Increased Susceptibility to Long-Term Complications in Leukemia Survivors Who Received Hematopoietic Stem Cell Transplantation During Adolescence.

Hematopoietic stem cell transplantation (HSCT) is essential for curing high-risk or relapsed acute leukemia but entails lifelong morbidities. Adolescents and young adults may be particularly vulnerable; however, data on the global risk of long-term complications in survivors who underwent HSCT during adolescence are lacking. Using the French Leucémie Enfant Adolescent cohort, we compared late complications and quality of life (QoL) in survivors transplanted at ≥15 versus <15 yr. We included 168 adolescents (≥15 yr of age at HSCT) and 446 children (<15 yr of age at HSCT) matched at best 1:3 on

Involucrum Formation Leading to Healing in Stage 3 Medication-Related Osteonecrosis of the Jaws.

INTRODUCTION: An involucrum refers to the formation of new bone by the periosteum surrounding a bony sequestrum. Over time, the sequestrum undergoes resorption via creeping substitution, ultimately leading to complete bone healing. This phenomenon is well-documented in the context of long bone osteomyelitis. METHOD: We report a case of an 82-year-old woman who healed spontaneously from stage 3 medication-related osteonecrosis of the jaw following dentoalveolar surgery while on denosumab. AIM: The aim of this article is to highlight a rare instance of resolution through the process of involuc

Effect of combined anteversion on outcomes in total hip replacement.

Osteonecrosis of the femoral head is a debilitating disease where the bone tissue dies as a result of impaired blood flow, which affects young adults and in most cases, it leads to collapse in the absence of surgical intervention. In this analytical cross-sectional study, 30 patients with osteonecrosis of the femur head were compared to determine the treatment outcomes of operative management using total hip arthroplasty (THA) and conservative treatment over 18 months. Operative group showed considerably better results with a change in Modified Harris Hip Score of 457 to 756 at 6 months compar

Triple-Modality Approach for Kienböck Disease (Stage II and IIIA): Outcomes of Distal Radius Core Decompression, Radial Shortening and Scapho-Capitate K-Wiring in 18 Patients.

Background: Early-stage Kienböck disease (KD), particularly stages II and IIIA, requires timely intervention to prevent progression and preserve wrist function. This study evaluates the outcomes of a triple-modality approach combining distal radius core decompression, radial shortening osteotomy (RSO) and scapho-capitate K-wiring. Methods: A retrospective case series of 18 patients with stage II and IIIA KD were managed with this triple-modality approach at a single centre. Pain was assessed using the visual analogue scale score. Functional outcomes were assessed using the Modified Mayo Wrist

Anatomy of the intraosseous arterial microvasculature of the adult lumbar vertebral body and its role in fracture healing: a literature review and historical perspective.

BACKGROUND AND OBJECTIVE: Although awareness of vertebral body osteonecrosis following fractures has increased, it remains underdiagnosed and represents one of the most challenging and unpredictable complications after spinal trauma. The arterial supply of the vertebral body may play an important role in fracture healing; however, this remains unproven and is not currently incorporated into treatment algorithms. This study aims to conduct a narrative review of the intraosseous arterial microvasculature of the adult lumbar vertebral body and its role in fracture healing, including a historical

Risk Factors Associated With Osteonecrosis of the Femoral Head After Undergoing Lumbar Decompression and Fusion.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) after spinal fusion is poorly characterized. This study sought to identify factors associated with ONFH following lumbar, lumbo-sacral, or lumbo-pelvic fusion. We hypothesized that abnormal postoperative spino-pelvic alignment and modifiable intraoperative factors may increase ONFH risk. Furthermore, we felt these factors may reduce survivorship free from total hip arthroplasty (THA) after lumbar fusion. METHODS: We retrospectively reviewed 9,380 lumbar fusion patients from 2013 to 2021 at a single institution. There were 50 patients who ha

Corticosteroid-associated osteonecrosis in paediatric compared with adult patients: are they the same disease?

Osteonecrosis of the femoral head (ONFH) is a debilitating condition with a poorly characterized pathogenesis. Among the reported aetiologies, excessive alcohol consumption and corticosteroid administration are leading causes. Corticosteroids are often administered as a life-saving treatment for primary oncological or autoimmune conditions, but the subsequent development of ONFH can significantly impair the quality of life of both paediatric and adult patients. This review provides a comprehensive summary and critical analysis of recent advances in the understanding of corticosteroid-associate

A New Stage and Classification of Osteonecrosis of the Femoral Head Based on MRI With Pathophysiological Interpretation.

BACKGROUND: To explore the influence of the new stage and classification system based on the correlation between MRI and pathological features on the collapse, symptoms, and function of femoral head necrosis. Existing radiographic staging systems primarily evaluate structural changes and lesion morphology but provide limited information regarding intraosseous biological activity and pathological heterogeneity. Clinically, patients within the same ARCO stage often demonstrate markedly different symptoms, collapse patterns, and treatment responses, highlighting the need for an MRI-based classifi

Osteonecrosis of the Humeral Head Following Anterior Shoulder Dislocation in a Dialysis-Dependent Patient: A Case Report.

CASE: A 75-year-old woman on long-term hemodialysis sustained anterior shoulder dislocation without a fracture. Four months later, she presented with persistent shoulder pain and limited motion. Imaging revealed humeral head osteonecrosis (Cruess stage V) and a massive rotator cuff tear. Reverse shoulder arthroplasty was performed with favorable outcome. CONCLUSION: Osteonecrosis of the humeral head following anterior shoulder dislocation without a fracture is rare. To our knowledge, this is the first reported case in a dialysis-dependent patient. This case highlights the need for increased c

Deep learning in the diagnosis, staging, and prognosis of osteonecrosis of the femoral head: a systematic review.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) is a progressive ischemic bone disorder that often leads to structural collapse and early arthroplasty in adults. Early detection and accurate staging are essential for joint-preserving management. Deep learning (DL) has shown high promise in musculoskeletal imaging; however, the methodological quality and How well these ONFH-related DL models translate to clinical practice remain unclear. This systematic review synthesized current evidence on DL applications for ONFH diagnosis, staging, segmentation, and prognosis across imaging modalities,

Every Six-Month versus Single-Dose Adjuvant Zoledronate in Early Breast Cancer.

BACKGROUND: Three-year data from a randomized trial comparing a single zoledronate infusion with infusions every 6 months were previously reported showing that a single infusion was associated with increased patient convenience, less toxicity, and lower rates of treatment discontinuation at 3 years. We now report the trial's final, prespecified 5-year follow-up data. METHODS: Patients who were postmenopausal (either naturally or treatment induced) with early breast cancer were randomly assigned to receive a single infusion of zoledronate (4 mg intravenously) or an infusion every 6 months for

Cortical Support in Unstable Intertrochanteric Fracture without Fixation of Posteromedial Fragment.

BACKGROUND: It is well known that the posteromedial (PM) buttress plays an important biomechanical role in ensuring stability in unstable intertrochanteric (INT) fractures. However, the necessity for the fixation of PM fragments remains controversial. This study aims to assess surgical outcomes, including fracture site collapse and fixation failure, when PM fragments are not fixed in patients who are treated with intramedullary nailing for unstable INT fractures with large PM fragments. METHODS: We retrospectively analyzed 658 hip fracture surgeries performed at a tertiary referral hospital b

Multiscale mechano-biological characterization of necrotic, sclerotic, and healthy zones in femoral head osteonecrosis: linking viscoelastic heterogeneity to mechanical failure.

Osteonecrosis of the femoral head (ONFH) is a progressive disorder characterized by regional trabecular bone deterioration that ultimately results in collapse. While elastic property changes in diseased bones are well documented, the contribution of region-specific viscoelastic behavior and its relationship to multiscale microarchitecture and composition remain insufficiently defined. Eighteen femoral heads (mean age: 40.8 ± 7.1 years; 15 males, 3 females; diagnosis: Ficat Stage III/IV) were retrieved during total hip arthroplasty. Patient-matched trabecular cores were harvested from three dis
