circCDR1as/miR-671-5p Axis Modulates Chondrocyte Ferroptosis and Macrophage Reprogramming via Targeting NCOA4 and TLR4 in Legg-Calvé-Perthes Disease.

Legg-Calvé-Perthes disease (LCPD) causes malformations of femoral head in children, which presents as a treatment challenge due to its unclear pathogenesis. This study aimed to investigate the influence of circCDR1as on LCPD and its related mechanisms. An in vivo LCPD model was established in spontaneously hypertensive rats (SHRs) and HMGB1-stimulated chondrocytes were used to mimic LCPD in vitro. We found that circCDR1as was up-regulated, while miR-671-5p was down-regulated in LCPD patients, which exhibited a negative correlation. Depletion of circCDR1as restored the proliferation and inhibit

Variability in Bracing Design and Bracing Protocols for the Non-Operative Management of Legg-Calvé-Perthes Disease: A Scoping Systematic Review.

Legg-Calvé-Perthes disease (LCPD) is a self-limiting avascular necrosis of the femoral head in children, with management aimed at preserving hip joint congruency and reducing the risk of long-term degenerative changes. Bracing is a commonly used containment strategy; however, its effectiveness remains controversial owing to variation in brace design, biomechanical principles, and treatment protocols. This systematic review evaluates and synthesizes the variability in bracing design and bracing protocols for non-operative management of LCPD. This review adhered to the PRISMA-ScR guidelines. A c

LongTerm Outcomes of Suspected Transient Synovitis of the Hip in NonAspirated Patients.

BACKGROUND: Transient synovitis is a common cause of pediatric hip pain that must be distinguished from septic arthritis. Although the literature includes outcome data for aspirated hips, long-term outcomes in patients managed presumptively without aspiration remain limited. This study aimed to characterize the long-term clinical outcomes of children with presumed transient synovitis managed without hip aspiration. METHODS: A retrospective study of pediatric patients diagnosed with transient synovitis and managed presumptively without aspiration. Collected variables included demographics, cli

Advances in endothelial cell targeting by AAV vectors.

Adeno-associated virus (AAV) vectors have become a cornerstone of in vivo gene delivery. However, although the endothelium is the first cellular interface encountered after systemic delivery, native AAV serotypes exhibit poor endothelial transduction, favoring hepatocytes, muscle cells and, neurons instead. This limitation represents a major barrier to gene therapies targeting cardiovascular, neurovascular, and inflammatory diseases. This review summarizes recent advances in redirecting AAV tropism toward endothelial cells (ECs) through genetic capsid engineering, peptide display, and non-gene

Isolated Proximal Femoral Valgus Osteotomy for Hinge Abduction Improves Femoroacetabular Relationship in Patients With Perthes Disease.

BACKGROUND: Proximal femoral valgus osteotomy is a recognized salvage procedure for managing hinge abduction in Perthes disease. Although proximal femoral valgus osteotomy aims to improve joint congruency, previous studies have been limited by heterogeneous methodologies and concomitant procedures, making it difficult to determine its isolated effect. Furthermore, no study of which we are aware has specifically examined how proximal femoral valgus osteotomy influences acetabular development or lower limb alignment over time. QUESTIONS/PURPOSES: (1) Is isolated proximal femoral valgus osteotom

Characterization of an Injectable Poly(vinyl alcohol)-gelatin Hydrogel for Growth Factor Delivery in an Orthopedic Application.

Injectable hydrogels have attracted considerable interest because of their minimally invasive delivery and adaptability to patient-specific defects. This study aimed to investigate the suitability of a photo-crosslinkable tyraminated poly(vinyl alcohol)-gelatin (PVA-GT) polymer network as an injectable platform for tissue engineering and growth factor delivery. Two formulations were developed by varying the concentration of ruthenium/sodium persulfate (Ru/SPS), resulting in formulation-specific physical properties with fast (18 days) and slow (36 days) degradation rates. Their injectability wa

The outcomes of treatment and prognostic determinants of patients with Perthes' disease : reanalyses of large prospective multicentre study data using the sphericity deviation score.

AIMS: The outcome of treatment for Perthes' disease has historically been assessed using the Stulberg classification, which is categorical and subjective, as reported in a large prospective multicentre study by the original Perthes' Study Group in 2004. The aim of this study was to reanalyze these outcomes and prognostic determinants using a quantitative outcome measure, the sphericity deviation score. METHODS: We reanalyzed the demographic and radiological data, which included 319 patients followed up for a mean of 8.7 years (2.2 to 15.3), until skeletal maturity. The sphericity deviation sc

Horizontal Cartilage Hypertrophy and Onset Age are Prognostic Indicators of Femoral Head Deformity During Containment Treatment in Legg-Calvé-Perthes Disease.

BACKGROUND: Recent animal studies have suggested that cartilage hypertrophy may occur in Legg-Calvé-Perthes disease (LCPD), potentially influencing femoral head morphology during containment treatment. We aimed to analyze cartilage hypertrophy in children with LCPD using magnetic resonance imaging (MRI) and assess its relationship with long-term femoral head outcomes. METHODS: Fifty-five patients with unilateral LCPD who underwent conservative containment treatment were evaluated. Cartilage thickness was measured in 10 distinct regions on coronal and sagittal MRI scans obtained during the ear

Radiological outcomes in late-onset Perthes after femoral varus osteotomy.

PURPOSE: Treatment of late-onset Legg-Calvé-Perthes disease (LCPD) remains challenging, with outcomes varying by age and extent of involvement. This study aimed to evaluate the radiological outcomes following femoral varus osteotomy (FVO) in children presenting with LCPD after 8 years of age, to determine whether containment surgery provides benefit even in older children. METHODS: We retrospectively reviewed patients aged > 8 years with LCPD who underwent FVO between January 2008 and April 2019 at our institution, with a minimum two-year radiographic follow-up. Disease severity was graded usi

Is Hip Center Elevation an Acceptable Choice for Total Hip Arthroplasty for Legg-Calvé-Perthes Disease? Perioperative Complications and Patient-Reported Outcomes.

BACKGROUND: When positioning the acetabular component for total hip arthroplasty (THA) in patients with sequelae of Legg-Calvé-Perthes disease (LCPD), elevating the center of rotation (COR) of the hip is often unavoidable. We aimed to compare the outcomes between the preserved and elevated COR groups in patients with sequelae of LCPD. METHODS: We enrolled 53 patients who underwent primary THA for sequelae of LCPD between 2006 and 2019. Patients were divided into 2 groups based on the postoperative COR position: 19 in the preserved COR group and 34 in the elevated COR group. The mean elevation

Is congenital thrombotic thrombocytopenic purpura a risk factor for ischemic osteonecrosis of the hip joint (Legg-Calvé-Perthes disease)?

Weise et al. reported on a 41-year-old patient previously diagnosed with congenital thrombotic thrombocytopenic purpura undergoing total hip replacement surgery for Perthes disease. This case raises the question whether ischemic osteonecrosis of the femoral head is another so far underrecognized comorbidity causally linked to congenital thrombotic thrombocytopenic purpura.

Preliminary Report on Hip Abductor Strength Following Proximal Femoral Varus Osteotomy and Greater Trochanteric Apophysiodesis for Legg-Calve-Perthes Disease.

BACKGROUND: Femoral varus osteotomy (FVO) is a common treatment for Legg-Calve-Perthes disease (LCPD). Due to concerns of decreased hip abductor strength, neck-shaft angle (NSA), and articular-trochanteric distance (ATD), concomitant greater trochanteric apophysiodesis (GTA) is often performed. While abductor strength remains a concern, no studies have quantified hip strength following FVO+GTA. The purpose of this study was to objectively quantify hip strength in patients treated with FVO+GTA using isokinetic testing and analyze contributors to abductor weakness. METHODS: A retrospective revi

The influence of extra-articular changes on hip function in young adults with a history of Perthes disease.

BACKGROUND: Residual deformities after Perthes disease, such as high-rising greater trochanter (HGT) and reduced femoral offset (FO), may compromise abductor function and influence long-term hip function. PURPOSE: To determine the prevalence of HGT and abnormal FO in young adults with healed unilateral Perthes disease and evaluate their associations with the Stulberg classification and hip function. METHODS: In this cross-sectional follow-up study, 180 individuals (mean age 28.2 years, 72% male) previously diagnosed with unilateral Perthes disease were examined with radiographs and clinical

Severe Radiographic Legg-Calvé-Perthes Disease With Favorable Long-Term Functional Outcomes Following Conservative Management: A Case Report.

Legg-Calvé-Perthes disease (LCPD) is a pediatric disorder characterized by avascular necrosis of the femoral head, with prognosis largely determined by the extent of epiphyseal involvement. Severe radiographic classifications are typically associated with poor long-term outcomes and often prompt surgical intervention. We present a case of a male patient diagnosed with unilateral LCPD at age three with extensive femoral head involvement by age six (Catterall Group 4, Salter-Thompson Group B, Herring Group C) who was managed non-operatively over two decades. Treatment consisted of physiotherapy,

Familial Cases of Legg-Calvé-Perthes Disease-Hemostatic and Molecular Markers.

Legg-Calvé-Perthes disease (LCPD) is a rare disease caused by avascular necrosis of the femoral head. Although its etiology is still not fully understood, evidence suggests that heritable prothrombotic and inflammatory factors, as well as environmental factors, may be implicated in its onset and progress. The objective of this study is to describe the genetic, biochemical, and environmental factors that may be associated with the etiology of LCPD. This study was conducted in three families and included seven related patients with an LCPD diagnosis. We evaluated the following gene alterations u

[Legg-Calvé-Perthes disease in early childhood: conservative versus surgical management : A risk-adapted containment approach].

BACKGROUND: Early-onset Legg-Calvé-Perthes disease (LCPD) carries a favorable prognosis in the majority of cases; key determinants of outcome are adequate containment (acetabular coverage) of the femoral head and preservation of range of motion during the initial Waldenström stages. DECISION-MAKING: We present practical classifications and a treatment strategy that prioritizes conservative management and focuses operative interventions on "hips at risk." In children younger than approximately 6 years of age, treatment includes activity modification and dedicated ROM therapy. "Hip-at-risk" sig

Determination of Modified Waldenström Staging in Legg-Calvé-Perthes Disease Using Deep Learning.

BACKGROUND: Age and disease stage are critical factors in guiding management in Legg-Calvé-Perthes disease (LCPD). The modified Waldenström system is the most common staging system for LCPD but has only moderate inter-rater reliability. Deep learning (DL) has gained popularity in orthopaedics due to its potential to improve consistency/efficiency in determination of image-based classifications. The current study aimed to develop a DL model for determination of modified Waldenström staging from standard hip radiographs. METHODS: Paired anteroposterior (AP) and frog-lateral hip radiographs from
