MRI assessment of muscle damage after posterolateral versus SuperPATH approach for total hip arthroplasty.

The minimally invasive SuperPATH approach for total hip arthroplasty (THA) is theorized to reduce soft tissue damage, but objective evidence comparing muscle injury to the conventional posterolateral approach (PLA) is lacking. This study aimed to objectively compare postoperative periacetabular muscle damage between the two approaches using magnetic resonance imaging (MRI). Hypothesis: The SuperPATH approach demonstrates superior efficacy in protecting posterior hip muscles compared to the PLA. In this retrospective comparative cohort study, patients undergoing primary unilateral THA for osteo

Magnesium-Doped Artificial Bone Combined With Pedicled Peri-Acetabular Bone Flap for ARCO Stage II Osteonecrosis of the Femoral Head: A Comparative Study.

OBJECTIVE: Magnesium (Mg) ions promote bone regeneration. Combined with a vascularized periosteal flap providing blood supply and mechanical support, this approach may improve outcomes for osteonecrosis of the femoral head (ONFH), but its clinical efficacy is unclear. This study compared this approach with conventional core decompression (CD) for Association Research Circulation Osseous (ARCO) Stage II ONFH. METHODS: A retrospective analysis was conducted on 76 patients (82 hips) with ARCO Stage II ONFH. The observation group (OG) (39 patients, 44 hips) received Mg-doped artificial bone combi

Italian regional data and oropharyngeal adverse events to drugs and vaccines: insights from a 3-year spontaneous reporting (2022-2024).

BACKGROUND: Oropharyngeal adverse events (O-AEs) represent a potential safety concern associated with several drugs and/or vaccines. Although often underestimated, these events may provide valuable insights into a patient's overall clinical condition, appearing initially mild but later worsening. Therefore, this study aimed to analyze O-AEs related to drugs and/or vaccines using structured safety data. METHODS: Safety reports from three Italian regions were retrieved from the national pharmacovigilance database (rete nazionale di farmacovigilanza, RNF) and analyzed for the period 2022-2024. A

Comparable clinical outcomes but increased technical difficulty after total hip arthroplasty following curved intertrochanteric varus osteotomy.

BACKGROUND: Total hip arthroplasty (THA) is technically challenging after curved intertrochanteric varus osteotomy (CVO) for osteonecrosis of the femoral head (ONFH) due to altered proximal femoral morphology, which might compromise clinical outcomes, implant alignment, and accurate leg length restoration. In this study, we aimed to compare the clinical outcomes and radiographic findings among patients undergoing THA after CVO and primary THA for ONFH. METHODS: This retrospective case-control study included 28 hips that underwent THA after CVO (CVO group) and 56 propensity score-matched prima

Optimizing instrument selection during core decompression for osteonecrosis of the femoral head: a 3D analysis.

BACKGROUND: Core decompression is the leading surgical treatment for pre-collapse osteonecrosis of the femoral head (ONFH). There are many different instruments that are used during core decompression, ranging from drill bits to expandable reamers, yet there is limited guidance regarding how to select the appropriate instrument for an individual patient's unique anatomy. We sought to examine how lesion characteristics affect instrument performance. To quantitatively compare instruments' efficacy at removing necrotic bone while preserving healthy bone, we developed an in-silico metric called th

Automatic diagnosis of osteonecrosis of the femoral head and segmentation of necrotic regions using deep learning at computed tomography: a real-world multicenter study.

BACKGROUND: Computed tomography (CT) is essential for diagnosing osteonecrosis of the femoral head (ONFH) due to its superior visualization of osseous structures, yet CT-based deep learning (DL) models remain underexplored in real-world settings. METHODS: This retrospective multicenter study developed a two-stage DL model integrating Vision Transformer (ViT) and Swin Transformer (SwinT) for ONFH diagnosis and necrotic segmentation, using coronal CT images from 2,312 patients (53,828 images) across four hospitals from February 2023 to June 2024. The data from three hospitals were split 7:1:2 f

Glucocorticoid Receptor Gene Polymorphisms and Femoral Head Osteonecrosis.

Background and Objectives: Non-traumatic osteonecrosis of the femoral head (ONFH) is a multifactorial disorder influenced by both environmental and genetic factors. The nuclear receptor subfamily 3 group C member 1 (NR3C1) gene encodes the glucocorticoid receptor, which plays a key role in bone metabolism, vascular regulation, and stress response. This study aimed to investigate the association between NR3C1 polymorphisms and susceptibility to ONFH, with particular emphasis on age-related genetic effects. Materials and Methods: A hospital-based case-control study was conducted using genotyping

Long-term exposure to ambient air pollution and risk of non-traumatic osteonecrosis of the femoral head: A nationwide cohort study.

INTRODUCTION: Non-traumatic osteonecrosis of the femoral head (ONFH) results from chronic ischemia and impaired bone remodeling, but whether long-term air pollution contributes to its development remains unclear. This study evaluated the association between chronic exposure to ambient air pollutants and the risk of ONFH in a nationwide cohort. METHODS: We analyzed 585,334 adults from Taiwan's Longitudinal Health Insurance Database 2000, followed from 1993 to 2013. Ten-year cumulative exposures to sulfur dioxide (SO₂), carbon monoxide (CO), particulate matter ≤2.5 μm (PM₂.₅) and ≤ 10 μm (PM₁₀)

Spontaneous Osteonecrosis of the Knee Is Associated With Increased Five-Year Implant Complications Compared to Osteoarthritis Patients Undergoing Primary Total Knee Arthroplasty.

BACKGROUND: Spontaneous osteonecrosis of the knee (SPONK) is a potential, although uncommon, indication for total knee arthroplasty (TKA). Unlike secondary osteonecrosis, SPONK typically affects a single femoral condyle in patients over 50 years of age and is sometimes treated with TKA once joint-preserving strategies fail. However, implant survivorship of TKA for SPONK versus osteoarthritis (OA) remains under-characterized. This study aimed to compare outcomes, including (a) mechanical complications (aseptic loosening, periprosthetic fracture), (b) periprosthetic joint infection, and (c) all-

Application of Quantitative Computed Tomography and Finite Element Analysis to the Lunate: An Exploratory Study of Bone Density and Strength in Young and Older Women.

PURPOSE: Lunate collapse in older adult women has been sporadically reported as a condition distinct from classical Kienböck disease, which is typically observed in young men, and may represent fragility fractures associated with osteoporosis. Nonetheless, age-related differences in lunate bone density and mechanical properties have not been quantitatively assessed owing to the bone's small and complex anatomy. METHODS: We conducted an exploratory cross-sectional study of 21 young women (20-29 years) and 57 older adult women (≥65 years). Using quantitative computed tomography with finite elem

Risk of Femoral Head Osteonecrosis Following Hip Trauma in Adults With Systemic Lupus Erythematosus.

BACKGROUND/OBJECTIVE: Systemic lupus erythematosus (SLE) is a known risk factor for femoral head osteonecrosis (FHON), but its impact on FHON risk following hip trauma has not been well characterized. We evaluated whether adults with SLE have an increased risk of FHON after hip trauma compared with patients without SLE. METHODS: We conducted a retrospective cohort study using de-identified electronic health record data from the TriNetX Research Network. Adults aged 18 to 65 years with an incident hip trauma diagnosis and no prior FHON were included. SLE was defined by the presence of an ICD-1

Emergence of unique hues from sparse coding of color in natural scenes.

BACKGROUND: Kümmell's disease is a severe post-traumatic vertebral osteonecrosis characterized by delayed collapse and kyphosis following osteoporotic vertebral compression fractures (OVCFs). The transition from acute fracture to nonunion remains poorly understood. Using a single-center retrospective cohort, we aimed to develop and validate a nomogram to predict Kümmell's disease in patients with acute OVCFs undergoing conservative management. METHODS: We included 626 patients (aged ≥50 years) with acute or subacute OVCFs treated conservatively between January 2021 and October 2024, randomly

Initiation of Collapse in Osteonecrosis of the Femoral Head: A Patient-Specific Finite Element Simulation Study.

Collapse in osteonecrosis of the femoral head (ONFH) is thought to result from stress concentration at the sclerotic boundary zone, but its precise initiation site has not been identified. We aimed to identify the initiation of collapse using patient-specific finite element models (FEMs) that incorporate the sclerotic boundary zone. Ten hips from 10 patients with type C ONFH who had both pre-collapse (stage 2) and post-collapse (stage 3) computed tomography (CT) images were analyzed. Actual collapse sites were identified on post-collapse CT images or, when inconclusive, determined using micro-
