Correlation Between Radiographic Parameters and Clinical Range of Motion in Slipped Capital Femoral Epiphysis.

BACKGROUND: Surgical procedures that improve motion for slipped capital femoral epiphysis (SCFE) deformity are typically guided by radiographic imaging. However, the specific aspects of the deformity that have the greatest impact on hip functional biomechanics remain unclear. This study investigated the relationship between radiographic parameters and clinical ROM measurements in patients with SCFE to better elucidate which elements of SCFE pathoanatomy have the greatest effect on joint mobility. METHODS: Patients who underwent in situ pinning for stable SCFE at a single institution between 2

Acetabular posterior wall morphology in slipped capital femoral epiphysis: a three-dimensional and true axial computed tomography matched-cohort study.

AIMS: Comparative assessment of both hip joints in patients with slipped capital femoral epiphysis (SCFE) was performed relative to the control group. METHODS: We created 3D CT reconstructions of the acetabulum in 27 hips with SCFE, 21 unaffected contralateral hips, and 108 hips of the age- and sex-matched non-SCFE control group. Following measurements performed using true axial CT scans were assessed: acetabular version angle (AVA°), posterior sector acetabular angle (PASA°), posterior wall angle (PWA°), and acetabular roofing percentage. RESULTS: Based on the morphology of the posterior ac

Screw start point and orthogonal relationship to the physis do not impact clinical outcomes for in-situ fixation of slipped capital femoral epiphysis.

UNLABELLED: In-situ screw fixation (ISSF) is the gold standard for treatment of slipped capital femoral epiphysis (SCFE). While central screw placement within the epiphysis is known to reduce the risk of intra-articular penetration, the impact of the screw starting point on the proximal femur and the resulting screw trajectory relative to the proximal femoral physis on radiographic or clinical outcomes remains unclear. This study evaluated whether screw starting point or inclination affects outcomes following ISSF. A retrospective review was performed of SCFE patients treated with ISSF at a si

Teenage Total Hip Arthroplasty Yields High Satisfaction and Excellent Survival up to 20-Year Follow-Up.

BACKGROUND: Total hip arthroplasty (THA) is increasingly performed in patients younger than 20 years who have destructive hip pathology. The unique anatomical and developmental characteristics of this population, along with the high likelihood of future revision surgery, have led to cautious clinical decision-making. Data on safety, effectiveness, and long-term (20 years) outcomes in teenagers remain limited. This study aimed to evaluate implant survival, patient satisfaction, functional outcomes, and complications following THA in teenagers. METHODS: This retrospective cohort study with cros

A Simple Modification for In Situ Pinning in Slipped Capital Femoral Epiphysis on a Radiolucent Table to Reduce Operative Time and Radiation Exposure.

BACKGROUND: In situ pinning is the mainstay treatment method in mild and moderate slipped capital femoral epiphysis (SCFE). Despite successful outcomes, some technical difficulties may be encountered during surgery, typically in overweight SCFE patients. The aim of the study is to compare the use of a bone marrow biopsy needle system to navigate the cannulated screw guidewire with the traditional in situ pinning technique. METHODS: We retrospectively reviewed patients undergoing in situ pinning for stable mild-moderate SCFE at 2 different institutions between 2022 and 2024. Included patients

Automated Detection of Pediatric Slipped Capital Femoral Epiphysis: A Deep Learning Approach Using Anatomically Informed Attention Guidance.

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is an adolescent hip disorder that is often missed on initial presentation due to subtle radiographic findings, leading to significant complications. Traditional diagnostic methods like Klein's line are subjective and prone to inter-observer variability. This study aimed to develop and validate an attention-guided deep learning model to automatically identify SCFE on pediatric pelvic radiographs. METHODS: A total of 174 pelvic radiographs (139 training, 35 testing) were retrospectively collected. A two-stage model was developed: first, a U-

Slipped capital femoral epiphysis: pathomechanism, clinical presentation, diagnosis, natural history and treatment. A review of the literature.

Slipped capital femoral epiphysis (SCFE) occurs in preadolescent and adolescent children. The exact pathomechanism of the slip is not known. Several risk factors have been identified including hormonal derangement. Of the classification systems, stability and severity classification systems are most relevant to everyday practice. There is a strong agreement regarding the management of a mild slip. Controversies exist in the treatment of moderate and severe slips and also regarding the prophylactic pinning of the opposite side. Discrepancies exist amongst countries and surgeons as to the number

Contemporary Total Hip Arthroplasty for Sequelae of Slipped Capital Femoral Epiphysis: 15-Year Outcomes of 165 Cases.

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a pediatric hip disorder whose sequelae may require total hip arthroplasty (THA) at a young age. The outcomes of contemporary THA in this population need to be defined further. We aimed to evaluate survivorship, complications, radiographic results, and clinical outcomes in this population. METHODS: We identified 165 THAs performed for sequelae of SCFE between 2000 and 2022. The mean age at THA was 45 years, 71% were men, and they had a mean body mass index of 33. There were 133 hips (81%) with prior surgical treatment, most commonly in s

Outpatient Management of Slipped Capital Femoral Epiphysis Is Common, Safe, and Saves Costs: A Single-Institution Analysis and Results From a National Database Survey.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is a condition requiring surgical fixation on a semiurgent basis. Although recent studies have evaluated the safety of outpatient SCFE management, none have examined cost in this population. Furthermore, it is unclear what percent of in situ SCFE fixation is done on an outpatient basis nationally. METHODS: A retrospective chart review of patients treated between the years 2015 to 2024 was conducted at a large tertiary-care children's health system. Patients undergoing in situ fixation for stable SCFE and a minimum 90 days follow-up were i

Slipped capital femoral epiphysis in an adult with an unidentified Kallmann syndrome. Case report and etiology literature review.

A 23-year-old male consulted for a subacute-onset left hip pain, being evident on radiographs a slipped capital femoral epiphysis (SCFE) while the contralateral proximal femur physis was still slightly open. Urgent percutaneous in-situ fixation with two cannulated screws was performed. Further investigations due to the atypical features of the case revealed that the patient had anosmia, which allowed for Kallmann syndrome (KS) diagnosis, being responsible for the delayed skeletal maturity of the patient. Very few cases of a delayed-onset SCFE in association with KS have been described in the l

Valgus Slipped Capital Femoral Epiphysis Associated With Osteosclerotic Dysplasia Due to Hyperphosphatemic Tumoral Calcinosis: A Case Report.

CASE: A 13-year-old boy presented with a painful limp for 2 weeks without trauma and guarded (L) hip movements with spasm. Radiographs revealed a valgus slipped capital femoral epiphysis (SCFE) of the left hip with generalized osteosclerosis. Elevated serum calcium, phosphate, and parathyroid hormone levels were noted. Whole-exome sequencing identified a homozygous FGF23 variant, confirming hyperphosphatemic tumoral calcinosis. He was managed with phosphate binders, dietary restriction, and in situ fixation of both hips. At the 2-year follow-up, alignment and function were normal. CONCLUSION:

The Swedish experience of capital realignment surgery for slipped capital femoral epiphysis: results from a nationwide retrospective cohort of 28 patients.

BACKGROUND AND PURPOSE: Optimal treatment for slipped capital femoral epiphysis (SCFE) has been controversial for decades. Capital realignment surgery (predominant technique: modified Dunn) is a technique to restore anatomy, but it is associated with varying risks of avascular necrosis (AVN) of the femoral head. The aim of our study was to describe outcomes following capital realignment for severe SCFE in a nationwide cohort. METHODS: A longitudinal, nationwide, retrospective cohort study of SCFE patients treated with capital realignment between 2009 and 2021 was conducted, with radiographic

Outcome of secondary intertrochanteric osteotomy for residual deformity after slipped capital femoral epiphysis.

BackgroundAlthough in situ pinning (ISP) is the standard primary treatment for slipped capital femoral epiphysis (SCFE), some patients are left with residual deformity that may lead to long-term complications, including an increased risk of osteoarthritis. This study evaluates the radiographic and clinical outcomes of a secondary corrective procedure, the Pre-Operative computed Tomography-assisted intertrochanteric Flexion (POTOF) osteotomy, for treating these persistent deformities.MethodsWe retrospectively reviewed patients who underwent a POTOF osteotomy as a secondary procedure for SCFE be

Thinking Beyond the Hip: A Clinico-Radiological Approach to Atypical Paediatric Hip Pain for the Generalist.

Paediatric hip pain is a common clinical presentation and can be caused by a wide range of aetiologies, many of which are uniquely related to the growing immature skeletal system such as developmental hip dysplasia, Perthes disease and slipped capital femoral epiphysis. However, atypical or referred causes of hip pain often require a broader diagnostic approach beyond the hip joint itself, and can pose diagnostic challenges for clinicians. This article aims to provide a comprehensive review from an imaging perspective, highlighting common pathologies outside the hip that can also present with

Direct anterior approach for open correction of Slipped Capital Femoral Epiphysiolysis: surgical technique and clinical outcome.

BACKGROUND: Slipped Capital Femoral Epiphysis (SCFE) is an idiopathic hip disorder in adolescents. With or without surgery, avascular necrosis (AVN) is the most dangerous complication. For open reduction and internal fixation, the modified Dunn procedure is considered the gold standard. This study presents an open epiphyseal realignment through the Hueter-interval and reports on (1) surgical technique of direct anterior approach (DAA) based surgical realignment, (2) radiographic correction of biomechanics, and (3) AVN-incidence after surgery. METHODS: A single-center retrospective case series

Temporary in Situ Pinning With Subsequent Modified Dunn is a Safe Alternative to Primary Modified Dunn in Moderate and Severe Slipped Capital Femoral Epiphysis.

BACKGROUND: Surgical hip dislocation with modified Dunn procedure (MD) is a viable treatment option for moderate to severe slipped capital femoral epiphysis (SCFE). In the acute setting, an experienced surgeon may not be immediately available for the MD. However, in situ pinning (ISP) is a well-accepted initial treatment and may be more familiar to the on-call orthopaedic surgeon. Our aim was to compare outcomes at short and midterm follow-up in patients with moderate/severe SCFE undergoing ISP with subsequent MD versus primary MD. METHODS: Between June 2017 and July 2024, we treated 29 patie

Long-term outcomes of the modified Dunn procedure in moderate and severe slipped capital femoral epiphysis: a prospective case series with 7-year follow-up.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in adolescents and may lead to femoroacetabular impingement, early osteoarthritis, and long-term functional disability if inadequately treated. While in situ pinning remains the standard treatment for mild slips, it fails to correct the deformity in moderate and severe cases, potentially predisposing to degenerative changes. The modified Dunn procedure (MDP) was developed to restore proximal femoral anatomy through surgical hip dislocation while preserving vascular supply. The aim of the study is to evaluate
