Reduced 90-Day Cardiovascular and Infectious Complications After Open Reduction Internal Fixation of Ankle Fractures in Type II Diabetic Patients Using Semaglutide Preoperatively.

BACKGROUND: Patients with type II diabetes mellitus (T2DM) are known to have inferior postoperative outcomes after open reduction and internal fixation (ORIF) of ankle fractures. Semaglutide (the active agent in Ozempic) has encouragingly been associated with improved postoperative outcomes across multiple elective orthopaedic surgeries; however, this has not been assessed for patients with T2DM undergoing ankle fracture ORIF. METHODS: Adult patients with T2DM who underwent ankle fracture ORIF were identified from the 2010-Q1 2023 PearlDiver M170Ortho database. Exclusion criteria included the

[Basic principles of the diagnostics and treatment of malignant bone tumors in old age].

Abnormal X‑ray images and magnetic resonance imaging (MRI) findings in older patients are common and require prompt evaluation to distinguish malignant diseases from harmless incidental findings. A thorough medical history, including pre-existing diseases and previous imaging, is essential. Conventional X‑ray images provide initial indications of lesion dignity using the Lodwick classification. The diagnostics are supplemented by contrast-enhanced MRI and, if necessary, additional CT scans. The most common malignant bone tumors in old age are metastases, followed by lymphomas, plasmacytomas an

Diffuse Osteosclerosis in a Young Cat Presenting With Chronic Nasal Congestion: A Case Report.

CASE SUMMARY: A 5-year-old neutered male domestic medium-hair cat was referred to the internal medicine department with upper respiratory signs and limb pain. Computed tomography (CT) revealed diffuse osteosclerosis, showing severe osseous thickening of the nasal turbinates and near-occlusion of the nasal passages. No new or healed fractures were observed. Additional diagnostic tests revealed no infectious or neoplastic causes. The cat was treated supportively for pain and nasal inflammation. One year after the diagnosis, the cat was described as having an acceptable quality of life at home, d

Musculoskeletal Complications of Radiation Therapy and Immunotherapy.

Radiation therapy (RT) and immunotherapy are widely used in modern oncologic care and are both associated with a broad spectrum of musculoskeletal complications. These adverse effects can be acute or delayed and include bone fragility, insufficiency fractures, osteoradionecrosis, cartilage degeneration, inflammatory arthritis, myositis, and tendinopathy. Many of these complications can closely mimic metastatic disease or infection, creating diagnostic challenges. RT-induced musculoskeletal injuries are influenced by factors such as total dose, fractionation, and technique, with newer modalitie

Current State-of-the-Art 3D MRI Sequences for Assessing Bone Morphology with Emphasis on Cranial and Spinal Imaging: A Narrative Review.

Traditionally, CT has been the go-to method for visualizing bone structures, while MRI has been preferred for assessing soft tissues, because structures containing tightly bound water molecules - such as bones, tendons, cartilage, and ligaments - produce a rapidly decaying T2* signal, which conventional MRI sequences fail to capture. To address this limitation, spoiled gradient echo sequences were refined, and short-TE sequences were introduced, enabling radiation-free bone imaging. This advance is particularly crucial for pediatric patients and in scenarios where an MRI-only approach is prefe

[Radiological imaging of spinal trauma in special situations].

Spinal trauma is rare but associated with considerable morbidity and mortality. Its diagnostic evaluation poses significant challenges for radiologists, particularly in specific patient groups and clinical scenarios. This review article highlights the role of computed tomography and magnetic resonance imaging in acute and specialized imaging of spinal trauma under particular circumstances. Special attention is given to pediatric injuries, normal variants (e.g., pseudosubluxation, os odontoideum), discoligamentous injuries, age- and medication-related osteoporotic fractures, imaging in pregnant

Common Mistakes in Diagnosis of Subchondral Bone Lesions.

Non-neoplastic and noninfectious subchondral bone lesions incorporate several entities: acute traumatic injuries, insufficiency and fatigue fractures, primary or secondary osteonecrosis, osteochondritis dissecans, subchondral abnormalities related to cartilage loss, and marrow changes accompanying disuse or other causes of accelerated bone remodeling. Differentiating these conditions on imaging can be challenging due to overlapping imaging features, technical aspects, and insufficient clinical history. This review discusses how to avoid common mistakes in the imaging diagnosis of subchondral l

Predictors of Kyphoplasty Failures Requiring Surgical Stabilization in Patients With Cancer With Pathological Vertebral Body Fractures.

BACKGROUND AND OBJECTIVES: Pathological vertebral compression fractures (VCFs) cause significant morbidity in the population with cancer. Although both stabilization of fractures with kyphoplasty and pedicle screw fixation can alleviate pain and prevent neurological compromise in select patients, there are no criteria demarcating which patients can be treated with kyphoplasty alone vs pedicle screw fixation, particularly for those with intermediate spinal instability. The objective of this study was to identify predictors of kyphoplasty failure requiring subsequent surgical stabilization in pa

Management of skeletal-related events and fracture prevention in systemic mastocytosis.

Systemic mastocytosis is a rare condition caused by the accumulation of abnormal mast cells (MC) in various organs, including the bone. The diagnosis of mastocytosis is sometimes challenging because of the incidence rate, the heterogeneity of clinical manifestations, and the varying sensitivity of diagnostic tools. While mastocytosis is an uncommon cause of osteoporosis, conversely, systemic mastocytosis (SM) is often associated with bone loss, resulting in low bone mineral density (osteopenia or osteoporosis) and fractures, particularly vertebral fractures. SM must be ruled out in young patie

Deep Learning Model for Differentiating Between Neoplastic Pathologic Fracture and Nonpathologic Fracture Using Hip Radiographs.

BACKGROUND: Although radiographs are the first-line imaging modality, differentiating between neoplastic pathologic fractures and nonpathologic fractures on radiographs can sometimes be challenging. In this study, we aimed to develop and evaluate a deep learning model capable of distinguishing neoplastic pathologic fractures from nonpathologic fractures on hip radiographs to enhance diagnostic accuracy. METHODS: This retrospective, multicenter study analyzed anteroposterior hip radiographs from patients who visited the emergency department at 4 different institutions. The deep learning model

Novel Bone Materials and Adjunctive Approaches in Percutaneous Vertebral Augmentation for Neoplastic Vertebral Compression Fractures.

Neoplastic vertebral compression fractures are a debilitating complication of cancer, causing pain, limiting function, and reducing quality of life. Image-guided percutaneous vertebral augmentation (PVA) involves the injection of bone cement with or without a device into the vertebra. This offers effective stabilization and pain relief but has limitations. The standard polymethylmethacrylate cement is stiffer than native bone and lacks the ability to promote bone regeneration, raising concerns about poor biointegration and increased risk of future fractures. Moreover, exothermic polymerization

Unusual imaging presentation of gossypiboma with pelvic bone involvement: a case report and radiologic review of the literature.

Gossypiboma, although rare, is a serious surgical complication with potential medicolegal implications. We report a case of a 70-year-old female presenting with a progressively enlarging painless mass in the left iliac region, initially suspected to be a pelvic sarcoma. The patient had a history of femoral intramedullary nailing performed 16 years ago for a femoral shaft fracture, followed 2 years later by exchange nailing and iliac bone grafting due to non-union. Ultrasonography revealed a lobulated, avascular mass with acoustic shadowing. Computed tomography (CT) demonstrated a hypodense les

Bone Tumor Mimics.

Non-neoplastic bone lesions represent a heterogeneous group of conditions that stem from reactive, genetic, or metabolic processes. These lesions can sometimes closely mimic malignant tumors, posing diagnostic challenges. This article focuses on a few non-neoplastic bone lesions and their differential diagnoses, including florid reactive periostitis, Paget disease of bone, fractures (including compression/stress fractures), and melorheostosis, each of which exhibits distinct clinical, radiological, and pathologic features. Unlike neoplastic processes of bone, no specific immunohistochemical fe

The Compression Fracture of Thoracic Spine Due to Methotrexate-Associated Lymphoproliferative Disorder in Rheumatoid Arthritis: A Case Report.

Patients with rheumatoid arthritis (RA) are at high risk of developing cardiovascular disease, infections and malignancies during their lifetime, with lymphoma being the most common malignancy. These patients may clinically present with a wide spectrum of lymphoid proliferations or lymphomas, which are called lymphoproliferative disorders (LPD). The prediction sites of LPD can be divided into nodal and extranodal lesions, with the majority being extranodal lesions. The sites of extranodal lesions especially in RA patients with LPD receiving methotrexate (MTX) are the pharynx, liver, spleen, lu
