[Non-traumatic osteochondral lesions of the knee joint during growth : Juvenile osteochondritis dissecans (JOCD) of the knee].

BACKGROUND: Juvenile osteochondritis dissecans of the knee joint is the most common osteochondral lesion during growth, usually occurring between the 10th and 14th year of age. PATHOGENESIS: Repetitive microtraumata lead to a subchondral osseus lesion, which is commonly located at the medial aspect of the femoral condyle. Sport activities are considered to be the main cause, although genetic and hereditary factors as well as vitamin D3 deficiency also play a role. Current classification systems distinguish between stable and unstable osteochondral lesions, which is decisive for further treatm

Surgical treatment of atraumatic osteochondrosis dissecans of the immature talus-Clinical results and prevalence of radiographic joint degeneration after a median follow-up of 72.5 months.

BACKGROUND: This study aimed to assess the clinical and radiographic outcomes of different surgical procedures in atraumatic osteochondrosis dissecans (OCD) of the talus in youth and adolescence. METHODS: 32 joints in 30 patients (mean age 14.7 ± 2.2 years) were evaluated. Numeric Rating Scale (NRS), Foot and Functional Index (FFI), American Orthopedic Foot and Ankle Society ankle-hindfoot score (AOFAS), Pediatric Outcome Data Collection Instrument (PODCI), and sport participation were recorded. We compared preoperative and follow-up ankle radiographs to identify specific features in the OCD

Arthroscopic Treatment of Osteochondral Lesions of the Talus Utilizing Juvenile Particulated Cartilage Allograft: A Case Series.

The treatment options for osteochondral lesions of the ankle are scarce, and newer modalities are becoming available. We describe a minimally invasive arthroscopic approach with implantation of juvenile particulated allograft to facilitate the growth of true hyaline cartilage in patients with osteochondral lesions of the talus. The purpose of this study was to subjectively review clinical outcomes using the validated Foot and Ankle Outcomes Score in patients who underwent this technique. Our technique was performed on 82 consecutive patients with average follow-up of 24 (range 9 to 86) months.

Fragmentation of the dorsal distal aspect of the talus on weanling survey and pre-sale radiographs of juvenile Thoroughbreds: prevalence and 2- and 3-year-olds racing performance.

BACKGROUND: Fragmentation of the dorsal aspect of the distal talus (FDDT), at the dorsolateral articular margin of the proximal intertarsal joint (PITJ) on pre-sale radiographs of yearling Thoroughbreds has not been previously described and data to support decisions made by veterinarians to predict future racing potential of horses with these lesions are lacking. METHODS: In this retrospective case-control study we aimed to determine the prevalence of FDDT in juvenile Thoroughbreds and to report their race records. From a database of survey and repository radiographic examinations of 5709 hor

Arthroscopic Treatment of Osteochondral Lesions of the Talus Using Juvenile Articular Cartilage Allograft and Autologous Bone Marrow Aspirate Concentration.

Juvenile allogenic chondrocyte implantation (JACI; DeNovo NT Natural Tissue Graft®; Zimmer, Warsaw, IN) with autologous bone marrow aspirate concentrate (BMAC) is a relatively new all-arthroscopic procedure for treating critical-size osteochondral lesions (OCLs) of the talus. Few studies have investigated the clinical and radiographic outcomes of this procedure. We collected the clinical and radiographic outcomes of patients who had undergone JACI-BMAC for talar OCLs to assess treatment efficacy and cartilage repair tissue quality using magnetic resonance imaging (MRI). Forty-six patients with

Incidence and characteristics of juvenile tarsocrural osteochondrosis in purebred Angus bulls.

Osteochondrosis (OC) of the bovine tarsus has been suggested to contribute to osteoarthritis. The objective of this prospective cohort study was to provide data specific to the Angus breed. Clinical and radiographic exams evaluating OC lesions, effusion and osteoarthritis were performed in 50 purebred bull calves at three time points between 5.8 and 21 months of age. The likelihood of OC was lower at a median age of 12.4 months (P<0.001), primarily due to resolution of distal talus changes (P<0.01). Significant associations were observed between medial malleolus lesions and effusion at median

[Arthroscopic treatment of chondral lesions of the ankle joint. Evidence-based therapy].

Ankle sprains are the most relevant injuries of the lower extremities and can lead to damage to ligaments and osteochondral lesions. Up to 50 % of patients with a sprained ankle later develop a lesion of the cartilage in the ankle joint or an osteochondral lesion of the talus. This can lead to osteoarthritis of the injured ankle joint. Spontaneous healing is possible in all age groups in cases of a bone bruise in the subchondral bone but in isolated chondral injuries is only useful in pediatric patients. In many cases chondral and osteochondral injuries lead to increasing demarcation of the af

[Osteochondritis Dissecans of the Talus - A Critical Review].

The etiology remains unclear. Mechanical factors seem to play an important role. Most histologies report clefts between the cartilage and bone and necrotic areas. Several classifications have been published based on observations from imaging techniques or on intraoperative findings. Clinical symptoms are unspecific. Imaging techniques are of great importance: X-rays of the ankle joint enable a quick diagnostic overview. MRT is the imaging-technique of choice for diagnosing OCDT-lesions. Regarding treatment, a tremendous number of retrospective publications exist. Only a few are classified as l

Radiographic findings of juvenile osteochondral conditions detected in 392 foals using a field radiographic protocol.

Radiographic examination in young horses is commonly used to detect juvenile osteochondral conditions (JOCC). The aim of this study is to present the radiographic findings (RF) detected at the age of 6months in the joints of 392 foals from three breeds: French Trotter Standardbreds (FT), Selle-Français Warmbloods (SF) and Thoroughbreds (TB). Radiographic examination included lateromedial views of the fore and hind digits, tarsi and stifles, and dorsopalmar projections of both carpi. In all areas RF indicative of JOCC were classified according to a four-grade severity scoring system. Distributi

Trauma of the pediatric ankle and foot.

This article presents a brief overview of the injuries to the ankle and foot encountered in children and adolescents. Trauma to the ankle or foot may result from acute, chronic, or repetitive forces. The role of the different imaging modalities in the assessment of ankle and foot trauma in the growing patient is discussed. Plain radiographs remain the mainstay in the diagnosis of most acute traumas, whereas CT may be helpful to unravel the complex anatomy of certain fractures like the triplane or juvenile Tillaux fracture. In the evaluation of chronic injuries, including osteochondrosis dissec

[Histological study of cartilage channels in the epiphyseal cartilage of young dogs and their relationship to that of osteochondrosis dissecans in the most frequently affected locations].

Osteochondrosis is a disease occurring in many species of domestic animals. There are different factors like genetic predisposition, housing and feeding, trauma and stress, which are thought to be the causes or part of the causes of the disease. There have been studies about swine, finding a connection between cartilage channels in juvenile epiphyseal cartilage and the development of osteochondrosis. The aim of the study was to find out whether those connections exist in dogs as well. Special interest has been laid upon the localisation, the contents and the surroundings of cartilage channels

Osteochondrosis and juvenile spavin in equids.

Thirty-six of 50 young equids examined at necropsy for gross pathologic and histopathologic evidence of osteochondrosis were determined to have lesions characteristic of this disorder in the distal joints of the tarsus. Abnormalities ranged from retained endochondral cores underlying undisturbed articular cartilage surfaces to clefts, subchondral osseous cyst-like lesions, and cartilage ulceration. Our findings supported the conclusion that osteochondrosis may cause spavin in the juvenile equid.

Spinal stenosis subsequent to juvenile lumbar osteochondrosis.

This paper describes eight patients with spinal stenosis associated with marked osteochondrous changes in the vertebral bodies due to juvenile lumbar osteochondrosis (Scheuermann's disease). In no case was the midsagittal or interpedicular diameter of the spinal canal indicative of bony stenosis. On the other hand, in the myelograms the sagittal diameter of the dural sac was in all cases significantly narrowed, a diagnostic sign of central spinal stenosis. Therefore, myelography should always be contemplated when osteochondrous changes are present and spinal stenosis is suspected clinically re

[Differential diagnosis of epicondylitis humeri radialis].

Intensive amateur sport will lead especially in the untrained to frequent traumatisation. Exact discussion of the symptomatics of the epicondylitis seems to be necessary. Sufficient therapy of various pain symptoms, which are usually described as epicondylitis is only possible after exact differentiation. Paraarticular pain can be caused by the ligamentum-anulare-syndrome as well as by the classic insertion tendinosis. A hypertrophy of the radio-humeral disc can also be the reason of pain after posttraumatic synovitis. Osteoarthritis and rheumatoid arthritis can easily be differentiated from p

A classification of atypical femoral osteochondroses with children is offered and explained by pertinent examples. In every single case has the diagnosis Perthes disease to be confirmed by securing a skeletal retardation by means or the carporadiogram. If there is no adequate skeletal retardation present or if it is exceedingly high, and if the children are more than 9 years old at the beginning, or if the course of the disease is unusually slow, we have to take into consideration the occurrence of a symptomatic osteochondrosis of the femoral epiphysis. Symptomatic in this context does not mea
