Ninety-eight consecutive patients with 344 collapsed vertebrae underwent conventional and/or digital radiography and MRI. Vertebral collapse was due to osteopenia (16 cases), trauma (17 cases), and vertebral osteonecrosis (3 cases). Other causes were: spondylodiscitis (9 patients), primary neoplasm (4 patients), metastases (37 patients), and hemomyelopathies (7 patients); 5 patients bore vertebral angiomas. We describe a 74-year-old woman who presented with referred thigh pain associated with multiple osteonecroses of the T12, L1 and L2 vertebrae. Magnetic resonance imaging was found to be a valuable diagnostic aid, revealing signs of spinal cord and nerve root compression related to the collapsed vertebrae. Biopsy of T12 confirmed the benign nature of the vertebral compression fracture. Our patient also showed histologically documented osteoporosis.

Osteonecrosis of the hip is a well known complication in renal transplantation patients who are treated with corticosteroids for immunosuppression. In a consecutive series of 10 patients with osteonecrosis of the hip, 16 primary uncemented bipolar endoprostheses were inserted between April 1984, and February 1986. Between January and December 1989, among 396 patients receiving liver transplantation, 27 have developed 33 post-transplantation osteoarticular complications (27 non septic and 6 septic episodes). Twenty-three patients who required resection arthroplasty because of pyogenic infection of a hip joint were reviewed. Nine of the patients were ambulatory, and the others were either paraplegic or bed-ridden because of chronic neurologic disease. The average duration of symptoms was more than two months. Shortening of the femoral neck and proximal displacement of the greater trochanter are main problems of coxa vara after avascular necrosis of the capital epiphysis. In 24 patients an osteotomy to lengthen the femoral neck was performed to normalize the anatomic and functional situation. A clinical and radiological follow-up showed good or satisfactory results of the operation in 23 cases. Elimination or relief of pain and a negative Trendelenburg sign are important short-termed aims. Prevention or deceleration of degenerative changes in the hip joint are primary long-termed aims.

Since 1975, none of our patients with Perthes' disease have received any active treatment, except for managing severe pain. We have studied the functional and radiological results in order to evaluate the consequences of this approach and to demonstrate the natural history of the disease. We have followed up all patients over the age of 19 years; out of 20 only 6 were free of pain and only 2 reached 100 points on the Iowa hip score. After congenital dislocation of the hip, Perthes' disease and some other conditions, the femoral neck may be short and the greater trochanter in a relatively proximal position. Distal transfer of the greater trochanter is an effective and relatively simple operation to correct this deformity. We have reviewed 26 patients (27 hips) at a mean follow-up of eight years. Hip arthroscopic examination of 196 joints was performed in 104 patients treated during the past 4 years. Of these, 11 joints were treated by arthroscopic surgery. We have developed a technique using a two-directional approach that facilitates a global view of joint areas and allows simpler performance of surgical procedures. Magnetic resonance imaging has been increasingly used in recent years to screen patients with suspected spinal metastases. The authors present four cases, all of whom were elderly patients with vertebral compression fractures, who were diagnosed with magnetic resonance imaging to have probable neoplastic involvement. In each case, vertebral body biopsies indicated a benign pathology. The authors discuss the diagnostic criteria of magnetic resonance imaging of spinal metastases and the limitations of magnetic resonance imaging in the diagnosis of pathologic vertebral fractures.

Spontaneous osteonecrosis about the knee typically is a disease of the elderly characterized by an acute onset of pain. The exact cause of this condition has long been debated, although a causative relationship between meniscal tears and spontaneous osteonecrosis about the knee has been postulated. Intrinsic disorders that can cause shoulder pain include arthritis, gout, pseudogout and osteonecrosis. In its mildest form, impingement syndrome may cause only minimal discomfort. At its worst, impingement syndrome may lead to rotator cuff tear. Bicipital tendinitis and rupture of the biceps tendon may also be associated with impingement. Early rehabilitative intervention is important. Physical therapy is directed toward restoring range of motion and muscle strength.
