Clinical features and factors associated with severe illness in pediatric acute respiratory tract infections due to Escherichia coli: a retrospective tNGS-based study.

To investigate the detection rate, clinical characteristics, and factors associated with intensive care unit (ICU) admission in hospitalized children with acute respiratory tract infection (ARTI) involving Escherichia coli (E. coli). Bronchoalveolar lavage fluid samples from 4,199 hospitalized children were tested using targeted next-generation sequencing (tNGS). Clinical characteristics, laboratory parameters, imaging findings, and outcomes of E. coli-positive children were analyzed, with comparisons between ICU and non-ICU groups. E. coli was detected in 107 cases (2.5%). The median age was

Disseminated Bartonella or Post-Transplant Lymphoproliferative Disease in Pediatric Transplant Patients: A Diagnostic Challenge.

BACKGROUND: Post transplant lymphoproliferative disease (PTLD) and infection remain the two most common complications of solid organ transplant and carry a risk of significant morbidity and mortality. PTLD is a highly variable disease with clinical signs and symptoms that are non-specific and require a high degree of suspicion to detect. The most common presenting symptoms of PTLD are malaise, fatigue, fever, and a mononucleosis-like picture; night sweats, weight loss, and lymphadenopathy are also frequent manifestations. An infection with Bartonella Henselae is another known cause of local ly

Immune signatures of common exposures through co-occurrence of T-cell receptors in tens of thousands of donors.

BACKGROUND: Memory T cells are records of clonal expansion from prior immune exposures such as infections, vaccines and chronic diseases. Some of the receptors of these expanded T cell clones in a typical immune repertoire are highly public (present in many individuals) because they respond to the same peptide from a prevalent immune exposure, presented by the same Human Leukocyte Antigen (HLA) allele. Only a tiny fraction of public T-cell receptor β sequences (TCRs) have known associations with exposures or specific peptides. METHODS: We mined the TCR repertoires of tens of thousands of dono

Multiple Sclerosis and Viruses: « Liaisons Dangereuses ».

Multiple sclerosis (MS) is a complex, multifactorial neurodegenerative disease with an aetiology that is still partially elusive. Several risk factors are associated with MS development including genetic predisposition and various environmental stressors; however, recently, viral infections have gained significant attention as a crucial environmental trigger. Despite the well-established link between Epstein-Barr virus (EBV) and MS susceptibility, a single-pathogen model fails to clearly characterise the molecular cascades driving the disease, suggesting that the transition from infection to a

Pediatric Guillain-Barré Syndrome Post Organ Transplant: Acute and Chronic Complication Requiring High Index of Suspicion.

BACKGROUND: Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy most often triggered by infections such as Campylobacter jejuni, Epstein-Barr virus (EBV), or cytomegalovirus (CMV). Although transplant-associated GBS is recognized in adults, it remains rare in pediatric recipients, and distinguishing immune-mediated disease from medication toxicity or critical illness can be challenging. METHODS: We describe two pediatric cases of GBS following organ transplantation, one after liver transplant and one after small bowel transplant. Clinical features, diagnostic eva

Augmenting immune reconstitution after adult allo-haematopoietic stem cell transplantation: current developments and modifiable determinants.

PURPOSE OF REVIEW: Immune reconstitution after adult allogeneic haematopoietic stem cell transplantation (allo-HSCT) shapes infection risk, vaccine responsiveness, relapse and nonrelapse mortality. Advances in graft-vs.-host disease (GvHD) prophylaxis, serotherapy exposure management, cytomegalovirus (CMV) control, functional immune monitoring and adoptive cellular therapy have changed which aspects of recovery can be modified and which augmentors are realistic in current practice. This review sets out what has evolved in adult practice and where intervention can now improve it. RECENT FINDIN

Viral RNA-Targeting Locked Nucleic Acid Gapmers: Design and In Vitro Evaluation.

Recurrent viral outbreaks, such as coronavirus disease 2019 (COVID-19), seasonal influenza, and flavivirus infections, highlight the urgent need for rapid-response therapies against viral diseases. These outbreaks are mainly caused by viruses with RNA genomes. Accordingly, antisense oligonucleotides (ASOs), which bind target RNA in a sequence-specific manner to modulate its functions, represent a promising modality for developing antiviral agents. Recent studies, particularly those on RNase H-recruiting ASOs incorporating the nucleic acid analogue locked nucleic acid (LNA), known as LNA gapmer

Natural Killer Cells in Kidney Transplantation: Dual Role in Allograft Injury and Immune Adaptation.

Kidney transplantation is the optimal treatment for end-stage renal disease, yet long-term graft survival remains limited by allograft rejection. While T and B cells and donor-specific antibodies (DSA) have been the main focus, accumulating evidence indicates that natural killer (NK) cells play a significant dual role in both graft injury and immune adaptation, including in DSA-negative rejection. This narrative review summarizes current knowledge on the dual role of NK cells in kidney transplantation, with emphasis on antibody-independent mechanisms, adaptive NK-cell subsets, and their potent

Safety and Efficacy of Therapeutic Agents in Viral Pneumonia in Immunosuppressed Patients.

INTRODUCTION/OBJECTIVE: Immunosuppressed Patients (ISP) - defined by congenital or acquired deficiencies in systemic defense mechanisms - are disproportionately susceptible to viral respiratory infections. Agents such as SARS-CoV-2, Respiratory Syncytial Virus (RSV), influenza A and B, and Cytomegalovirus (CMV) cause significant morbidity in both immunocompetent and immunosuppressed individuals. Clinical manifestations range from mild influenza-like illness to severe pneumonia, respiratory failure, multiorgan dysfunction, and concurrent bacterial infections. METHODS: A narrative synthesis of

Common viral infections seed regionally distinct resident memory T cells in the human CNS.

T cells persist in the central nervous system (CNS) and can drive both protection and neurological disease. How these cells are organized in humans and what they recognize is largely unknown. Here, we profiled CD8⁺ T cells across anatomically distinct CNS regions, obtained through on-site autopsies and temporal lobe resection surgeries, using single-cell RNA sequencing, paired T cell receptor sequencing, and DNA-barcoded tetramers. Resident memory T cells (TRM) specific for Epstein-Barr virus, cytomegalovirus, influenza A, and SARS-CoV-2 were identified across CNS compartments. Anatomical loca

Understanding barriers to human cytomegalovirus prevention: a mixed-methods study on hygiene recommendations for pregnant women.

PROBLEM: Human Cytomegalovirus is the most common congenital infection worldwide, yet recommended hygiene measures to prevent maternal infection are difficult to implement consistently during pregnancy. BACKGROUND: Despite their effectiveness, adherence to hygiene recommendations remains suboptimal, suggesting a gap between recommendations and women's everyday realities. AIM: This study aimed to assess the real‑world applicability of hygiene recommendations for the prevention of cytomegalovirus during pregnancy in French-speaking Switzerland and to explore perceived barriers to their impleme

Cytomegalovirus Infection Is Associated with Increased Disease Activity and Worse Long-Term Course in Ulcerative Colitis.

BACKGROUND: Human cytomegalovirus (CMV) infection is frequently detected in patients with ulcerative colitis (UC), particularly in severe or refractory disease. Its impact on disease activity, long-term course, and clinical outcomes remains controversial. METHODS: In this retrospective single-center cohort study conducted at the University Hospital Münster (Germany), adult UC patients undergoing colonoscopy between May 2002 and May 2024 were included. CMV status was determined by tissue-based PCR from colonic biopsies at baseline colonoscopy (BL). Patients with at least one follow-up colonosc

Single-cell landscape of human liver sinusoidal γδ T cells in the context of latent human cytomegalovirus infection.

Human γδ T cells are typically divided into Vγ9Vδ2 and non-Vγ9Vδ2 cells, but their detailed heterogeneity remains to be fully elucidated, especially in the liver where they are enriched. Here we analyze liver sinusoidal γδ T cells from healthy donors, with or without latent human cytomegalovirus (HCMV) infection, by performing single-cell RNA sequencing with antibody-derived tags. Vγ9Vδ2 cells are characterized by PLZF expression, and classified into type 1 and type 3 immunity-associated clusters. In contrast, non-Vγ9Vδ2 cells are characterized by Helios expression, and their clusters display

Case Report: Severe mixed autoimmune hemolytic anemia associated with acute cytomegalovirus infection in an immunocompetent adult.

BACKGROUND: Cytomegalovirus (CMV)-associated hemolytic anemia (HA) is rare in immunocompetent hosts and may present with heterogeneous serologic phenotypes, including direct antiglobulin test (DAT)-negative hemolysis, warm autoimmune hemolytic anemia (AIHA), cold antibody-mediated AIHA, and rarely mixed AIHA. Because mixed AIHA requires evidence of both warm autoantibodies and clinically significant cold autoantibody activity, this phenotype may be underrecognized without comprehensive immunohematologic testing. CASE PRESENTATION: We report a case of CMV-associated HA that was diagnosed as mi

Evaluation of Cytomegalovirus (CMV) T-Cell Mediated Immunity in Children With CMV DNAemia After Allogeneic Hematopoietic Cell Transplantation.

BACKGROUND: Cytomegalovirus (CMV) is among the most common infectious complications following allogeneic hematopoietic cell transplantation (allo-HCT). The CMV T-cell immunity panel (CMV-TCIP, Viracor) assesses CMV cell-mediated immunity (CMV-CMI); however, its utility among pediatric allo-HCT recipients remains unknown. METHODS: A retrospective analysis (January 2020-January 2024) among pediatric (≤ 18 years) allo-HCT recipients with CMV DNAemia and at least one CMV-TCIP test was performed. A %CMV-CD4+ or %CMV-CD8+ IFN-γ ≤ 0.2% was categorized as a poor response, whereas a result > 0.2% was

Single-center, open-label, randomized controlled trial comparing the tolerability of maribavir 400 mg twice daily versus standard-dose valganciclovir for cytomegalovirus prophylaxis in high-risk kidney transplant recipients.

Cytomegalovirus (CMV) remains a major contributor to morbidity after a kidney transplant. Although valganciclovir (VGC) prophylaxis is effective, its use is limited by leukopenia. Maribavir's (MAR) role in primary CMV prophylaxis is not established. We conducted a 12-month, single-center, open-label, randomized controlled trial including 70 adult recipients at high CMV risk randomized 1:1 to MAR 400 mg twice daily or VGC 900 mg daily. The incidence of clinically significant leukopenia during prophylaxis did not differ between arms (22.9% vs 5.7%, P = .084). For the secondary endpoint, VGC reci

Cytomegalovirus infection and epicardial adipose tissue in liver transplant recipients-Results from a nationwide prospective cohort study.

OBJECTIVES: Liver transplant recipients face increased cardiovascular disease risk, but the pathogenesis is not clear. Cytomegalovirus (CMV) infection, which is common after liver transplantation, has been associated with cardiovascular events. Epicardial adipose tissue (EAT) could represent an unrecognized mediator of CMV-related cardiovascular risk. We aimed to investigate if prior CMV infection was associated with EAT volume in liver transplant recipients. METHODS: From the national prospective Danish Comorbidity in Liver Transplant Recipients Study, we invited liver transplant recipients
