Risk Factors for Developing Perthes Disease: A Comprehensive National Analysis Spanning 2 Decades.

BACKGROUND: Perthes disease is an uncommon pediatric condition affecting the hip joint, causing varying degrees of femoral head necrosis. The underlying cause of Perthes remains unknown, thus it is crucial to identify risk factors associated with its development to aid in early diagnosis and intervention. This study aimed to analyze risk factors associated with Perthes in a large cohort. METHODS: A case-control study was conducted using data from a U.S. national database from 2003 to 2023. Patients diagnosed with Perthes were compared with those without the disease. Variables potentially asso

Anterior Hip Clearance in Residual Legg-Calvé-Perthes Disease.

BACKGROUND: The mechanical environment in the hip in people with residual Legg-Calvé-Perthes disease (LCPD) deformity is still poorly understood. Anterior impingement is thought to contribute to poor long-term outcomes such as early-onset osteoarthritis, but it has not been measured directly using imaging in high flexion. Our objective in this study was to determine the association between radiographic morphologic scores of LCPD deformity and reduced anterior hip clearance. METHODS: We measured the anterior clearance (β-angle) of 20 LCPD-affected hips (17 patients) scanned in 4 functional pos

Legg-Calvé-Perthes disease- surgical treatment options.

Legg-Calvé-Perthes disease (LCPD), is a rare avascular osteonecrosis of the proximal femur usually occurring in children between 5 and 10 years of age. The cause of ischemia leading to necrosis of the femoral head remains unknown. The goal of surgical treatment for LCPD is to improve the containment of the femoral head to restore the function of the hip joint and prevent further damage to the femoral head leading to premature hip osteoarthritis. Although a causal therapy is not available, the main aim is to maintain or restore the containment of the affected hip joint. The specific surgical tr

Genetics and Epigenetics of Legg-Calvé-Perthes Disease.

» Multifactorial Pathogenesis: Legg-Calvé-Perthes disease (LCPD) may result from a complex interplay of genetic, epigenetic, and environmental factors, culminating in avascular necrosis of the femoral head in children aged 4 to 10 years.» Genetic Contributions: Mutations in COL2A1 weaken cartilage integrity, and polymorphisms in IL6 drive inflammatory responses, exacerbating bone resorption and necrosis.» Role of Epigenetics: Epigenetic mechanisms, such as altered DNA methylation and miRNA dysregulation, may modulate disease progression by linking genetic susceptibility to environmental influe

Early age-related changes to articular cartilage T1ρ in hips with Legg-Calvé-Perthes disease deformity.

OBJECTIVE: To determine whether the relationship between cartilage T1ρ relaxation time and age during adolescence and young adulthood is different in hips with Legg-Calvé-Perthes disease (LCPD) deformity compared to hips without LCPD deformity. DESIGN: We recruited 17 participants with LCPD deformity and 15 control participants aged 10-25 years in this frequency-matched cohort study. Quantitative magnetic resonance images were obtained for both hips in LCPD participants and one hip from each control using a T1ρ MAPSS sequence. The average T1ρ relaxation time in the weight-bearing articular ca

How are adults who had Perthes' disease functioning after total hip arthroplasty? : patient-reported outcome results from an international web-based survey.

AIMS: Perthes' disease is a hip disorder that presents in childhood but can lead to diminished quality of life (QoL) in adulthood from hip deformities and secondary arthritis. Little is known about adult outcomes following total hip arthroplasty (THA) from the patient's perspective. We employed a web-based survey to gather data on treatment history, demographic details, and patient-reported outcome measures (PROMs) from adults with Perthes' disease who underwent THA. We then compared these PROMs to those of age- and sex-matched normative cohorts, and a Perthes' disease cohort not treated with

LOCAL INJECTION OF HUMAN DENTAL PULP STEM CELLS FOR TREATMENT OF JUVENILE AVASCULAR NECROSIS OF THE FEMORAL HEAD: PRELIMINARY RESULTS IN IMMATURE PIGS.

INTRODUCTION: Legg-Calvé-Perthes disease is a major cause of hip joint deformities in children. Currently, experimental research is directed at investigating biological therapies, including the use of human dental pulp stem cells (hDPSC), which have not yet been studied for this purpose in swine models. This study aimed to evaluate whether local injection of hDPSC induces bone mineralization in the proximal femoral epiphysis in an experimental model of avascular necrosis of the femoral head in immature pigs. METHODS: Ten immature pigs underwent surgery to induce osteonecrosis of the proximal

Intercondylar Longitudinal Stress Fracture of Femur After Growth Suppression for Limb Length Discrepancy: A Case Report.

CASE: An 11-year-old boy with leg length discrepancy (LLD) secondary to Legg-Calvé-Perthes disease underwent growth suppression using a tension-band plate (TBP) in the distal femur. At 13 years of age, an intercondylar longitudinal stress fracture of the femur was identified on radiographs before implant removal, despite successful LLD correction. Although 3D CT revealed significant asymmetry in the bone morphology, conventional radiography showed no apparent changes. The fracture healed uneventfully after implant removal. CONCLUSION: This case underscores the need for careful postoperative m

A New Skeletal Maturity Methodology for Children With Legg-Calve-Perthes Disease.

BACKGROUND: Legg-Calve-Perthes disease (LCPD) outcomes are largely determined by their age and maturity at onset. In LCPD, there is a known association with delayed skeletal maturity of up to 1.9 years in affected children based on maturity assessment with the Greulich and Pyle (GP) maturity atlas. The GP atlas is the standard for assessing bone age but requires obtaining a separate radiograph of the hand. A new methodology for assessing skeletal maturation in comparison to the GP bone age in children with LCPD was sought. METHODS: A retrospective review of a prospective, multicenter study of

Morphologic and Histologic Assessment of Porcine Model of Legg-Calvé-Perthes Disease at a Clinically Relevant Time Point.

Previous studies using the porcine model of Legg-Calvé-Perthes Disease (LCPD) focused on treatments initiated 1 week following osteonecrosis (ON) induction, which represents the early avascular necrosis (AVN) stage (Stage Ia). Most patients, however, present to the clinic during later stages, like the late AVN (Stage Ib) or the early fragmentation stage (Stage IIa), when development of deformity and revascularization is initiated. This study's purpose was to determine the histologic and morphologic changes in the porcine model at 3 weeks after the ON induction. We hypothesized that the 3-week

Successful Reimplantation of a Femoral Stem Fracture After Cementless Total Hip Replacement Using the Femoral Window Technique in a Small Dog.

Total hip replacement (THR) is a common procedure used in veterinary medicine to treat hip joint diseases, particularly in medium- and large-sized dogs. Although cementless techniques have become the standard owing to concerns regarding the aseptic loosening of cemented implants, complications such as stem fractures can still occur. This study reports a case of a 1-year-old, 2.8 kg Toy Poodle that underwent cementless THR using the Zurich mini-cementless hip system to treat Legg-Calvé-Perthes disease. Six months post-surgery, the dog developed a fractured stem, and a second surgery was perform

Targeted transarterial embolization of the femoral head: development of a minimally invasive approach to model Legg-Calvé-Perthes disease in piglets.

Clinical management of children with Legg-Calvé-Perthes Disease (LCPD) is hampered by incomplete understanding of how the extent of ischemic injury and the duration and quality of subsequent repair determine patient outcome. The traditional piglet model of LCPD is limited to capturing global femoral head ischemia; thus, a new model is needed in which the extent of ischemia can be varied to replicate the spectrum of disease seen in children. In this exploratory study, we used an iterative approach to test and refine methods to bilaterally occlude vessels supplying the femoral heads in n = 8 you
