Legg-Calvé-Perthes disease (LCPD) is a self-limiting avascular necrosis of the femoral head in children, with management aimed at preserving hip joint congruency and reducing the risk of long-term degenerative changes. Bracing is a commonly used containment strategy; however, its effectiveness remains controversial owing to variation in brace design, biomechanical principles, and treatment protocols. This systematic review evaluates and synthesizes the variability in bracing design and bracing protocols for non-operative management of LCPD. This review adhered to the PRISMA-ScR guidelines. A c

Developmental dysplasia of the hip represents a spectrum of disorders ranging from mild acetabular dysplasia to complete hip dislocation. Early diagnosis and treatment are essential to prevent long-term complications. The Pavlik harness remains the first-line treatment for infants with reducible hip dysplasia or dislocation diagnosed within the first months of life and has demonstrated high success rates. However, complications related to its use may occasionally occur. One of the rare but significant complications is obturator (inferior) dislocation of the hip during treatment. We present the

OBJECTIVE: To explore the feasibility of virtual reality (VR) technology combined with reduction plasty of the femoral head in the treatment of coxa plana and evaluate its effectiveness.
METHODS: Three patients with coxa plana were selected as the research objects between October 2018 and October 2020, all of them were male, aged 15-24 years. Preoperative surgical planning was performed using VR technology; 256 rows of CT data of hip joint were imported into software to generate three-dimensional (3D) imaging, simulate the surgical process, and determine the matching relationship between the

Legg-Calvé-Perthes disease (LCPD) often produces a residual deformity, typically consistent with coxa magna, coxa plana, and ellipsoidal shape. Depending on the degree of asphericity and flatness, this morphology was classified by Stulberg in stages III and IV. Thus far, few studies have investigated physeal injury as an etiological cause or evaluated its progressive profile throughout Waldenström's reossification stage and the remodelling stage. In this study, we analysed the ellipsoidal process of the femoral head. This was a retrospective control case study involving 83 unoperated hips with

Pathogenic sequence variants in the solute carrier family 26 member 2 (SLC26A2) gene result in lethal (achondrogenesis Ib and atelosteogenesis II) and non-lethal (diastrophic dysplasia and recessive multiple epiphyseal dysplasia, rMED) chondrodysplasias. We report on two new patients with rMED and very rare compound heterozygous mutation combinations in non-consanguineous families. Patient I presented in childhood with waddling gait and joint stiffness. Radiographs showed epiphyseal changes, bilateral coxa plana-deformity and knee valgus deformity, for which he underwent surgeries. At present

The purpose of this study was to describe a family with spondyloepiphyseal dysplasia caused by a novel type II collagen gene (COL2A1) mutation and the family's phenotypic diversity. Clinical and radiographic examinations of skeletal dysplasia were conducted on seven affected family members across two generations. The entire coding region of COL2A1, including the flanking intron regions, was analyzed with PCR and direct sequencing. The stature of the subjects ranged from extremely short to within normal height range. Hip deformity and advanced osteoarthritis were noted in all the subjects, rang

BACKGROUND: Impingement ensures a narrow arc of pathologic loading in adolescent hips with sequelae of Perthes Disease. Proper surgical interference when the hip is still in the pre-arthritic stage restores function and protects the young hip from early degenerative changes.
METHODS: Osteochondroplasty at the femoral head-neck junction and relative lengthening of the femoral neck by distal transfer of the greater trochanter was performed for 12 adolescents with combined intra-articular/extra-articular impingement due to sequelae of Perthes disease in terms of coxa magna, coxa plana, coxa brev

OBJECTIVES: The objective of this study was to investigate cases of femoroacetabular impingement (FAI) and analyze the risk factors for a painful hip at skeletal maturity after Perthes disease. We hypothesized that FAI occurs as a sequela of Perthes disease and that coxa plana and triple osteotomy of the pelvis (TOP) may be risk factors.
METHODS: Ninety-five hips were included from 1981 to 2011, 56 of which were operated on with TOP (53) or shelf acetabuloplasty (3). The results were evaluated at a mean 13years of follow-up (range, 2-23years) with Oxford score and hip radiograph analysis incl

OBJECTIVE: To review the results of the treatment of coxa vara associated with femoral neck shortening and high-standing greater trochanter using the new technique of intertrochanteric valgus-lengthening-femoral neck osteotomy.
MATERIAL AND METHOD: Twelve of the fifteen hips treated by the present technique of osteotomy were reviewed. Seven of the twelve cases were posttraumatic (four malunion, two nonunion and one childhood fracture of the femoral neck) and five were developmental conditions (three ischemic necrosis of the femoral head and one physeal dysplasia, all of which secondary to DDH

Femoroacetabular impingement is an undesirable contact between acetabular rim and femoral neck and presents abnormality of proximal femoral part and acetabulum, as well. Two forms may appear: cam impingement and pincer impingement. Femoroacetabular impingement related to Legg-Calvé-Perthes disease may be caused by various reasons, as the consequence of the disease itself, and as the consequence of its treatment. Coxa magna deformity (large femoral head and neck) and coxa brevis deformity (shortened femoral neck) may produce cam femoroacetabular impingement during hip flexion. After the disease

OBJECTIVE: To study early femoral head necrosis radiographic imaging features to provide objective basis for the clinical differential diagnosis and therapeutic schedule's choice.
METHODS: Ninty-one patients with femur head necrosis of ARCO stage I and II whom were diagnosed by clinic and MRI and treated from March 2007 to June 2008 were analyzed retrospectively. There were 32 males and 59 females with an average age of (34.83 +/- 9.57) years ranging from 21 to 60 years. The course of disease were from 2 to 12 months. There were 17 cases of unilateral and 74 of bilateral; 48 cases caused by s

BACKGROUND: In Legg-Calvé-Perthes disease (LCPD), 4 major patterns (coxa plana, coxa magna, coxa vara, subluxation) of the femoral head are commonly observed. However, direct observation of pathological specimens is rarely possible. An animal model of LCPD may clarify the pathogenesis of femoral head deformity.
ANIMALS AND METHODS: In 26 piglets, we interrupted the vascular supply to the capital femoral epiphysis by cutting the ligamentum teres and ligating the femoral neck containing the epiphyseal artery. 6-7 piglets in each experimental group were killed at early (2 and 4 weeks: P2 and P4)

PURPOSE OF THE STUDY: Developmental hip dysplasia in adults causes several anatomical changes which can lead to premature and progressive secondary osteoarthritis. The principle of Chiari osteotomy is the medialisation of the subluxated femoral head. This improves the biomechanics of the hip. The goal of the study was to assess the long term results of Chiari innominate osteotomy.
MATERIAL: The authors evaluated 130 hips after Chiari ostetomy from 230 patients operated on in the sears 1969-1985 at the Ist Orthopaedic Department of St. Anna University Hospital in Brno, Czech Republic. The mean

We report on a Japanese family of four generations with an autosomal dominant precocious osteoarthropathy. The cardinal clinical manifestations of affected individuals were painful weight-bearing large joints, which started in late childhood or adolescence. The radiological hallmarks included coxa plana, mild epiphyseal dysplasia of the knee, and round talar domes with tibiotalar slant in childhood, which evolved into degenerative joint diseases in adulthood. The disease phenotype was cosegregated with a mutation of the cartilage oligomeric matrix protein (COMP) gene in the family members, who

By definition, the signs of osteoarthritis in this context are absent or minor. Isolated or predominant bone situated anywhere in the femoral head, are more or less well demarcated and occasionally multiple or multilocular. They affect men three times more frequently than women with a mean age of 34 years. Painful disability is generally moderate and intermittent for many years. Limitation of movement is minor or absent. The clinical course is very slow and rarely required surgical treatment. Aetiology: in the 23 cases observed over a period of 20 years, two thirds involved a dysmorphic femora

Eighty-four hips of sickle cell anemia patients were followed up during adulthood after the occurrence during childhood of avascular necrosis of the femoral head. Roentgenograms showed residual dysplasia at completion of growth in a substantial number of cases: coxa magna was found in 11 hips, coxa plana or loss of capital sphericity in 53 hips, and true osteochondritis in 3 hips. Only 20 hips exhibited normal morphological features at completion of growth. Anomalies were found not only in the proximal femur but also in the acetabulum which was altered in 14 hips (inadequate lateral coverage i

The authors have followed 140 patients with sickle cell disease anemia (101 cases) or related hemoglobinopathies (39 cases). Among them hip involvement was noted in 55 (104 hips). Forty three times the hip involvement occurred in childhood and twelve times in adult life. When the necrosis appeared in childhood (84 hips) the average age was twelve. The deformity involved the femoral head (coxa plana, coxa magna) as well as the neck (short neck, coxa vara). After an average follow-up of nineteen years, clinical and radiological examinations evidenced 64 functional impairments and 25 arthrosis, 1
