[Femoral head osteotomy for complex deformity after Legg-Calvé-Perthes disease].

Complex hip deformities after Legg-Calvé-Perthes disease are characterized by an enlarged, asymmetrically deformed femoral head, a short femoral neck, a high-riding greater trochanter, and secondary acetabular dysplasia. These features lead to intra- and extra-articular impingement, instability, abductor weakness, and early osteoarthritis. Conventional osteotomies or isolated acetabular reorientations do not directly address the femoral head deformity and, therefore, show limited results. Detailed analysis of the vascular anatomy enabled the development of intracapital osteotomy with central s

Radiological Evaluation of Distal Trochanteric Transfer in Patients with Proximal Femoral Growth Disturbance.

OBJECTIVES: Proximal femoral growth disturbance (PFGD), resulting from conditions such as Legg-Calvé-Perthes disease and developmental dysplasia of the hip, causes femoral head deformity and trochanteric overgrowth. This study evaluates the effectiveness of distal trochanteric transfer (DTT) in anatomical restoration using objective radiological parameters. METHODS: Patients with unilateral PFGD who presented with pain, abductor muscle weakness, and Trendelenburg gait and subsequently underwent DTT were retrospectively included in this study. All patients had skeletal growth potential, confir

Temporospatial parameters and gait symmetry in hip pathology patients: Pre and post hip preservation surgery.

BACKGROUND: Clinicians specializing in hip preservation surgery perform procedures to increase longevity of the diseased hip. To assess patient ambulation, clinicians use temporospatial parameters (TP), to identify underlying deficits, along with asymmetry. The purpose of this study was to investigate the differences in TP and gait symmetry in a group of adolescent and young adult patients undergoing HPS. METHODS: A retrospective query was conducted for these diagnoses: acetabular hip dysplasia (AHD), femoroacetabular impingement (FAI) and Legg-Calvé-Perthes disease (LCP). Pre-surgery and pos

Peutz-Jeghers Syndrome With Incidental Legg-Calvé-Perthes Disease: A Rare Radiological Association in a Pediatric Patient.

Peutz-Jeghers syndrome (PJS) is a rare autosomal dominant hamartomatous polyposis disorder characterised by mucocutaneous pigmentation and multiple gastrointestinal polyps, most commonly involving the small intestine. Paediatric patients frequently present with complications, such as abdominal pain, gastrointestinal bleeding, and recurrent intussusception. We report the case of a nine-year-old female with classical clinical and radiological features of PJS, including multisite gastrointestinal polyposis and a prior history of intussusception. Contrast-enhanced computed tomography revealed mult

Proximal Femoral Varus Osteotomy Performed in the Early Stage of Legg-Calvé-Perthes Disease Increases Femoral Head Revascularization: A Serial Perfusion MRI Study.

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) is idiopathic osteonecrosis of the femoral head. The effects of proximal femoral varus osteotomy (PFVO) on femoral head revascularization are unknown. We evaluated whether patients with early-stage LCPD treated with PFVO had a higher revascularization rate than those receiving nonoperative treatment with serial perfusion magnetic resonance imaging (pMRI). METHODS: Forty-nine patients (49 hips) in early-stage LCPD with ≥2 pMRIs treated nonoperatively (n = 22) or with PFVO (n = 27) were retrospectively analyzed. The mean age of diagnosis was 8.2 ± 1

MiRNA-223-5p inhibits hypoxia-induced apoptosis of BMSCs and promotes repair in Legg-Calvé-Perthes disease by targeting CHAC2 and activating the Wnt/β-catenin signaling pathway.

Legg-Calvé-Perthes disease (LCPD) involves femoral head osteonecrosis caused by disrupted blood supply, leading to joint deformity and early osteoarthritis. This study investigates the role of miRNA-223-5p in regulating hypoxia-induced apoptosis and enhancing osteogenesis in bone marrow mesenchymal stem cells (BMSCs). Utilizing a juvenile New Zealand white rabbit model of LCPD established through femoral neck ligation, we transfected BMSCs with miR-223-5p mimics, inhibitors, and controls, followed by hypoxic exposure. The impact of miR-223-5p on BMSC apoptosis was assessed using qPCR, Western

Thrombophilia, hypofibrinolysis and osteonecrosis.

Anticoagulation in most patients with familial thrombophilia-hypofibrinolysis and primary osteonecrosis (ON) before hip or knee collapse relieves pain, prevents joint collapse and usually averts the need for joint replacement but is not successful in secondary ON or if started after joint collapse. Anticoagulation in Perthes disease and in ON acutely appearing in post-COVID patients, particularly when factor V Leiden is present, may be valuable as an approach to prevent the otherwise high likelihood of subsequent joint failure. Anticoagulation in primary ON with concurrent thrombophilia-hypofi

Evaluation of the Influence of Changes in Bone Mineral Density and Increases in Articular Cartilage Thickness on Blood Supply of the Femoral Head in Legg-Calvé-Perthes Disease.

PURPOSE: Although the etiology of Legg-Calvé-Perthes disease (LCPD) is not well understood, based on a new theory, it may be caused by a decrease in the supply of blood to the femoral head. The objective of this study was to examine the effects of a decrease in bone mineral density (BMD) and an increase in thickness of articular cartilage on the supply of blood to the femoral head in this group of patients. MATERIALS AND METHODS: This case study was based on a simulation analysis. Computed tomography scan images of a subject with Perthes disease were used to create a three-dimensional model o

Progress in understanding Legg-Calvé-Perthes disease etiology from a molecular and cellular biology perspective.

Legg-Calvé-Perthes disease (LCPD) is a hip disease caused by ischemia of the femoral epiphysis in children, which occurs in children aged 4-8 years (mean 6.5 years), with a male-to-female ratio of about 4:1. The disease has been reported for more than 100 years, but its etiology has not been elucidated. In recent years, a considerable amount of research has been carried out on the etiology of the disease, and the development of the disease is believed to involve a variety of molecular biological alterations, such as the COL2A1 mutation, which may be one of the causes of necrotic collapses of t
