB contacts with and without T2D in South Africa and obtained paired

Advancements in photodynamic therapy for tuberculosis treatment.

The emergence of drug-resistant strains of Mycobacterium tuberculosis (Mtb), coupled with lengthy treatment cycles and adverse side effects of traditional therapies, poses a significant global challenge in the form of tuberculosis (TB). Photodynamic therapy (PDT) generates reactive oxygen species that target specific tissues or cells via light irradiation at specific wavelengths. It directly disrupts critical structures and components of Mtb to exert its anti-TB effects. Unlike traditional antibiotics that rely on specific targets for efficacy, PDT prevents the occurrence of drug resistance at

Human pluripotent stem cell-derived skin organoids enabled pathophysiological model of Mycobacterium tuberculosis infection.

Cutaneous tuberculosis (CTB) is an infectious disease highly associated with extracellular matrix remodeling and granuloma-driven fibrosis. Fibroblasts play crucial roles in this fibrotic process, but their specific roles in Mycobacterium tuberculosis (Mtb) skin infections remain unclear due to the lack of proper in vitro models. Here, we demonstrate that skin organoids (SKOs) derived from human induced pluripotent stem cells can model CTB infected by Mtb. Single-cell RNA analyses reveal an increase in fibroblasts, upregulation of genes involved in collagen synthesis, and enhanced collagen deg

A microbial safari: finding evidence of Mycobacterium bovis DNA in soil from the Kruger National Park, South Africa.

UNLABELLED: The primary pathogen causing animal tuberculosis (TB), Mycobacterium bovis, can infect a wide range of mammals, including humans. This pathogen is considered endemic in the Kruger National Park (KNP), South Africa, where it threatens the health and conservation of African wildlife, including endangered species. This mycobacterial pathogen is spread through close contact with an infected host by aerosols or ingestion but may also be transmitted indirectly through the environment. Detecting environmental M. bovis is challenging due to the complexity of the sample matrix and may requi

Neonatal BCG vaccination to prevent respiratory infections in the first 5 years of life: results from the MIS BAIR randomised controlled trial.

BACKGROUND: Respiratory tract infections (RTI) are among the leading causes of hospitalisation and death for children under 5 years. We aimed to determine if neonatal BCG vaccination reduces early life hospitalisations for lower RTI (LRTI) in a low-tuberculosis endemic setting. METHODS: Neonates were randomised 1:1 to receive BCG-Denmark vaccination (BCG group) or no intervention (Control) in a phase 3 randomised controlled trial in Australia (NCT01906853). Hospitalisation for infection was determined by parent-reported questionnaires. The effect of BCG vaccination on the incidence of hospita

Dual activity of low-molecular-weight polymeric β-cyclodextrins against tuberculosis.

Tuberculosis remains the leading cause of death from infectious diseases. While effective therapies are available, their prolonged duration and severe adverse effects compromise patient adherence and promote antimicrobial resistance, highlighting the urgent need for innovative therapeutic approaches. We previously identified polymeric β-cyclodextrin (pβCD) as a promising drug delivery platform, possessing intrinsic antibacterial activity and serving as a carrier for the second-line drug ethionamide. Here, we investigated the biological properties of pβCD of different molecular weights. Strikin
