Programmed necrotic cell death (necroptosis) is a key antiviral defense pathway, yet its regulation during human cytomegalovirus (HCMV) infection remains incompletely defined in physiologically relevant cell types. The M45 protein of murine cytomegalovirus, a homologue of UL45, inhibits necroptosis and contributes to MCMV dissemination, providing a rationale for examining UL45 in HCMV. Previous studies of UL45 have largely relied only on fibroblasts or immortalized cell lines, which lack the necroptotic machinery necessary to reveal viral anti-necroptotic functions. To investigate the role of

INTRODUCTION: Fetal infections, particularly those caused by TORCH agents (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes), may significantly affect fetal growth and development, resulting in a substantial risk of morbidity and mortality. Since infections are mostly preventable causes of congenital anomalies, early diagnosis relies on serological and molecular testing and fetal ultrasound.
OBJECTIVE: The aim of this study was to assess the prevalence of fetal infections and their detectable ultrasound features.
METHOD: This retrospective s

This clinical report addresses recognized knowledge gaps regarding congenital CMV infection (cCMV) in infants and expands on information present in the 2024-2027 Red Book. The clinical report reviews the epidemiology of cCMV, clinical manifestations of cCMV disease and cCMV with isolated sensorineural hearing loss (SNHL), diagnosis of cCMV, and treatment of cCMV disease and cCMV with isolated SNHL. The current advantages and disadvantages of different approaches to screening for cCMV infection are reviewed with a recommendation for hearing-targeted (HT) plus expanded testing. Additionally, the

TORCH infections (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes) constitute the classic group of congenital infections that may result in severe fetal and neonatal consequences. Transplacentally transmitted infections have been a critical component of prenatal care for decades. Although screening for this group of infections has been performed in Hungary since 1971, diagnostic approaches and screening strategies have undergone significant transformation. Current international recommendations, as well as national professional guidelines, no

BACKGROUND: To assess the association between prenatal ultrasonography and intrauterine Cytomegalovirus (CMV) infection and to elucidate its clinical characteristics.
METHODS: Cases were selected from three medical centers (2015-2024) in which amniotic fluid CMV-DNA testing was performed following prenatal ultrasound findings suggestive of CMV infection. CMV infection rates were analyzed in relation to distinct sonographic abnormalities. Associations between amniotic fluid viral load, imaging characteristics, and pregnancy outcomes were subsequently evaluated.
RESULTS: Among 1,174 pregnant w

BACKGROUND AND OBJECTIVES: Congenital cytomegalovirus (cCMV) is the most common congenital infection and the leading nongenetic cause of sensorineural hearing loss. Despite expanding legislative screening requirements across the United States, there is no consensus guidance on the screening approach, resulting in wide variation in implementation. Our objective was to assess the outcomes and provide operational insights of initiating cCMV screening within a large hospital system.
METHODS: A multidisciplinary team representing key clinical and laboratory specialties developed a comprehensive cC

High dose valacyclovir treatment of pregnant women has become a common treatment for primary CMV infection, to prevent vertical transmission to the fetus. We present a case of a healthy young pregnant female with primary CMV infection during the first trimester who was diagnosed with acute kidney injury secondary to valacyclovir treatment. This case serves as a reminder to clinicians that prescription of high dose valacyclovir, to prevent vertical transmission of CMV to the fetus, carries a risk of approximately 1% of AKI.

Human cytomegalovirus (HCMV) infection remains a major challenge in allogeneic stem cell recipients. Current immune monitoring largely relies on IFN-γ-based readouts following ex vivo stimulation, which cannot fully capture the complex anti-HCMV immune defense. Therefore, we established a multiparameter HCMV-reactive whole blood (WB) assay that can be initiated at the bedside, requires minimal blood volume, preserves physiological cellular interactions, and enables integrated cellular and cytokine readouts within a closed-tube workflow. Using enhanced α-CD40 co-stimulation for robust detection

Human cytomegalovirus (HCMV) is a frequent complication in kidney transplant recipients (KTRs), often impacting immune regulation. TH9 cells, a subset of CD4+ T cells, are characterized by their secretion of interleukin-9 (IL-9). This study aimed to analyze the phenotypic profile of TH9 cells and their associated cytokine expression in KTRs, and to evaluate mRNA levels of IL-4 and transforming growth factor-β (TGF-β), key cytokines involved in TH9 differentiation. Ten HCMV+ and 10 HCMV- KTRs, along with 10 age- and sex-matched healthy controls, were enrolled. HCMV viral load was quantified usi

The pathogenesis of Graves' disease (GD) has not been fully elucidated, and antibody-mediated immune responses following microbial infection may be involved in its development. We conducted a bidirectional two-sample Mendelian randomization analysis using genetic variants associated with GD and 46 antibody-mediated immune-response phenotypes obtained from large-scale genome-wide association studies. In the forward analysis, none of the antibody-mediated immune-response phenotypes was significantly associated with GD risk after Bonferroni or false discovery rate (FDR) correction. Three Epstein-

Cytomegalovirus (CMV) reactivation is a serious complication after allogeneic hematopoietic cell transplantation (alloHCT), with heightened risk in recipients receiving post-transplant cyclophosphamide (PTCy)-based prophylaxis. Although letermovir reduces clinically significant CMV infection (csCMVi), its prohibitive cost limits access, highlighting the need for affordable alternatives. Unlike solid organ transplantation, there are no published studies reporting the utility of valganciclovir for CMV prophylaxis following PTCy-based alloHCT. The aim of this study was to evaluate the deliverabil

BACKGROUND: Cytomegalovirus (CMV) infection is an underrecognized complication in children with acute lymphoblastic leukemia (ALL). While CMV disease is well documented in transplant recipients, its clinical course in nontransplant settings remains poorly defined. Disseminated ganciclovir-resistant CMV infection is a rare complication in nontransplant children with ALL, and its optimal management remains uncertain.
OBSERVATIONS: We describe an 8-year-old patient with T-ALL receiving maintenance chemotherapy who developed high levels of CMV viremia without initial end-organ disease and subsequ

The CX3CL1-CX3CR1 axis, comprising the chemokine fractalkine (CX3CL1) and its immune cell receptor CX3CR1, links immune activation to vascular pathology in people with HIV (PWH). Chronic inflammation persists in PWH despite viral suppression with antiretroviral therapy and contributes to high rates of cardiometabolic comorbidities. This is partly due to dysregulated leukocyte trafficking and endothelial activation. This review synthesizes preclinical, genetic, and clinical evidence on the multifaceted roles of CX3CR1 across immune, vascular, and cardiometabolic processes. CX3CR1 is expressed o

BACKGROUND: Epidemiological studies have consistently associated cytomegalovirus (CMV) seropositivity with adverse cardiovascular outcomes. However, the mechanisms by which cytomegalovirus infection impacts pathophysiology in the heart remain poorly understood. In this study, we sought to dissect how latent murine cytomegalovirus infection impacts cardiac immune cell dynamics at steady-state and during postmyocardial infarction (MI) repair.
METHODS: Experimental MI studies were conducted in C57BL/6J mice previously infected with murine cytomegalovirus. In situ inflammatory responses were char
