Mid-term outcomes of the modified Dunn procedure for slipped capital femoral epiphysis: results from a north African pediatric hip unit.

BACKGROUND: Slipped Capital Femoral Epiphysis (SCFE) is a prevalent pediatric orthopedic condition. Treatment options range from in situ pinning to various osteotomies, with the Modified Dunn procedure gaining significant attention over the past two decades. However, the suitability of this procedure for different SCFE subtypes and the risk of avascular necrosis (AVN), particularly in moderate and severe cases, remains controversial. This study aims to report the midterm clinical and radiographic outcomes of the Modified Dunn procedure in treating SCFE, emphasizing the factors contributing to

Effect of Vitamin D Deficiency on Development of Slipped Capital Femoral Epiphysis.

OBJECTIVE: Slipped capital femoral epiphysis (SCFE) is one of the most common hip disorders in adolescents, often linked to obesity. However, other factors, such as vitamin D deficiency, may also contribute to SCFE development. This study investigates the impact of vitamin D deficiency on SCFE development in a large cohort. METHODS: We utilized the TriNetX national database to query pediatric patients with documented calcidiol levels. Patients with a recorded visit below the age of 9 and subsequent documented calcidiol levels were followed until SCFE occurrence or age 18. Patients were catego

Do we need to dislocate the femoral head? A retrospective comparison of unstable slipped capital femoral epiphysis (SCFE) cohorts treated by two variations of the modified Dunn procedure.

PURPOSE: Evaluate a modification of the original modified Dunn technique, for unstable slipped capital femoral epiphysis (SCFE), where the proximal femoral epiphysis is not dislocated from the acetabulum. METHODS: We compared two cohorts of patients with unstable SCFE: one operated with dislocation of the femoral epiphysis (DG) and the other without (NDG). Groups were compared demographically and radiographically. Femoral head bleeding was determined after reduction of the slip. Operative time, development of AVN and other complications were registered. The influence of surgical technique and

Delayed slipped capital femoral epiphysis with hypothyroidism in children: A case report.

RATIONALE: Slipped capital femoral epiphysis (SCFE) is a common adolescent hip disorder, but its association with hypothyroidism remains rare and poorly understood. This case highlights the importance of considering endocrine disorders, such as hypothyroidism, as potential risk factors in atypical SCFE presentations, particularly when trauma or other common etiologies are absent. PATIENT CONCERNS: A 14-year-old boy presented with progressive, nontraumatic left hip pain that worsened over 6 weeks, eventually leading to complete immobility. Initially managed unsuccessfully with traditional Chin

Lateral Proximal Femoral Epiphyseal Vessel Avulsion in Slipped Capital Femoral Epiphysis: A Newly Identified Mechanism of Avascularity: A Case Report.

CASE: We report 2 cases of Loder-unstable slipped capital femoral epiphysis (SCFE). Both patients had initial pinning followed by surgical hip dislocation where complete avulsion of the lateral epiphyseal vessels from the nutrient foramina was identified. Both patients underwent epiphysiodesis to revascularize the femoral head but developed osteonecrosis with collapse 9 months after their injury. CONCLUSION: Avulsion of the lateral epiphyseal vessels currently represents a nonreversible mechanism of avascularity in SCFE. Although Loder previously theorized the existence of this mechanism, it

Pinning and Arthroscopic Trimming for Mild Slipped Capital Femoral Epiphysis: Comparative Study With Traditional In Situ Pinning and Modified Dunn Procedure.

INTRODUCTION: Mild slipped capital femoral epiphysis (SCFE) was traditionally treated with in situ pinning (ISP), a technique that stabilizes the physis but does not restore the anterior-superior femoral head offset. This residual deformity can lead to femoroacetabular impingement and early joint degeneration. Therefore, immediate arthroscopic trimming was associated to in situ pinning (ISPAT). The primary goal is to assess the clinical and radiologic outcomes of patients treated with ISPAT. Secondarily, outcomes were compared with traditional in situ pinning (ISP) and modified Dunn's procedur

Bone Scintigraphy Prediction of Postoperative Femoral Head Avascular Necrosis in Children With Hip Trauma and Slipped Capital Femoral Epiphysis.

BACKGROUND: Femoral head avascular necrosis (AVN) is a devastating complication that can occur in the setting of trauma to the pediatric hip. Bone scintigraphy (BoS) can be used to evaluate femoral head perfusion, but current evidence in support of its use in the pediatric population is lacking. The purpose of this study was to evaluate the sensitivity and specificity of BoS to diagnose femoral head AVN in children. METHODS: We retrospectively reviewed patients who underwent BoS to assess femoral head perfusion following treatment of traumatic femoral neck fracture, hip dislocation, or SCFE a

Total Hip Arthroplasty for Treatment of Pseudoarthrosis of the Hip Following Slipped Capital Femoral Epiphysis: A Case Report.

CASE: A 22-year-old man with worsening hip pain was found to have chronic complete slippage of a previously pinned slipped capital femoral epiphysis (SCFE) and subsequent pseudoarthrosis between the femur and ilium. The deformity was successfully treated with total hip replacement. We report 13 months of postoperative follow-up with adequate restoration of leg length and symptomatic relief. CONCLUSION: This rare case demonstrates the importance of careful preoperative planning to ensure appropriate alignment of rotation of the hip and restoration of leg length in the setting of a chronic SCFE

Incidence of slipped capital femoral epiphysis is associated with low childhood opportunity index.

Slipped capital femoral epiphysis (SCFE) has historically been associated with Black race, male gender, and Hispanic ethnicity, obesity, and adverse social determinants of health. The child opportunity index (COI) 2.0 is an aggregate measure by ZIP code of 29 features of communities that can impact the life outcomes of children. We aimed to investigate the relationship between the incidence of SCFE and residential ZIP code COI. We retrospectively queried the surgical database of a tertiary-referral children's hospital in a medium-sized metropolitan area. We identified subjects who underwent in

How Urgent Are Stable SCFEs? A Multisite Retrospective Study of Surgical Timing and Complications Among Patients With Stable Slipped Capital Femoral Epiphysis.

BACKGROUND: Stable slipped capital femoral epiphysis (SCFE) is often considered semi-urgent, prompting admission for in situ screw fixation (ISF), which may increase the cost/burden of care. Avascular necrosis (AVN) affects 25% to 50% of patients with unstable SCFE, yet it is uncommon after stable SCFE. Among patients presenting with stable SCFE, little is known about the relationship between diagnosis and surgical timing with regard to slip progression or complications. METHODS: This retrospective observational study included all patients younger than 18 years with stable SCFE at initial dia

The Epidemiology of Slipped Capital Femoral Epiphysis in Children and Adolescents: A Systematic Review of Risk Factors and Incidence Across Populations.

BACKGROUND: Childhood obesity is a growing global health crisis with significant health and orthopedic complications such as slipped capital femoral epiphysis (SCFE), a hip disorder characterized by the displacement of the metaphysis relative to the epiphysis. SCFE always requires surgical intervention to prevent severe outcomes such as avascular necrosis, gait abnormalities, and lifelong disability and deformity. Obesity is a well-established risk factor for SCFE; however, emerging evidence suggests that elevated leptin levels may independently contribute to the development of SCFE, regardles

Combined Surgical Dislocations and Proximal Femoral Osteotomies for Treatment of Complex Proximal Femoral Deformities: A Systematic Review.

BACKGROUND: Complex proximal femoral deformities such as slipped capital femoral epiphyses (SCFE) or residual Perthes disease pose challenging clinical problems due to the severity of pathological femoral head and neck changes from long-standing childhood disease. Utilizing the combination of surgical dislocation (SD) and proximal femoral osteotomy (PFO) is an option to address both the intra-articular pathologies as well as the maximal correction of the proximal femoral deformities. The purpose of this systematic review was to report clinical and radiographical outcomes for patients undergoin

Radiological outcome and complications after subcapital shortening osteotomy for the treatment of slipped capital femoral epiphysis- a case series.

PURPOSE: There is growing evidence that after moderate and severe slipped capital femoral epiphysis (SCFE), in-situ fixation can result in femoroacetabular impingement (FAI). Several different realignment procedures have been described but their use remains controversial due to high complication rates and technical complexity. Our study aims to evaluate the radiological outcomes and complications of patients who underwent open reduction with subcapital shortening osteotomy for moderate or severe SCFE. METHODS: Radiographic and clinical data of patients with SCFE treated with subcapital shorte
