Sex-dependent and long-term effects of cisplatin on bone mineral density in juvenile mice.

BACKGROUND: Cisplatin is an anti-neoplastic treatment widely used to treat pediatric cancers. Negative effects of cisplatin treatment on bone mass have previously been reported; however, long-term effects of cisplatin on skeletal health and potential sex differences have largely been unexplored. METHODS: Four-week-old BALB/c mice were treated with cisplatin (4 mg/kg, intraperitoneally, twice weekly for four weeks). At 13 weeks of age, we assessed structural changes in the tibiofemoral region using microcomputed tomography. We also quantified, using immunohistochemistry and confocal microscopy

Clinical Outcomes of Bovine Bone Xenografts Following Sequestrectomy in Advanced Medication-Related Osteonecrosis of the Jaw.

Background/Objectives: Bisphosphonates, a class of drugs that are widely used in the treatment of neoplastic diseases, can lead to the development of medication-related osteonecrosis of the jaw (MRONJ). This condition is challenging to manage due to the high incidence of postoperative complications: superinfections, local wound dehiscence, or fractures in pathological bone. The aim of this study is to evaluate the therapeutic role of bovine-derived xenografts in the management of MRONJ. Methods: This retrospective observational study evaluates the clinical outcomes of patients with confirmed s

The 5-factor modified frailty index predicts local control and overall survival following spine stereotactic body radiation therapy.

PURPOSE: Frailty is an established prognostic factor in neurosurgical and oncologic populations. The association between frailty and outcomes following spine stereotactic body radiation therapy (SBRT) has not been previously evaluated using a validated clinical index. METHODS: A prospectively maintained, single-institution database of consecutive patients with spinal metastases and primary bone tumors treated with SBRT from 2007 to 2024 was analyzed. Frailty was assessed at the time of treatment using the 5-factor modified frailty index (mFI-5), with 205 patients stratified into pre-frail (mFI

Uncoupled bone remodeling drives myeloma bone disease in Vk*MYC mouse model of multiple myeloma.

Multiple myeloma (MM), the second-most frequent hematologic malignancy, is caused by the neoplastic expansion of clonal plasma cells. Up to 80% of MM patients develop myeloma bone disease (MMBD), a hallmark of which is the development of osteolytic lesions that can lead to skeletal related events such as fractures, spinal cord compression and bone pain. In the Vk*MYC mouse model of MM, MYC is expressed in germinal center B cells, facilitating the formation of malignant plasma cells and development of major characteristics of MM, including MMBD. However, the extent of skeletal changes and under

Imaging of spinal metastases: modality performance, complications, and differential diagnosis.

Spinal metastases are the most common malignant involvement of the axial skeleton and a major source of cancer-related morbidity. Imaging is central throughout patient management, from initial detection and differential diagnosis of vertebral lesions to staging, treatment planning, and early identification of complications. Magnetic resonance imaging (MRI) is the reference modality owing to its high sensitivity for marrow infiltration and its ability to assess epidural extension and spinal cord compression in a single examination. Computed tomography (CT) remains essential for evaluating corti

Management strategies and outcomes of pediatric unicameral bone cyst: a narrative review.

Unicameral bone cysts (UBCs) are benign, fluid-filled lesions that predominantly occur in the long bones of children and adolescents. Although histologically non-neoplastic, these cysts can weaken bone structure and predispose patients to pathological fractures. Despite extensive research, the optimal management of UBCs remains controversial. Current treatment strategies-ranging from simple observation to minimally invasive procedures and open surgery-lack standardized guidelines, and clinical decisions are often driven by lesion activity, fracture risk, and surgeon experience. This narrative

Giant cell tumor of bone in pregnancy: A case series from the Eastern Asian Musculoskeletal Oncology Group.

Giant cell tumor of bone (GCTB) typically affects females of reproductive age and may be discovered during pregnancy. Pregnancy entails adaptations that could foster a microenvironment favorable to tumor growth. Although a few case reports have described aggressive features, larger studies in this setting are lacking. Thus, this study aimed to examine the clinical characteristics, obstetrical context, diagnostic process, therapeutic interventions, and prognostic outcomes of patients with GCTBs who presented during pregnancy. Patients were included if they developed pain or swelling during preg

Beyond SINS: A Critical Review of Biomechanical, Microstructural, and Radiomic Biomarkers for Predicting Fracture Risk in Spinal Metastases

Background/Objectives: Although the Spinal Instability Neoplastic Score (SINS) is widely used to estimate spinal metastases fracture risk and guide decisions on stabilisation procedures, prior studies have demonstrated mixed results. Patients with the same score exhibit clinically heterogeneous outcomes, with some SINS criteria correlating less well with the estimated fracture risk than others. There are also barriers to implementation such as the time burden required for manual calculation and interobserver variability associated with qualitative morphological criteria. SINS also lacks sensit

Imaging diagnosis of pseudotumors and nonmalignant masses in athletes.

Athletes are commonly imaged for focal swelling, persistent pain, or a new palpable abnormality after training or injury. These imaging findings are frequently not neoplastic, although several benign processes can appear sufficiently mass-like or aggressive to raise concern for sarcoma or an aggressive lesion. Such "pseudotumors" are more common in routine musculoskeletal practice than primary malignancy. This review approaches the problem as it is encountered in practice: first, the palpable abnormality that prompts imaging, and second, the incidental or symptomatic injury-related lesion that

A retrospective cohort analysis evaluating the utility of the spinal instability neoplastic score in screening for spinal instability and vertebral compression fracture risk in plasma cell neoplasm and lymphoma lesions.

BACKGROUND CONTEXT: Spinal column tumors with Spinal Instability Neoplastic Scores (SINS) suggesting instability often trigger referrals to spine surgeons. Plasma cell lesions and lymphoma are highly radiosensitive histologies, and may reossify after radiation therapy. The SINS score, designed to assess the need for surgical stabilization for spinal neoplasms, may therefore overestimate instability in patients with these radiosensitive histologies. PURPOSE: Herein we seek to determine if the SINS score is significantly correlated with lesion instability and vertebral compression fracture prog

Predicting mechanical failure in spinal metastases: from clinical scores to patient-specific biomechanical modeling.

Spinal metastases are a major cause of pain, vertebral fracture, instability, and neurological compromise. The Spinal Instability Neoplastic Score (SINS) has substantially improved the standardized assessment of neoplastic spinal instability and remains the reference clinical tool for evaluating the mechanical component of metastatic spinal disease. However, it remains a semi-quantitative score, does not directly quantify vertebral strength, and is particularly limited in the intermediate SINS range (7-12), where uncertainty regarding true mechanical risk and therapeutic strategy is often grea

Prediction of compressive strength of vertebral body with metastatic lesions based on quantitative computed tomography-based subject-specific finite element models.

Pathologic vertebral fractures are a major complication in metastatic spine disease. However, current clinical scores, such as Spinal Instability Neoplastic Score (SINS), show limited predictive capability, particularly within the indeterminate range where most clinical uncertainty lies. This study aimed to develop and evaluate quantitative computed tomography (qCT)-based subject-specific finite element (SSFE) models to predict vertebral strength in presence of different metastatic lesion types. Twelve ex vivo human spine segments, each containing one metastatic (n = 12) and one adjacent contr

Bone metastases from hepatocellular carcinoma: Guidance on the management of spinal and long bone lesions.

Bone metastasis from hepatocellular carcinoma (HCC) is a marker of advanced disease and a potential cause of preventable functional loss. HCC skeletal lesions are commonly osteolytic and may be hypervascular, creating risks of pathological fracture, spinal instability, metastatic spinal cord compression (MSCC), neurological deficit, and loss of ambulation. This structured narrative review integrates HCC-specific clinical evidence with established orthopedic and spine oncology frameworks to identify when specialist consultation should occur before irreversible skeletal or neurological deteriora

Prognostic value of the Epidural Spinal Cord Compression scale, Revised Tokuhashi, and Spine Instability Neoplastic Scores in primary stereotactic body radiotherapy for spinal bone metastases.

OBJECTIVE: Stereotactic body radiotherapy (SBRT) has emerged as a highly effective, noninvasive treatment for spinal bone metastases (SBMs). However, long-term outcomes of primary SBRT and the comparative prognostic performance of commonly used decision-making tools remain incompletely defined. This study evaluated long-term local control (LC) and systematically compared the predictive value of the Epidural Spinal Cord Compression (ESCC) grade, Spine Instability Neoplastic Score (SINS), and revised Tokuhashi score (rTS). METHODS: The authors retrospectively analyzed 330 patients with 537 SBMs

Pelvic heterotopic ossification causing bladder compression: a rare post-traumatic case report.

INTRODUCTION: Heterotopic ossification (HO) is the pathological formation of bone within soft tissues, most commonly following trauma or surgery. Pelvic HO that causes direct mechanical compression of the urinary bladder is exceptionally rare. We present a case that remained asymptomatic for over a decade before causing significant lower urinary tract symptoms. CASE PRESENTATION: A 30-year-old man presented with a 4-month history of dysuria, gross hematuria, and left testicular pain, more than 10 years after a high-energy pelvic fracture. Multimodal imaging revealed an exophytic ossified mass

Precardiac mass and femur fracture caused by multiple myeloma in a ferret (Mustela putorius furo).

A 5-year-old female spayed ferret was presented with lameness of the left hind limb and dyspnea. Five months earlier, the animal had been diagnosed with dilated cardiomyopathy and an undifferentiated precardiac mass. Clinical, radiological, and laboratory examinations revealed a left femoral neck fracture, mild anemia, and hyperglobulinemia. Due to the poor general condition and unfavorable prognosis, the animal was euthanized. Pathological examination identified the precardiac mass as multiple myeloma with dissemination to the mediastinum, spleen, liver, kidneys, adrenal glands, lymph nodes,
