Efficacy and safety of denosumab de‑escalation in giant cell tumor of bone.

Giant cell tumor of bone (GCTB) is a locally aggressive intermediate bone tumor. Denosumab has shown effectiveness in GCTB treatment; however, the benefits of denosumab de-escalation for unresectable GCTB have not been well discussed. The present study investigated the efficacy and safety of denosumab de-escalation for GCTB. The medical records of 9 patients with unresectable GCTB or resectable GCTB not eligible for resection, who received de-escalated denosumab treatment at Okayama University Hospital (Okayama, Japan) between April 2014 and December 2021, were retrospectively reviewed. The de

The Landscape of microRNAs in Bone Tumor: A Comprehensive Review in Recent Studies.

Cancer, the second greatest cause of mortality worldwide, frequently causes bone metastases in patients with advanced-stage carcinomas such as prostate, breast, and lung cancer. The existence of these metastases contributes to the occurrence of skeletal-related events (SREs), which are defined by excessive pain, pathological fractures, hypercalcemia, and spinal cord compression. These injurious incidents leave uncomfortably in each of the cancer patient's life quality. Primary bone cancers, including osteosarcoma (OS), chondrosarcoma (CS), and Ewing's sarcoma (ES), have unclear origins. MicroR

Osteopoikilosis-the incidental finding of a rare bone dysplasia: A case report.

KEY CLINICAL MESSAGE: Osteopoikilosis is an asymptomatic osteosclerotic dysplasia, of autosomal dominant inheritance, which does not cause deformity or alteration in bone development, of incidental diagnosis. The differential diagnosis should be made with osteoblastic metastases, among others, especially if it is asymmetric and in patients over 50 years of age. ABSTRACT: Osteopoikilosis is a rare benign bone disease, characterized by the appearance of bone islands in the osseous tissue, which could be confused with bone metastasis. We present the case of a 69-year-old man, in whom the presenc

Safety and Efficacy of Balloon-Assisted Kyphoplasty Followed by Stereotactic Body Radiation Therapy for Pathological Fractures.

BACKGROUND AND OBJECTIVES: In patients experiencing pain secondary to pathological compression fractures, balloon-assisted kyphoplasty and subsequent stereotactic body radiation therapy (SBRT) may allow for restoration of vertebral height and irradiation of the underlying malignancy to control local disease progression. The aim of this study was to evaluate the safety and efficacy of kyphoplasty treatment before SBRT in patients with spinal metastases and benign tumors. METHODS: An analysis of a prospectively collected database of 70 patients and 75 metastatic and benign spinal lesions that u

Clinical challenges in prostate cancer management: Metastatic bone-tropism and the role of circulating tumor cells.

Prostate cancer (PCa) metastasis is one of the leading causes of cancer-related mortality in men worldwide, primarily due to its tendency to metastasize, with bones of axial skeleton being the favored target-site. PCa bone-metastasis (PCa-BM) presents significant clinical challenges, especially by the weakening of bone architecture, majorly due to the formation of osteoblastic lesions, leading to severe bone fractures. Another complication is that the disease predominantly affects elderly men. Further exploration is required to understand how the circulating tumor cells (CTCs) adapt to varying

Plasmablastic multiple myeloma presenting as a pleural mass: a diagnostic challenge.

Plasmablastic multiple myeloma (MM) is a rare and highly aggressive variant of MM that presents significant diagnostic and therapeutic challenges. This variant is characterised by a bone marrow infiltration of ≥2% plasmablasts and is distinguished by its atypical pleomorphic morphology, unique immunohistological profile and extensive extramedullary involvement. The anaplastic features of plasmablastic MM can closely mimic those of high-grade lymphomas, leukaemia, non-haematopoietic malignancies and high-grade carcinomas, often leading to initial diagnostic errors. Accurate diagnosis requires a

Reactivating Sleeping Intramedullary Nail in a 16-Year-Old Female with Polyostotic Fibrous Dysplasia: A Case Report on Complications and Potential Solutions.

BACKGROUND: Fibrous dysplasia (FD) is a rare condition in which normal spongy and cortical bone is replaced by non-neoplastic fibrous tissue, leading to weakened bone matrix and increased risk of pathological fractures and deformities. Treating these deformities poses a significant challenge for surgeons. While various cases of surgical stabilization and limb lengthening using intramedullary nails have been reported, there is limited evidence on the use of Motorized Intramedullary Limb-Lengthening Nails (MILLNs) in FD patients. This case report presents the clinical history of a patient with F

Free vascularized fibula transfer in single- or double-barrel technique for reconstruction of segmental bone defects following oncological resection or posttraumatic bone loss.

BACKGROUND: Significant osseous defects or osteonecrosis, precipitated by open fractures, infections, or neoplastic conditions, represent infrequent yet critical medical conditions. The free vascularized fibular graft (FVFG) is a challenging but straightforward, reliable surgical intervention for the reconstruction of defects across various anatomical regions. This study aims to compare, quantify, and demonstrate the FVFG's versatility. The utilization of a single- or double-barrel approach, contingent on the defect's characteristics, optimizes length conservation or enhances the stability of

Two Healthy Elderly Females with Contemporaneous Subchondral Insufficiency Fractures of Bilateral Femoral Heads: A Case Report.

INTRODUCTION: Unilateral subchondral insufficiency fractures (SIF) of the femoral head have been reported in many cases; however, bilateral cases occurring simultaneously are very rare. CASE REPORT: We report two cases of relatively active elderly females (aged 73 and 67 years), in which bilateral hip pain appeared simultaneously without any specific triggers. There were no notable lifestyle or medication histories. Imaging and histopathology of the bone excised surgically supported the diagnosis. In Case 1, there was a decrease in bone mineral density and poor congruency between the femoral
